Urolift for a LARGE Median Lobe - I was wrong

  • Previously, I made a video about some of the limitations of the Urolift procedure ( • Limitations of Urolift... ). In that video, I listed a large median lobe as a limitation, however, a recent case showed me that Urolift does work for some large median lobes.
    In this video I show how I used the Urolift procedure to help a man who was too unhealthy to undergo any other procedure get out of urinary retention and urinate on his own again.

    Dr: What a rare find, a surgeon who admits he could be a bit wrong. My dad was an MD, I was a pre-med, .... Great to see and understand the finesse of the median lobe. Most layman don't know a physician has a life time of learning, things and procedures change; hopefully for the better...Great instruction Dr...I'll be thinking about doing this as the Rezum procedure didn't really change anything for me....

    Thank you for the detailed education about the UROLIFT being applied to patient with BPH with LARGE median lobe. My only regret is that you are not practicing in Jacksonville, FL.

    Urolift was a TOTAL failure for me. ZERO improvement. Now the same doctor wants to do a TURP.

    I have been reading about PAE Prostate Artery Embolization. Could a patient with a large prostate over 100gram have that procedure first to shrink the prostate and then have the Urolift performed if needed?

    Urolift completely and permanently destroyed my life !!!! Now I am faced with my new urologist wanting to do another surgery (greenlight) to correct to the botched UROLIFT procedure.
    I am told I would have to self catheterize everyday possible for the rest of my life. My symptoms now are WORSE than before Urolift. Please be advised and don't say I didn't warn you if you get this procedure !!!!!

    I had the euro lift procedure, but the doctor could not pin back the large medium low. I had the rezum procedure after that which was successful.

      I’m very happy to hear Rezum fixed your enlarged median lobe. I’m booked in for rezum and urolift. Urologist will decide which procedure to use when he’s had a look inside.

    My urologist has tried using UroLift to treat Median Lobe problems but found that anchoring the tissue to the bladder wall caused bladder irritation which he considers a significant enough problem to discontinue using UroLift for those with a Median Lobe.

      Thanks for sharing. I have an obstructive median lobe and my urologist wants to pin it to the bladder wall with Urolift. I’m very worried about bladder problems. What does urologist recommend for obstructive median lobes?

      @@jackfromthe60s He recommends one of the procedures that removes tissue, which I will not have done unless it is absolutely necessary.

      @@nickace8149 OK. Of those methods it seems Rezum has the least side effects. My personal view is not to wait until it’s absolutely necessary. But I’m not a doctor. Good luck.

    Thank you Dr for admit your errors and you're very honest in recognized it. I'm 73 years old. I live in NYC and I have severe prostrate enlargement with blockage of urine flow the urinary bladder. I'm taking medications that doesn't work. So, what is your suggestions about me? Thank you for your time and consideration regarding this matter.

      Your urologist will probably give you three options. Urolift, TURP or perhaps iTind.

    What would make a surgeon suggest a Urolift vs a Rezum procedure and could the Rezum procedure be used in the case of an enlarged middle lobe?

      My urologist is reluctant to perform Rezum on my obstructive median lobe due to possible heat damage to the bladder neck, which could result in incontinence. He wants to pin the lobe to the bladder.

    I've seen patient reviews that give Urolift a 2 out of 5. These are patients not selected by doctors or corporations to give only favorable reviews.

    I had an enlarged median lobe and the procedure was a total failure!! Painful and no relief. Urinary issues were worse. After that experience, had a TURP with preserved ejaculatory function. No sexual side effects and urinary issues fixed!!

      Wow!! You are one lucky man! Many are stuck with retrograde ejaculations permanently "especially" if the seminal vesicle's ejaculatory duct tissues have been destroyed too much in the TURP procedure and if this is the case, for many, the orgasmic's peak enjoyment can easily drop to a 2/10 instead of the normal 10/10 great orgasmic. A normal external ejaculation by far will give anyone the best orgasms. According to a group research with BPH patients in a China Hospital, the less this ejaculations duct us touched/damaged in surgery the better chances you have to have normal and pleasurable external ejaculations just like before ine's prostate problems began. Make sure to ask your Urologist if he can do TURP the ones they did in China. All the best.

    My friend is using “Amiodaron” 4 m.g. 5 days a week,
    for preventing irregular heartbeats and blood pressure.
    Is this a problem for his planned “Urolift” treatment?

    Is it true that the Eurolift procedure is more about improving flow rather than addressing frequency? I have a relatively small prostate (21.48 mm) but I feel the need to urinate quite often. Also when I lay down at nigh I feel like there is pressure on my bladder making me want to urinate. It's not really nocturia as I'm not really going to sleep. In fact the irritation is stopping me from sleeping. I've been to the urologist and all he says is that I have a slightly enlarged prostate. Would I be a good candidate for Urolift? Thanks!

      I have 28cc of prostate...am taking alfoo drug and massaging prostate everyday...rlly helping me improve symptoms

    Do you need a cat after the surgery. My urologist is trying to get me to go for the urolift but I don't do bags

      Urolift normally no cat. Small percentage might need one.

    Had this procedure done six months ago and I had some improvements in the beginning now have to go a lot more often and cannot empty all the way. Seeing my urologist next month and do not want to go through the painful procedure again I do not know what is his plan. Any suggestions?

      You need to return to your Urologist and he may want to work up and cystoscope you to see what may be causing recurrence of your symptoms. I find that sometimes you just need more clips. Some prostates can be very soft and additional areas of your prostate may "pooch" out despite placement of clips. If you have additional obstructive areas of your prostate, there are other surgical options too, but they all have some sexual side effects that you will need to consider.

      @@marinacheng3947 I am considering a PAE for my large median lobe enlargement. It is offered by interventional radiologists, not urologists, which is why they never bring up the subject. There are no sexual side effects from a PAE.

      @@JOESMITH-ic9vb Isn't Urology fun? I have a situation where I am extremely sensitive to bladder irritants. This puts me in near-retention from time to time. The last time was full retention. Bladder irritants: coffee, tea, orange juice, beer, wine, any alcohol, carbonated soft drinks, especially colas, spicy foods, chocolate, and on and on. I would risk a PAE if it might help me. Sucks to be me right now.

      I had the Urolift done a year ago and now have the same symptoms you describe. I need to get up 6-8 times per night and I can tell I am not emptying all the way. More than about an hour of continuous sleep is impossible. Going back to the same doctor (70 miles from my home) to see what to do next...Probably the TURP procedure. The Uurolift and my inability to pee at all, (and no catheter was installed before I was sent home), was the worst pain of my 79 year old life.......

    I think he means the lobe is acting like a check valve not a ball valve. I’m having a urolift tomorrow.

    When will this not work for a large median lobe? I've been diagnosed with a moderate large median lobe with mild bladder obstruction. I can pee but it's pretty weak at times. My urologist said i wasn't a candidate for Urolift and that he could not do it in the median lobe. I then found this video.

    This is done thru the urethra? What about the little holes ‘firing the clip’? I ask because I’m only 52 and have been told I need this or that steam one or lastly the TURP. I find out Thursday my prostate ‘volume’ size?!

      Look into Prostate Arterial Embolization, or PAE. Urologists won't tell you about it. It is offered by interventional radiologists.

    Yes I had the procedure 3 years ago and It was one of the WORST decision I have ever made. I regret it. I am now suffering the consequences because the #Urolift procedure has made my condition worse than it was before.

    URO Lift ;....a waste of $ 9000 as NO insurance would cop for it, after 5 years with daily intake of Flowmax it is 100% useless and ineffective, take #3 had TURP performed...!
