Their ROM would be recorded from the 90deg goniometer to their limit (slightly plantarflexed). The ROM would be a negative number e.g. -10 deg of dorsiflexion If the patient cannot get to the 90deg starting point when measuring dorsiflexion, you cannot use the base of the 5th. Hope this helps.
