I suspect that comedians often use jokes to hide deep seated anguish or past insecurities. The self deprecating humor is closer to truth than comedy. Joan Rivers making fun of her looks may have been influenced by her past experiences growing up, and perhaps in the course of her career. Remember Robin Williams? One of the funniest fast talking comedians but fighting crippling depression his whole life, to his tragic end.
