This was resolved by tech support. They had me turn the silver coupler on the right side one full turn counter-clockwise and it was enough to allow the extruder to sense the pad.

I just purchased and installed a V3 dual. My V2 FlexyDually is working fine. I had the same problem as you but with the right rear corner. When you say the silver coupler, do you mean the support that the leveling washer is screwed into?

It's the Z axis adjustment as shown in this picture: www.lulzbot.com/store/parts/helical-flexible-shaft-coupling-2-pack

    Maybe the nozzle is clogged?

Has your dual v3 had a blockage yet?

Yes and I tried to clear the blockage. I had to try to pull the head off but it twisted off so it's dead right now. AO doesn't sell spare parts and E3D makes the head but they don't sell the parts yet either so I'm back to a single extruder and I have a $500 paper weight.

You have to plug in the second extruder silly

Of-course it was plugged in. The problem was the Z axis on the right side needed adjustment.
