A little far fetched, but other stuff in the series leans into borderline GTA territory, so no point splitting hairs

In some stretches of New Mexico and West Texas, you can drive for well over a hundred miles and not see another car. (I've experienced this in Alaska, too). There are huge stretches of the US that are just isolated and very thinly populated. And given the nature of what Gus is doing, it wouldn't be surprising if he has drivers deliberately take less traveled routes.

Carbon monoxide molecules bind themselves to the red cells in your blood. That in turn prevents the red cells from delivering oxygen to your tissues, asphyxiating you on the blood level. Breathing through the holes would not help, since there was still a small CO intake, that kept stacking up in their cells.
The size of the holes were much too small to overcome the monoxide they were breathing in. And it wouldn't have mattered, if they did manage to open the door they would have just been gunned down.
