It's not fan-made, it was shared by one of the producers. It's actually an unfinished mix that was scrapped when Gaga planned her version with Beyonce.

“Telephone” was originally made for Britney's album “Femme Fatale”. Fortunately she replaced it, so Gaga recorded her version and put it on “The Fame Monster” making another huge hit song!

No, it wasn't for Blackout. Fernando Garibay (the producer) and Gaga worked for Britney's 6th studio album in 2008, Gaga wrote 2 songs for her that Fernando produced along with other tracks he pitched to Britney's team, but only 2 of them made the cut, Quicksand (written by Gaga) and Amnesia. Britney still wanted to release Telephone but as a duet with Gaga in her next compilation album, in the end that didn't happen either.

it's a fanmade mix of the official version and the britney version, which is a demo version yes.
It's not fanmade, this is an official demo mix made for Britney's 2009 Singles Collection but was never finished because Gaga had already made her mix with Beyonce which was released the same year.
Actually, Gaga originally wrote the song for Britney, so this is mixed with the demo of her singing it but she rejected it in the long run. So then Gaga decided to record it with Beyonce since Britney wasn't going to use it.
Gaga wrote it for Britney along with another song called Quicksand, which was released in some versions of Circus, however Telephone didn't make the cut. Then Britney's team wanted to release with Gaga for Britney's Singles Collection and comissioned the new mix with Gaga which wasn't finished because Gaga had already planned a duet with Beyonce and that's what got released instead.
This version was uploaded by the producer of the mix as part of his online portfolio but people only found out about it a couple years ago.
