I don't know, I think he ordered the same stuff as everyone else combined plus the extra stuff, then he ate the extras, his duplicates of everyone else's orders, and everyone else's food. I felt that the developers had him eat a positively absurd amount of food for comedic effect.

I like to give credence to the theory that Smoke was ordering for everyone.
1) Smoke is the wealthiest of the four at this stage - and would probably pay for everyone to have a fee - hence, he orders.
2) He asks Carl what he wants to eat.
3) Carl is pointing out on the menu what he wants, he's not talking to the box, hence "Carl, what do you want? I'll take a number 9, FATBOY" (I.e. Smoke).
4) Ryder is looking at the menu too, not talking to the box.
5) Sweet is odd: perhaps it's his favourite and know the number by now.
6) It looks like Smoke is the one in line with the box, so he's placing the order for everyone.
Therefore Smokes meal is actually:
1 x Number 9 Large
1 x Number 7
2 x Number 45's (one with cheese)
1 x Large Soda
So 4 'burgers', 1 large fries, 3 medium fries, 3 medium drinks and 2 large drinks - which is more realistic considering burger size, and you know, the 'average' meal for a fat person with fast food.
Also, if he was ordering for himself, it means he would have eaten 10 meals before getting back to Grove Street, which I'm not buying. It's more plausible that he ate 7 orders total (which would be tough), but is more realistic.
