But that same strategy would deny any item. which would mean it is only done because the cigarette is strong when it comes down to remaining players.
Yeah, think I should have finished my thought in that instance: I meant you're not typically going to shoot the next player to go, especially if they have cigarettes. If you shoot the last person to go relative to you and they have cigarettes, it's less likely to do anything because that player may just die before it ever gets back to their turn. We'll have to see how much health there will be overall, but I could be wrong about ciggies :^)

You went first. every item can be used when you go first except the healing items. I think you even once said the cigarette was a bigger advantage to the dealer. (Though I am unsure of what your reasoning was if you said it.) The second thing is though is that you are no longer trying to win a bunch of games in a row against a foe that often shoots themselves. Now you are aiming for more wins than losses against players that are presumably competent. This means the percentage outside of 2 health lethal rounds increases. thus increasing chance the item gets used. (Unless they are all 2 health rounds.)

I think it is actually better if players can gang up on a target. If all you can do is shoot the person to the right of you the variance of strategy is greatly reduced. Also it means if the first person just chooses to hold onto their item rather than shoot the second player. the second player can do outright nothing to take advantage of it. Only the fourth player who might not use their item on your either way can attack you. If any player can target anyone the first player hoarding their item risks drawing the ire of more than one player.

Here's an idea. What if when you are handcuffed, instead of waiting for a full loop they break immediately after skipping. That way the next person can strategically use the remote/reverse hoping the skipped player shoots whoever cuffed them.

The other item shown earlier, doubt it but it'd be interesting to see an item where you can directly guarantee a live or blank for a certain shell. Love bluffing and social deduction :) There is also definitely a third new item above the pack of cigs in the remote image, but it's cut off and I cannot tell what it is. I think the structure will probably be battle royale, but I think it'd be hilarious if the remaining players just ravaged a player's inventory when they are knocked out. Just taking turns at it like vultures.
due to the remote, my guess is that players will be TV-heads with their steam icons on the screen, similar to inscryption. also, i wonder if the multiplayer will lean into 'fairness' or not-the thought of 'one-shot protection' comes to mind when thinking of players who are not the first to handle the gun. if mag, inverter, and hand saw are all still in play for multiplayer, then this would be a thoughtful mechanic

Hmm here is an argument actually why I think cigarette may potentially be the best. Lets you have one item that increases health by 1 or an item that does 1 damage. Each player has 2 health. The item that deals 1 damage: you reduced 1 health from the pool of enemy players. that leaves 5 health left with you two health left. The item that raises health by 1: it is your 3 health vs the 6 health of the enemies. The crux of this is that in the main game your best way to survive was to eliminate the enemy fast. Now that there is 4 players your best chances of surviving is to be the last one standing. Thus an item that increases your chances of living I'd say would give the most value. There is other factors. But thing is at the end of the day. a magnifying glass may get you the kill on one person. but it won't do anything about the other two people. only reduce the competitors playing.
A tiny QoL improvement I would appreciate to see is a live/dud round count in one of the corners. It would help my ADHD riddled ass keep track. I don't know how many runs I've lost because I lost track of the bullet count
