There is a Gatetown in Planescape’s outlands called Bedlam, which connects to the Chaotic Neutral/Evil plane of Pandemonium. I think the origin and habitat of this beast is pretty clear by the namesake. The main characteristic of both Bedlam and Pandemonium is its endless howling and whistling wind and its insane inhabitants.
it could also be named that because that is where its first recorded sighting was(like how the Spanish Flu was actually probably from Kansas in the US, Spain was just covering it up for the war effort)
As a GM who knows that anything intelligent can choose to go against its nature I'm imagining a black beast of bedlam just being the hyper weirdo hoarder recluse wizard that it previously killed and is now hiding as because "Lol this is fun why not ill stay as this its funny" because a creature of chaos is so chaotic it might bend to order and just do funny ha ha weirdness and have a really weird awkward moment with the party, just the party asking how long the wizard they knew has been dead and the beast just says "The wizard never even met you. You've only known me." and just absolutely devastate your paladin or something i dunno
It’s like finding out your bestie was secretly The Thing (John Carpenter, not Fantastic Four) the entire time, but the worst thing he’s ever done is encourage you to make really bad decisions after a few beers for the lols
This is very on brand with one NPC in my campaign my players love OOC and hate IC, Francis the Lich, who is overall just a very chaotic and chill dude. Makes magic items, but they're all cursed and he just can't figure out why. Doesn't even actually feed on souls because of the setting he's in, he's found another way to go about it, so the gods kind of just- gave up on trying to get rid of him. He's having fun. He isn't actually harming anyone. He's just a massive (helpful) pain in everyone's ass, and he wears shades for no other reason that he likes them, same reason why he's collecting small Eiffel Tower replicas.
You know, this would be great for a Horror themed dungeon crawl. Like, it starts out as an asylum, or maybe even just a hospital, but then they enter the mental ward and find a staircase to what should be the basement, but the basement looks like another level of the other floors with patient rooms, and what's more, it seems to keep going down, and down, and down. As the party goes down, things keep getting worse. At first it was some trauma victims, but as they descend deeper the patients get more insane, and then violent, but then more sickly, and then doing deeper still, there's undead in patient gowns, and then more things, shadows, then demons, all the while the floors, while still having the same layout, get more and more decrepit, and right when they think enemies are going to get even worse... nothing, the floor is empty, or maybe only a few shambling zombies. At this point its more atmospheric, more like a Call of C'thu'lu game than a D&D game, until they go even deeper still, coming across more amorphous creatures like oozes or aberrations, maybe a blood elemental, who knows. But then it finally culminates with Chaos beasts. Maybe one appeared earlier, but now there's several, along with a mix of the other creatures up to this point. And then, finally at the bottom, they encounter the Black Beast of Bedlam, maybe it's not immediately a combat and its in the human form of the (former)director of this facility, except they're now rambling madly.
The more he described the more I pictured John Carpenter's The Thing. Also the version I would run would probably require greater restoration to cure the infection, make ot a genuine threat.
Bah, amorpheous chaos monsters that turn people into flesh blobs get a bad rap I say! Done correctly, dropping one on your party is the perfect way to get them in character to say "What in the nine hells IS THAT THING!" Also don't treat wisdom as dump stat I guess.

I've been looking for reasonably powerful aberrant horrors for use in a campaign. This one is a bit mean but I'm gonna use it as inspiration for some homebrew monsters.
Well, I didn't expect a short history lesson at the start, and now have to wonder what other words are more resent than I thought were old/ancient. And the flesh warping, always the worst, and no Dark elfs in sight to blame it on!

A friend of mine suggested that I send you this, and it’s an idea I had at the spur of the moment after seeing a random miniature. Basically, it was a small dragon without wings, and it just instantly hit me, displacer dragon, a dragon that uses the same techniques that they displacer be uses to get around, you’re more than free to come up with whatever you want about it. I just thought I would send it to you and see what you thought.
OK at this point, I know at least 30 times I’ve asked for the gut fiend, parasite devil or demon. I can’t remember monster it might be in affliction. It might be an actual monster, but from what I know it was an actual monster and it’s from one of the lesser known books.
With it's inspiration of The Thing kinda obvious, I can't help but imagine a shipboard adventure with the Black Beast as a stowaway, turning crewmembers into Chaos Beasts and using it's powers to sow fear and paranoia among the party and ships officers.

Campaign idea: the party was kidnapped and dragged into the city's sprawling sewers that stretch far below the upper layers and manage to escape after beating some low level beasts and then must warn and rally the city to fight the fleshcrafer biding their time under the city by delving into the deep caverns under the city with exclusively horrific monster such as the beast in the video with its minions, flesh golems, blood elementals, nercmantic flesh walls , and other whatever horrific things you want to throw in. I feel l8ke this could be a whole campaign by itself if used right. Maybe the fleshcrafter is cursed and must use their monsters because they can't leave their lab deep under the earth or somthing.
hmm, that wisdom loss bit of the transformation goes uhhh, little scary with the sanity rules I use... cooool toooo knooooow... **note to self: don't throw at party, make them see it at a distance, so they lose sanity, but don't fight it so they probably go insane and diiiiee...**
Hey Dungeon Dad, if you haven't seen it yet, you should watch the animated series Scavenger's Reign (and its pilot short on youtube, Scavengers). Its about some humans shipwrecked on an alien planet teeming with life. Its an absolute buffet of fantastic biological sci-fi concepts that might spark some inspiration in you. Plus its a gorgeous show, fantastic plot. Seriously some of the best tv I've seen in years. The short alone is basically just a bunch of different ideas for "organic versions of stuff you'd find in an adventurers bag" in a big, mysterious, beautifully animated box.
Fun fact, the presence of this place made people of the times carefully articulate 'Bethlehem' when referring to the biblical location; this asylum was so notorious, it was a social faux pas to mumble-slur the birthplace of Jesus as 'Bedlam'. That specific articulation was reserved for the madhouse.
Funny enough when you said imagine this guy erupted into this beast it reminded me of Dark SOuls 3 giant abyssall snake things that break through random enemies form
Something very similar appears in the War of the Spider Queen books! It is described as a Nether Spirit that embodies pure chaos, with a form that is constantly changing. Although its effects are more transmutational. Getting near it might cause your arms to turn into wings, or become made of gold, or cheese. I might use the black beast of bedlam as a base and create a random effect table to go with its touch attack
I wonder if this is what inspired Arcsystemworks to create the Black Beast in their blazblue franchise. Essentially also an eldritch horror that defies reality.
Oh. Oh. I found my end boss. It was going to be a beefy terrasque but this fits so much better. Leaning into 'certain manifestations of magic function like (fantasy) radioactivity' could easily justify a genius sludge monster. There's no BBEG, but i still really want to give them a memorable final battle.
Got another gelatinous horror for you, the Teratomorph from 3rd edition monster manual 2 (p 194-195), a massive rainbow ooze that entirely lacks any cognitive capacity for being conniving, but that doesn't mean you should go anywhere near this thing. Unlike the bedlam beast being from the realm of madness, this slime monster is a manifestation of entropy; among it's terrifying arsenal its slam attack effectively has true polymorph on impact, it randomly casts planeshift on anything within its vicinity every round, and has a 1/5 change of just ignoring being hit even against spells.

Ahh, how I wish you did named creatures sometimes xD I understand why you don't but I would so kill to see a solid stat block for sigils "The Kadyx" xD and a lore breakdown. But, it's also something with very minimal sources, and even then, it's a unique experiment, not a race. Alternate really bad setting - One where the party is high level, and at point in the campaign where the party is "gentling" threats they cannot directly face by currying favor with said threat - usually by swinging some other conflict the big evil is involved in to the side of the party's "ally". Most DM's present this as a matter of helping the lesser evil defeat the greater evil in exchange for some stability and kind regard. So, your party is setting out to help the Mind-Flayer cult, or the Evil Dragon Underdark nation, deal with what it considers to be a pressing problem. They have been fighting chaos beasts for several years now, rather handily, but the fights have become more taxing as the chaos beasts seem to have further shifted into some form of life-stealing undead with promethean forms. Deep in the caverns where no scouting party has yet reached, a Black Beast of Bedlam made a very terrible mistake. It fought a lich and killed it with surprising ease. It only figured out the truth when it took the lich's skull as its new mouthpiece and immediately faced a concentrated magical attack on its chaotic mind. Yep, it's a BBoB with additional demi-Lich powers. The "Screeching Skull" attack now has a range of 30ft and a 17DC (instead of 18), but failure drops the character to 0 hit points, and success does 2d10 psychic damage and frightens everyone within range. If you want to fully merge the abilities, anyone within 60 feet, but outside 30 saves against 18DC and is frightened on failure. Call the combo - CR RocksFallEverybodyDies
DC18 for a permanent mutation is pretty wild man, also what does the title have to do with anything? Did "shifting mass of black chaos" really open up some new part of your mind? Since I don't know much about D&D myself (and I don't know if a playable race like this exists), but if say you were running some type of character that was elemental in nature or mechanical, would they be immune to the "flesh touch" since they don't really have flesh per-say to transform? Would they just become a lump of whatever they're made of (like a lava lump for fire elementals, lump of iron for a golem or something, etc.)?
I had forgotten about the origins of Bedlam as a word. But that's kind of the funny thing, isn't it? 'Bedlam' is a common phrase. People know about it. Chaos Beasts were a well-known monster: maybe not 'common', but common enough to be in the 3.5 monster manual. And the origins of both are monstrous, and were mostly-forgotten. Until now.
Okay, here me out: a fake hero. Imagine for a moment a proud, strong, handsome, charismatic man that has managed to capture a Beast of Bedlum. He lures unsuspecting travellers to its lair, lets the Beast change them into a Chaos Beast, and then sets it loose on the town. After it has caused enough destruction, our "hero" swoops in and slays the monster in front of the entire town. Most people shower our "hero" with praise. Others get suspicious and begin snooping around. Those people usually end up being turned themselves. The Beast of Bedlum, meanwhile, is swearing vengeance for its capture. It's doing everything it can to get out and to make our "hero" suffer for this indignaty.
Just because they know Common, it does not mean they speak Common. I imagine that they communicate like SCP-058 (Heart of Darkness) at best. Sure, they are intelligent, but they are chaos itself.
The party walks down a dark forest path. They have been hearing shufflingand rustling along the path for a while, but it could just be animals right? Then they hear it. "Help" Then again, the exact same intonation and tempo, like a recording being replayed. "Help" It doesn't sound stressed or fearful. "Help" Then a shape emerges from between the dark trees. The white shape of a human skull, its jaw opened almost two feet wide. Roll for initiative.
Anything from 3.5e is absolutely insane, not gonna lie. I'm a very new DM and I've tried adapting a ton of 3.5e stuff to 5e and it's not recommended. 😭 Granted, I am new like I said. If there's more experienced DMs that think different pls tell me cause I wanna learn from you wise old DMs.
Considering the etymology of "Bedlam," this creature would make a perfect nurse practitioner with it's speed, multiple appendages, high intellect and abilities allowing it to thrive in chaos. Darkness, Disguise Self and Phantasmal Killer magic would be great for problem patients!

Not entirely true. Beggars used to say they were patients, but the hospital denied they allowed inmates out to beg.

Something very similar appears in the War of the Spider Queen books! It is described as a Nether Spirit that embodies pure chaos, with a form that is constantly changing. Although its effects are more transmutational. Getting near it might cause your arms to turn into wings, or become made of gold, or cheese. I might use the black beast of bedlam as a base and create a random effect table to go with its touch attack

Oh. Oh. I found my end boss. It was going to be a beefy terrasque but this fits so much better. Leaning into 'certain manifestations of magic function like (fantasy) radioactivity' could easily justify a genius sludge monster.
There's no BBEG, but i still really want to give them a memorable final battle.

Got another gelatinous horror for you, the Teratomorph from 3rd edition monster manual 2 (p 194-195), a massive rainbow ooze that entirely lacks any cognitive capacity for being conniving, but that doesn't mean you should go anywhere near this thing. Unlike the bedlam beast being from the realm of madness, this slime monster is a manifestation of entropy; among it's terrifying arsenal its slam attack effectively has true polymorph on impact, it randomly casts planeshift on anything within its vicinity every round, and has a 1/5 change of just ignoring being hit even against spells.

Ahh, how I wish you did named creatures sometimes xD I understand why you don't but I would so kill to see a solid stat block for sigils "The Kadyx" xD and a lore breakdown. But, it's also something with very minimal sources, and even then, it's a unique experiment, not a race. But dang I would love to see a collective lore breakdown too, surprisingly few videos I can find on youtube.

Alternate really bad setting - One where the party is high level, and at point in the campaign where the party is "gentling" threats they cannot directly face by currying favor with said threat - usually by swinging some other conflict the big evil is involved in to the side of the party's "ally". Most DM's present this as a matter of helping the lesser evil defeat the greater evil in exchange for some stability and kind regard.

Yep, it's a BBoB with additional demi-Lich powers. The "Screeching Skull" attack now has a range of 30ft and a 17DC (instead of 18), but failure drops the character to 0 hit points, and success does 2d10 psychic damage and frightens everyone within range. If you want to fully merge the abilities, anyone within 60 feet, but outside 30 saves against 18DC and is frightened on failure.

Since I don't know much about D&D myself (and I don't know if a playable race like this exists), but if say you were running some type of character that was elemental in nature or mechanical, would they be immune to the "flesh touch" since they don't really have flesh per-say to transform? Would they just become a lump of whatever they're made of (like a lava lump for fire elementals, lump of iron for a golem or something, etc.)?

'Bedlam' is a common phrase. People know about it. Chaos Beasts were a well-known monster: maybe not 'common', but common enough to be in the 3.5 monster manual.
And the origins of both are monstrous, and were mostly-forgotten. Until now.
Okay, here me out: a fake hero.
Imagine for a moment a proud, strong, handsome, charismatic man that has managed to capture a Beast of Bedlum. He lures unsuspecting travellers to its lair, lets the Beast change them into a Chaos Beast, and then sets it loose on the town. After it has caused enough destruction, our "hero" swoops in and slays the monster in front of the entire town. Most people shower our "hero" with praise. Others get suspicious and begin snooping around. Those people usually end up being turned themselves. The Beast of Bedlum, meanwhile, is swearing vengeance for its capture. It's doing everything it can to get out and to make our "hero" suffer for this indignaty.
Just because they know Common, it does not mean they speak Common. I imagine that they communicate like SCP-058 (Heart of Darkness) at best. Sure, they are intelligent, but they are chaos itself.
The party walks down a dark forest path. They have been hearing shufflingand rustling along the path for a while, but it could just be animals right?
Then they hear it.

Then again, the exact same intonation and tempo, like a recording being replayed.

Anything from 3.5e is absolutely insane, not gonna lie. I'm a very new DM and I've tried adapting a ton of 3.5e stuff to 5e and it's not recommended. 😭

Considering the etymology of "Bedlam," this creature would make a perfect nurse practitioner with it's speed, multiple appendages, high intellect and abilities allowing it to thrive in chaos. Darkness, Disguise Self and Phantasmal Killer magic would be great for problem patients!
