Extremely well explained, haven't found a more lucid description of htn treatment anywhere else. Only one thing, why can't we use low sodium salt (25% KCl) with ARBs in patients with normal kidney function. Studies have not shown hyperkalemia in such situations.
Thank you for your kind words! I’m glad you found the explanation helpful. Regarding your question, you’re absolutely right that studies have shown low sodium salts (with 25% KCl) to be generally safe in patients with normal kidney function who are on ARBs. However , the concern arises because ARBs can reduce potassium excretion, and combined with additional potassium from dietary sources, there’s a potential, albeit small, risk of hyperkalaemia, particularly in vulnerable patients. While the risk is minimal in those with normal renal function, as always, we have to apply individual clinical judgement. On this occasion, NICE has chosen to be extra cautious and more restrictive 🙏
Thank you sir. For the past 4 yrs i have been having higher bp in clinic setting but optimum bp at home between 99 to 107 sys. And 63 to 73 dias. Doctors seem not to believe me when i told them i only have high bp at the hospital. I refused to take the medicines. I recently did my blood work. Everything was normal except pericardial fluid at right ventricle measuring 0.85cm. i am not diabetic, good BMI, i try to live a healthy life, i exercise, kidney and liver function test were okay, triglycerides normal, ldl a little bit high 4.96 mmol but i notice my hdl is low 0.92.
Thank you for your comment. I’m pleased that your BP at home is good. Any specific concerns should be discussed with your doctor, as they have your full medical records but by what you describe, it doesn’t seem that you have anything to worry about 🙏
Thank you for your comment. While NICE guidelines focus on evidence-based medical treatments, it’s always interesting to hear about alternative approaches like Sarpagandha Ghanvati. Anyone considering herbal remedies should consult with their doctor to ensure they’re safe and won’t interfere with prescribed medications. 🙏
