You thought this was just a funny haha rhythm rat game, but nope it portrays the cycle of the Toxoplasma Gondii with someone who has a self aware consciousness being infected with it making it all the more terrifying now that we can comprehend it and experience it. Start of the game: "Rat God, let me kill the human, so I can die in piece." Near the end of the game: "Rat God, you leeching parasite, get out of my head so I can die in peace!!"
Gotta love how this song is split between the chaotic flow of the virus trying to take a hold and control the Mad Rat's brain so that he will finally obey, literally all that dissonance working to fill his head with distracting and disorienting noise to shut down his will to resist, and the beat and optimistic return of "MAD RAT, ALIVE?" implying the Heart is doing his absolute damnedest to, as Mad Rat asked of him before, "give me a beat" to give him something to concentrate on and fight back.
I like how you can hear parts of "Mad Rat, Alive?" in this, it gives it that sense of chaos with how this song already is against the final boss, sort of like you were never in control of all that's happened.
Since the Rat God can manipulate time, it's almost like they're constantly rewinding the song and throwing off the beat. Can't fight back if there's no rhythm to groove to.
Mad Rat Alive in this version seems to be more of a mocking tone as reminding you that you played to "Rat God's" hand the entire time like a puppet on a string.
fun fact: you usually don't hear rats squeaking at all because they're at such a high frequency humans can't hear it those rats you hear in this song? *they're squeaking in pain*
The hardest part of this fight is not the platforming or the enemies, but your lack or rhythm. Fitting since Parasite had been messing with Rat's head the entire time, it makes sense that it could alter his sense of rhythm.
And even better is that if you listen well you can still hear a faint, constant heartbeat amidst this chaotic tune, and that's exactly the beat you need to follow in order to ensure victory. Heart is literally guiding you through this psychotic nightmare. I love subtle details like that.

It’s so cool how the difficulty of this stage doesn’t come from the obstacles or the tempo of the song - but rather, the odd time signature and the constant swapping of time signatures. Camellia is more responsible for the difficulty than the level designers.
Not really, both are equally important. The level needs to be at least decently challenging to make the song work. Play it with a normal song and the fight itself is still one of the more difficult ones due to the precision it requires at times. Two sides of the same coin.

It would be actually funny seing Mad Rat just acting all crazy jumping back and forth dodging nothing.

Camillia's inclusion in this game's ost is now a permanent reminder in my head to check rhythm games for his inclusion so i can mentally prep myself for the BS difficulty.
This song is an irrefutable banger. But I DESPISE IT. It took me months of relentless attempts and practicing of the exact same patterns in order to beat this level on hard mode with an S+ rank. This ALONE kept me from the final trophy and total completion of the game. For MONTHS. And before I even got into hard mode, this was the first stage I ever ran out of time on. I had NEVER run out of time in a legitimate run until the final boss. I am legally obligated to admit how good of a song this is. But I can't bring myself to truthfully enjoy it because of how much utter torment I feel bubbling back up every time I hear it.
