I think a closer translation would probably be the slang Top and Bottom, but that still has more overt sexual overtones.
Why the pronunciation difference? → The M word has an "h" sound instead of a "k" sound in Dutch. The Japanese inherited a lot of Western words via the Dutch before England and others could trade freely with them.
