Laezel is a literal space alien to the other party members while Shadowheart is much more familiar to the rhers. In fact the ones who know what Laezel's race is all about will like her less since they're known for their cruelty. And Laezel herself doesn't help her case being continuously confrontational and saying downright evil stuff. Also she started the conflict and explicitly threatened Shadowheart's life. It's no wonder the others don't mourn her, in fact I'm surprised nobody said "good riddance"

First of all, revivify must be cast within one minute of death (in PnP at least). Second, the soul must be willing to come back and Laezel might be holding a grudge after Shart murdered her and you just watched. Third (real reason) is game design, the devs want your actions to have consequences and not be fixable with a simple spell

Did you dismiss her because the artifact automatically gets moved to the players inventory if you dismiss her.

There are certain choices in game. Companions forever dies if you do certain things you prove that you wont use them again. So Withers wont bother.
