This is great timing. I had my first BPPV episode a few days ago and thankfully quickly found out about the Epley maneuver and had great success. However I’m still feeling a bit woozy getting out of bed and walking around like you said. I can’t tell if I need to be doing more Epleys or if I just need to be doing the VOR exercises.

Glad you found this at a good time! Definitely start with the VOR exercises, and don’t be afraid to recheck and perform the Epley Maneuver again if needed 👍

This is so disheartening… I had vertigo for 3 years from a MC accident, until someone told me about Epley’s maneuver. Cured instantly for 20 years!
Now, it’s back every couple months (no idea why,) it takes a week to get to PT, which resolves it until the next time…
It’s so hard to sit and do nothing for a whole week! I can barely walk and am constantly nauseated.
The only difference is after my first Epley‘s treatment, they told me to stay upright for 72 hours, which I did perfectly. Now the PT tells me that is “old advice” that doesn’t matter anymore, but it sure seems like it matters. Any input on that?
Also, when I do the Hall-Dix to see which side it’s on, it’s on neither side-when I sit up from doing it, that’s when the room’s spinning. Any recommendations? I’m a little worried to try them all.
I have 4 more days til appointment and I’m so desperate and depressed. Thank you!

Staying upright for 48 hours or so is still good advice. Not sure why it would be considered "old advice". You don't have any vertigo at all when you do the D-H test, only when you sit up? If so, is it on both sides (vertigo when you sit up)? And have you tried the Supine Roll Test yet? One thought for you: if you've always have BPPV on the same side and same canal, you might consider doing the Epley on your own for that side. Maybe you won't have to wait for the PT this time!

@ Thank you so much for your quick reply! I absolutely will remain upright for 72 hours when it’s finally fixed again.
I had slight vertigo to the right this time after the D-H, but noticed no difference between sides when I sat up straight and was dizzy.
This incident I’ve determined the opposite several times using the D-H test; first it’s my left ear, I treat it as such w Epley, wait overnight, no improvement, then it’s the right ear… it seems to change sides and as I said, last time it was just in the middle (sitting up).
I haven’t tried the Roll test yet, as I’m unsure which way to start… Uggh! When I see my PT I’m going to ask her which ear has usually been affected. Thank you again!!

Hey Luke, so I don’t have the extreme room spinning vertigo. But I do get a little off balance and the maneuvers work. Can you have BBPV with milder symptoms?

Yes, you can have BPPV with milder symptoms. You can also have some residual vestibular "hypofunction", which vestibular exercises can help remedy.

Could I add that TMJD can be another cause of vertigo. My TMJD grinding of teeth started getting so bad that my ears were hurting, started to have vertigo, neck, and head pain. I guess all those muscles are connected to the ear and can pull on them when there is an imbalance. I usually have BPPV several times a year, but now, with severe jaw clenching I have constant feeling of dizziness. I checked to make sure it wasn't a crystal out of place and it wasn't. I'm miserable. I am seeing my dentist and a tmj specialist but so far nothing has worked and my ear, head, and jaw pain and dizziness is constant.

Yes, thank you for adding that! Have you worked with a PT or massage therapist who specializes in TMJ treatments? They can do some good things to release those muscles and decrease grinding. I'm not an expert in the area, but I've seen some good results.

Yes, I am in the process now, but I must be the one difficult patient that is not responding very well to anything. I even got botox which didn't do anything and tried a prescription orthotic splint which made things worse. I am running out of options. I guess, don't sleep anymore so I don't grind???

Hi I don’t know which ear is affected because it only happens when I lean forward or tilt my head back

    • @GordonPhysicalTherapy
      @GordonPhysicalTherapy  2 หลายเดือนก่อน

Start with the Dix-Halpike test to try to diagnose which ear is involved.

@ sorry to bother you again but I have done it about 4 times and no vertigo. But if I roll completely on to my side with my head still at 45/ then it happens but both sides this happens
