Depends on the model. P320 does need one, whereas the P365 XMacro doesn't, so it just ships without a spring.

My question is if I take my standard 17 round mags that came with my full size p320 and I put the magazine extensions on will it fit a x-grip

Yes, it should fit the 17 round mags with the XGrip.

It likely doesn't work, because the spring is narrower and it could bend or kink.
