- Hypoxic ischemic encephalopathy (HIE) is the most common cause of encephalopathy or brain dysfunction in the newborn and occurs usually due to hypoxia (lack of oxygen) or ischemia (lack of perfusion) as seen in asphyxia or newborn birth trauma.This video will give a description of HIE, hypothermia (or cooling) criteria and treatment, and outcomes.

Ms. Ederly...thank you so much for your kind words. And thank YOU for your amazing work with those little ones in the NICU. I'll be honest though... I'm probably one of the world's worst nurses. I don't think I would be the best as teaching skills at the bedside or preparing emergency drugs. That's not a skill I have...but what I DO want to do ...is a video with a nurse friend of mine highlighting those skills....so you and others can learn. Thank you for pointing this out.
Thank you very much for this content! My wife gave birth last week to our beautiful son, unfortunately as emergency C-Section as my wife felt that there could be something wrong with the baby in her womb (movements were less intense and less frequent). We went to the hospital during the day and they took her immediately for C-Section as they saw that there was meconium in the amniotic fluid and the vital signals of the baby were not ok.
The baby was born already with an infection and was provided from the beginning oxygen to assist his breathing. They gave him antibiotics and kept him in neonatal unit. After one day there they moved him to intensive care and were observing him. He had slightly bluish spots also his hand and feet were bluish. An ultrasound showed that liver and spleen were bigger than the norm. Enzymes of the liver were high as well. during the next days all values normalized, also the size of the organs. But his CRP levels and thrombocytes levels are not constant so they have to monitor these periodically. Also an ultrasound showed some "spots" in the brain which apparently are not normal and there they've planned an MRI to be performed in 4 days still (day 12 of life of the baby) to check what it is exactly. Doctors don't give us any diagnostic yet...they think it could be some metabolic, hematological or genetic disorder but I don't understand why. They keep asking us if there is any history in our families of genetic disorders or sudden death in any of our relatives. Our son is showing all the here described symptoms and to me is clear that his condition has been caused by the aspiration of meconium affecting the oxygen supply and causing the infections he had and possibly explaining the spots they see in the brain.
Day 8th:
The baby is still in the intensive care and the doctors are concerned of his neurological condition as he does not act and move like a "typical" newborn. We as parents have our ups and downs because doctors are not conclusive to his conditions and we still don't know what to expect 😟...I only found out about the hypothermia therapy only 2 days ago and I'm worried that the doctors didn't applied it due to negligence. We will speak to them now specifically about it. Thank you again!!
I had HIE, I was a month premature and I have a twin brother who was born an hour before me, because I was born later my mums placenta was having issues from my brother being born. I was completely silent when I was born and did not move at all. As a baby I got very ill and I needed to be in intensive care, I couldn’t walk until I was about 2 and a half years old. Weirdly, I learnt to talk very early (about 9 months). When I was 3 I showed symptoms of anxiety, I would cry and even throw up from stress when I had to go to school, the throwing up lasted until I was 10 and the crying lasted until I was 11. When I was 8 I was diagnosed with severe anxiety and bipolar disorder. Recently I’ve been experiencing health problems, the hospital thinks I have got some sort of autoimmune disease and they can see from a back X-ray that I have scoliosis. I still get very bad anxiety as a 14 year old but my school has been very helpful on making me feel more comfortable. Luckily, I do slightly shorter days at school due to my health issues. My son is 5 and had Severe Hypoxic ischemic encephalopathy from Complete Uterine rupture and complete placental abruption. ..He did the cooling and was part of the study that gave the babies an anemia med, that supposedly helps the brain heal?..for the extent of his injury he's doing ok. He is intellectually disabled, but should be WAY worse
I had HIE when I was born. Unfortunately I didn’t get therapeutic hypothermia since it was 2004. I also had post-hemorrhagic hydrocephalus due to premature birth
Fortunately I didn’t develop cerebral palsy and epilepsy although I do have some cognitive and behavioral impairments
Thank you for sharing. Yes, unfortunately laegest hypothermia studies occurred in 2005, after your birth. Amazing though that you didn't develop major deficiencies. Keep up the positive attitude

Erythropoietin is a hormone that promotes the formation of red blood cells, to treat newborns with asphyxia. Is this effective?

We're from Manila, Philippines. My grandson had hypothermia treatment a few minutes after he was born, just over a month ago. He's unable to swallow milk at the moment.
Are there other treatment methods in the US that can decrease or reverse the effects of HIE? Is stem cell therapy effective? Along with other possible treatment methods, (including erythropoietin, a hormone that promotes the formation of red blood cells, to treat newborns with asphyxia), will early treatment prevent the progression of the effects of HIE, such as cerebral palsy and many more? Thank you.
First, I'm sorry your little one and your family is living through HIE. This must be very difficult. You mention some therapies that are currently under studies, but are not standard of care. There is some potential with these in the future but not enough evidence right now to recommend in all babies. Finally, the earlier the treatment, yes, the better the outcome, but also the severity of the event that led to HIE and the clinical signs of course does determine future outcomes. My grandson will have a PEG feeding tube insertion 2 days from now. His protrombin time is low but still within normal range. The parents were asked to prepare for blood transfusion. Aside from prothrombin time, will it also help to find out the acidic level of his blood? Will it be necessary to give the infant medicines to improve clotting before PEG insertion? Will this be a risky operation?
The baby is unable to ingest milk at the moment, but was able to suck the nurse's gloved finger a few times. When milk was introduced in his mouth, it just flowed out. Will he be able to swallow milk eventually? Is there a therapy to improve his ability to ingest and swallow milk? Thank you.

What is the chance of 14 minute no O2 during birth asphyxia and when cooling is done within the 6 hr window for a full recovery? (36 wks+ 4, sluggish pupils, Apgars at birth 00& 01)

Does the term HIE only apply to pregnancy and child-birth?
If so, why not children or adults?
Yes, as a medical term, HIE is only for newborns. The physiology DOES occur in children and adults, meaning, events of low oxygen and lack of blood flow to the brain causing brain injury (think drowning as an example). However, HIE by definition occurs before or shortly after birth. Hope this helps

My third born suffered severe brain injury after induction of labour,never again will i go for induction, cesarian section will be my choice
I wanted to go for cesarian so badly, but everyone claimed "natural" is best for the baby. My own body nearly killed her and god knows what her life will be like. Cesarian is the best, I should've gone with my gut.

Is HIE 2 stage disability?
Disability is not measured by the grade...but by the specific criteria for insurance or by difficulties in certain areas medically.
@@thenicudoc my son is 9 months old and he does have occupation therapy, and they recommend him to wear special footwear, so i dont know where im stading😔 also he does stopping breathe during sleep time he doesnt sleep at night times he crying and i dont have no help from doctors 😔
