No, it's all technique. Microphones amplify, compressors make things even, reverb adds depth, but the sound itself comes from the throat. If you know how to growl, it doesn't hurt at all. Anatomically, what's happening is the vocal folds open completely and you make the trachea itself vibrate, much like the way a tiger roars (not kidding, that's what happens in both cases). As long as the singer has good technique, it's basically impossible to get hurt, because nothing is touching, it's just air pressure.
