  • Dr. Ebraheim's animated educational video describes hip fracture types and the fixation.
    This is a quick review of hip fracture types and its fixation.
    Femoral neck fracture (subcapital fracture)
    •High incidence of avascular necrosis (AVN) and nonunion.
    •High mortality rate in the elderly.
    Treatment:
    oNondisplaced: pinning by cannulated screws for the young and elderly patients.
    oDisplaced in young patients: open or closed reduction followed by pinning with cannulated screws.
    oDisplaced in elderly people: prosthesis (bipolar, unipolar or total hip).
    oIt is not advisable to do closed reduction and screw fixation in displaced subcapital fractures in the elderly.
    Intertrochanteric fracture
    Two types:
    1-Regular :
    •Reduction
    •Compression hip screw
    •Intramedullary nail (Long, short): if a long nail is used, make sure you do not perforate the anterior cortex of the distal femur.
    2-Reverse oblique: affects the lateral cortex of the femur
    Treatment:
    •Intramedullary nail
    •Blade plate
    •Fixed angle plate
    •IM rod: done in majority of cases
    •Compression hip screw: do not use. Medial displacement of the fracture due to pull from the adductor muscles. Compression hip screw is not a suitable device for reverse oblique fractures.
    Subtrochanteric fracture
    •The fracture line extends distally to the lesser trochanter
    •High-stress load
    •Common occurrence of nonunion
    •The subtrochanteric region has a hard cortical bone that does not heal very well.
    Treatment:
    •Intramedullary nail
    •Compression hip screw: not commonly used, higher chance of nonunion.
    •You may want to do this case closed in order to preserve the blood supply and the healing ability of the fracture.

    Thank you for the video. I recently had orthopedic surgery for a fractured femur/tibia/fibula, I have a screw through the hip which holds the rod im place with screws above the knee and under the knee, One intramedullary rod through the Tibia as well with screws below the knee and in the ankle to hold it in place.
    Could you please do a review on the surgery and healing process for femur/tibia/fibula intramedullary rod implantation?

    Mine was Inter-trochanteric Regular using the Compression Hip Screw. I fractured it 18 years ago. That hardware was just removed in preparation for a total hip replacement in 12 weeks. Awesome to see what actually happens when it was broken.

    My father got fracture last year june 2021 just below the heap neck and doctor performed the surgery also but after some days , a fluid starts coming out contl, and throughout tge year the problem continued, So it is infected implant case , then the rod (nail) needs to be removed out in order to control the infection. But after the nail removal surgery , the fractured bone is still not joined and now he can feel the fracture and the pain while sitting or walink by walker. So what could be the next procedure to be performed in order to make him walk again ?

    My brother fractured his hip very painful and he was walking on it for 3 days. He had surgery (new ball and socket) 5 days later (delayed) It was displaced with his right foot rotated outward some. How much of a factor is delaying it that long on weather it can be repaired with screws vs total hip replacement? He was 57 and in good shape, he fell and twisted leg on the break.

    Can someone tell me why I still cannot lay on my left side (where I had fractured my hip/ femoral about 9 months ago and I hear some clicking sounds too. And my achness(dull ache) goes to my left buttock too.

      She's just weaker, the surgery was successful and there were no complications. She just needs a completely revised diet because she can't digest many dishes and also can't have junk food. She can now walk too albeit with a walker

      @@kay9156 : Hi, she is recovering, Thankyou,, she had surgery & it was a success.. but they didn’t replace her hip in the end; they put a metal plate in and screwed the bone together, & they didn’t use general anaesthetic (they used an epidural) which made the op safer... she is back at home now and slowly recovering..

      It can take over a year for hip pins to settle down. I had surgery a year & a half ago - I feel them occasionally but am no longer in pain. It takes time!!
