    I’ve been a paramedic for almost 20 years and am now in nursing school. I write my mental health exam on Thursday. Thank you for helping me with the MSE. I’ve seen many psychosis patients and this has honestly got to be one of the scariest conditions to have. Nice acting btw. ❤

    Having worked about 8 months on the chronic ward of a large psychiatric hospital, I could only wish psychotic patients presented with such clarity and composure as the actress in this video did. I'm not certain that anybody but the greatest of actor could convincingly portray a paranoid schizophrenic, rendering this video only marginally instructive.

      This video is just a demonstration of what might happen in an OSCE. It’s just reflective of what a student might experience when they take this exam themselves. I think the main focus of the video was to focus on what techniques were used by the Dr/nurse.

    I went in saying I was being poisoned. Turns out I was having reactions to the medications I was given and I have brain damage from chronic migraine. Basically, the medication was poisoning me, I was tasting a metallic taste due to brain damage that was never diagnosed because the doctors never did an MRI, and some of the movement of stable objects was caused by nystagmus. The doctors almost killed me with their malpractice.
    It would be nice if every interviewer was this gentle and sweet sounding. She is very comforting.

    Usually 6mins given to exam in mrcs osce... Is there anything shorter way to check mse.... Thanks in advance

      As a psych nurse I think it’s very dependent on the client. Something we talk about a lot is the value of therapeutic silence. Sometimes people with schizophrenia experience thought blocking (depicted here) and it can cause the train of thought to be lost due to voices, etc. I think on examination the reality of the matter is that in the time you have you gather the info you can. How are they dressed - appropriate for weather or scenario, is it eccentric including makeup?,are they taking care of themselves (hygiene,weight, age), what I think is important in exams is 1) SI//HI/PD 2) mood/ affect and even sleep (Ask their mood - that can including understanding what they are saying about it not just rating it (subjective) Affect is important as it can be observed based on their expressions etc( objective). Asking if they are sleeping more or less and how many hours / when do they fall asleep.Then in the time they are explaining what’s happening you’ll be able to gather thought content info to a degree. I think that’s the most important info. It sounds like a lot but like I said I think in scenarios where it’s acting based it’s important to understand that you’re being graded on therapeutic communication above collecting data so leave room for silence if you can at times even when you’re timed

    management is missing sadly😢 watched this video before, loved it, came again as was confused what to do if I see this patient in GP setting? Refer to Psychiatry OPD? but she lacks insight, so will she go there by herself? what if she had suicidal ideation?

      This is a mental state examination, not a whole review.
      However, the end of the consultation suggested they would admit her to an inpatient psychiatric ward, which is appropriate given the patient's presentation, lack of insight/judgement and risk of dehydration/malnutrition.

    Great actress, but unfortunately you won’t be able to elicit such clear symptoms so quickly in your every day clinical practice. Psych interviews are not usually this informative and you require further assessment to fully explore pathology.

    Sometimes family, "loved ones" or professionals really are trying to kill vulnerable people they have access to... speak to any survivor 2 years after achieving safety! The mental 'disorder' will have disappeared along with the perpetrators.
