The fact that Luka actually changed genders was something that really made me confused the first time i watched the anime, but then i decided to think about it and came up with something, if he had a twin that didn't grow when they were at his mother's womb, it could be possible that this d-mail only changed which Luka grew, and would actually mean that they killed the other version, a kinda fucked up theory, but i can't think of another possibility.
People forget the key thing about the D-Mail, that it can completely change things that had absolutely nothing to do with it. It makes no logical sense for Ruka buying a lottery ticket to suddenly mean they had Dk Pepper in stock at the store, yet it happened. Thus it's perfectly reasonable that something as large as a complete change in diet could create a domino effect of alterations that just so happened to make Ruka not form a Y chromosome.
