He is a professor, after all, and one of the best. There's a real art to making complex information and making it understandable. One of my most important jobs as an RN was to translate what the doctor said into the patients' language.
If every other professor were half as succinct as Dr. B (or Dr. Huberman or Dr. Peterson or Joe Rogan;0), the cost of college might almost be worth the sacrifice
Thank you so much Dr Bikman, I bought and read your book and listen to most if not all your podcast, it took me only 6 months to see the reverse of pre-diabetes. My family doctor insists that diet and exercise will not work, and it is in our gene, (I am Asian 76 years old) she made me take metformin and ramipril, literary blackmailed me to take it. I took it for 7 days and I dump the meds into the garbage. I follow the 4 pillars, control carbs, prioritize protein, don't fear fat and the last one is fasting. During the last 6 months my weight from 148 pound down to 130 pound, I don't have mood swing, I have lots of energy throughout the day, and I am a happier person, and the world looks so much brighter. I didn't know I was in the fog until I am out of it. Blessings.

@@yennyjen5742 My A1C is down as well as Insulin, my blood pressure normal; my new doctor said I don't have to take metformin, or ramipril. Thank you. Due to my hyperinsulinemia and warning that I am in pre-diabetes mode, I joined the keto diet 10 months ago. While my hyperinsulinemia is still not fully cured, I lost 25 pounds and reached my A1c of 5.2. Lots of ‘goodies‘ also happened (for example, no more need for blood pressure pills!). At age 85, I‘m also visiting a gym every second day. Life is great!

Finally, straight talk that makes total sense! I wish medical practitioners would learn to speak in this common sense manner. When I asked my doctor some questions on the topic of insulin resistance, I was told I wasn't intelligent enough to understand and that I should do what I was told to do. Needless to say, he is no longer my doctor!
If they only cared enough to learn the lasted science it would be an improvement! I know it e they are incredibly busy but must are trapped in the tradition of treating the symptoms rather than looking for the root cause and actually CURING the problem. Wish I could find one off these!

i have very low body fat for a woman (about 11 percent) and am quite muscular. I find my glucose hovers between 95-120 all the time during the day. There's no way im insuline resistant- i eat only protein , fat, and a few high fiber veggies...my last insulin test was 4 so its low (before that it was 2.0). Do you think the strength training is the cause and is it harmful?? I wonder about all this and its hard to find people who address this and give reason.

@@shellygradin8635download carb manager and it will work out how many net carbs per day you can have, the heavier you are the more carbs you can have and stay in ketosis. I eat a lot per day but stick to fasting at Least 16 hours per day. There is a bread recipe on utube made with physillium husk, tastes good and has no carbs plus will keep you full and regular.
@shellygradin8635 increase your fat if you're hungry. I've failed at 20gms of carbs until I increased fat use say .. butter in coffee. Sometime I even eat a bit of bitter if I am hungry. It stops the hunger but won't give you the carbs. Hope this helps you
Eating fruit liberally kept my insulin high. Only when I completely cut it from my diet and went carnivore did I reverse my high insulin and I lost over 100lbs. I'm now 14 months strict carnivore and at 64 feel amazing with a fasted insulin of 4 and a HA1C of 5.1.
@@ctowtfI actually find my depression to be relieved the more I go with carnivore I am just not having the brain fog I used to. Also it’s eventually a way of life for people who want to get healthier and better and a way of life will give you motivation to keep going and not getting depressed. Nice video. I learned a lot. I became carnivore 6 weeks ago and my glucose was 329 at start. Now its 120 with diet alone. I'm thinking a little exercise and I will get it below 100.
Wow! In just 6 weeks? That's like a water fast as far as the effect on glucose goes. I'm very impressed. I'm transitioning to carnivore myself for the same reason. I'm ketovore already.
I have enjoyed reading your book, Why We Get Sick", and always gather knowledge from listening to you! I am 76 and always had desk jobs but recently, purposly ,took a strenuous job for four hours a day where i lift, push, pull and stay on my feet. I have noticed an ability to do this when I am eating strict Keto as opposed to getting food from carbs. I am so glad that I can keep the insulin under control and appreciate your excellent work in this area to guide us!
I’ve been low carb but always crept in enough carbs to keep me out of ketosis and fat burning so on January 1st went carnivore. Day 3 the inflammation disappeared, day 6 brain fog (KETO flu) disappeared since then have been steadily building muscle and burning fat but have not lost weight. Going for a DEXA scan the end of the month to get a more accurate BMI also gonna get a OGTT I believe the reason I haven’t been able to lose belly and man boob in the past is my fat is still extremely insulin resistant as going no carb has proved. Looking forward to getting ripped by summer at 58 years young.
Dr. Bikman, thank you for explaining so well on how to lower insulin resistance. I started a completely carnivore diet 24 days ago and already have found results in so many ways. I find it so easy and satisfying. I am looking forward to my next visit with my doctor in three months (I never thought I would say that!). I really wish she had suggested either keto or carnivore to reduce my insulin resistance and to lose weight but instead she said, I needed to eat 5 or 6 small meals every day of about 1600 kcal. to keep my metabolism revved up and get our and walk 60 minutes each day. Sounds good but constantly grazing does not work for my body. Keto worked for me but I do not like vegetables so I switched to carnivore and absolutely love it.
I am not meat eater I eat fish eggs and chicken drumsticks sometimes no carbs no processed meat I eat whole foods but my cholesterol is high what about your cholesterol blood levels?

Great presentation. Been in this space for 3 years eating animal meat and eggs. Over the last month after picking up "Why We Get Sick" and now "Insuin IQ" I've picked many metabolic health tips.
Thin fit guy that has always had borderline high blood pressure. Started intermittent fasting 18/6, reduced healthy carbs (quinoa, beans, etc...) and my blood pressure immediately dropped. I have always exercised, got 8-9 hours a sleep, monitored stress but reducing my insulin was the only thing that fixed my blood pressure. Wish I knew this info earlier in life.

My food pyramid = bacon, dark choc and coffee! On a more serious note, Ben has a way of helping me to understand why my health is what it is. I've been Type II over 20 years and with the METFORM and Glimep not working much for the past two years, my endo kept suggesting Mounjaro. After 6 months on the liquid gold, I've yet to experience the mind control benefit but I swear by using a CGM. My Dexcom\Libre is the one prescription that has actually helped me control what I eat and how much. At 6'2" and 200lbs. you would not suspect my A1C was over 9 but my endo said the GIP in mounjaro will make a big diff. I say all this because I have to say that learning from Ben has become the third leg on my bar-stool ( I mean foot-stool ) I don't drink or smoke. Compared to online classes that cost thousands, Ben's classes win HANDS-DOWN ! Ben - If you start offering one-on-one 10 minute video calls - I may have to put it on my shopping list. Hi Dr Bikman. Thanks I've watched quite a few of these and find them so interesting. I'm a retired medical doctor. Now 56 and menopausal and do lots of ex exercise. I generally have a high protein diet, with lots of and vegetables and daily fruit. I do eat meat, eggs, yoghurt, keffir and intermittent cheese. I'm lactose intolerant so I do eat pulses, nuts and seeds. I make most of my own food and don't eat processed food. I don't eat refined carbs day to day. I have 2 rows of 70% dark chocolate every day have 1 cup of 50:50 caffinated coffee in a morning and 1 cup of decaff in the afternoon. I make my own wholemeal bread, have brown rice, quinoa, freekah. I exercise daily-, stretching plus some resistance excercies ,walking on the moor uphill for 30 mjns-1 hr or interval running uphill (only 15 mins), now twice a week at the gym lifting weights and I mountain bike 1-2 a week 5 -20 miles . Long distances are on an E bike as I have long COVID. Since I've been menopausal I struggle to keep my blood sugar up . And have ended up having to walk my bike uphill. I saw an endocrinologist as everyone around me was concerned about my condition and how much I had to eat. After my own research and the advice of the endocrinologist I now have dextrose drink on the bike, dried mango or sweets, and snack bars when I'm on the bike. I did ZOE testing with a continuous blood glucose monitor in response to different foods, and my blood sugar control.is good , clearing fats not so good but by cholesterol, triglycerides hdl/ldl ratio is good and my bp/pulse are low. I had a 16 hr fast and my blood glucose was 4.4 but I felt out of it all day, struggled to concentrate on my work. On my bike I have to have banana/malt loaf before I set off. I sense I don't seem to create glucose easily but I'm fine if I have a steady supply while I ride. I'm now playful on my bike and have endurance like I did before menopause. I think the thing that I've tried and struggle to stick to with a busy life, dog and teenager is reliably eating my last meal early and therefore get to bed on time. I can get easily sleep deprived with long COVID and have to have a lie down in the day if I've gone to bed late/not been in bed for long enough. I am prone to increased fat storage around my abdomen which varies. My waist has increased by 6 inches since menopause although my BMI is fine, and 2x my waist is just under my height. Although I would dearly like it to be much less. I think that late meals and inadequate sleep may be contributing to this. I feel much better when those two things are in check. I have watched your sleep/insulin resistance episode. Do you know or can you site evidence regarding the impact of long COVID on metabolism/muscle functioning? Thanks.

I have type 2 diabetes and being the knowledge nerd I am, I have been devouring info since my diagnosis last year, and managed to lower my A1C to a pre diabetic level with diet and no meds. I still have a ways to go because I’d like to continue to reverse my insulin resistance. I find you very knowledgeable and you share info in a concise and digestible fashion. While I’ve also appreciated other keto and carnivore experts, I sometimes find their generalized recommendations and promises a little suspicious when they fail to integrate gender, age (I’m 59) or insulin resistance into real consideration. Hi. I'm in a similar situation but can't lower my A1c even though my fasting insulin and glucose is always normal. My Hdl is always above 85 (maybe too high) and my triglycerides under 60mg/dl but ldl always over 200. So good markers but my blood sugar always takes too long to come down after a meal. Also maybe stress increases the blood glucose. I urinate a lot at night. I only weigh 49kg so no weight issue. And I walk a lot. A puzzler. Doctor is pretty useless. Thinking of getting a cgm. Anyway, do you have any tips?
Dr Bickman, please help the young people suffering from long covid mitochondrial damage and odd changes in blood glucose, fat and muscle loss, etc. We are desperate for help from a mind like yours.
I am. But my case is mild compared to my 19 year old daughter and the many thousands whose bodies have been hijacked much more severely. There muscles are wasting, their minds are altered, their cortisol, insulin, glucose, thyroid, sex hormones, virtually all hormones, guts, vascular, central, peripheral, autonomic, and sympathetic nervous systems are all highly involved. Yet their tests are random, spiking oddly at odd times. I just bought her a CGM without insurance to try to track better. It is more like mitochondrial or glycogen or glucogon or insulin cell disease, like the mitochondria only receive or processes ketones or sugar or glutamine intermittently, what seems randomly. It is not just micro clots. But I truly think Dr Bickman could figure this out. These people are suffering daily, and fading fast, disabled and reaching death. My daughter is beautiful, smart, motivated. She was active, strong, healthy. Now she can hardly walk, has lost excessive weight, can't sleep half the night, utter fatigue most hours of the day, brain fog, random anxiety. Her basic bloodwork is usually normal, but at times slightly off in random odd ways, then normal. The typical doctors don't have time, lack the heart, mind, curiosity, or care, or are lost, or in denial (caused by governmental issues similar to what caused our country's sugar problem). Honestly, I am going to start begging Dr Bickman to please, please, please help. He is equal to this extreme challenge. For such a time as this....

Briefly, my Naturopathic doctor recommended low calorie and high protein and cardio exercises. The absolute opposite of what I have learned this year as to how to address health issues caused by poor metabolic systems. I have been fasting 18/6 for 6 months now and have lost 21 pounds. Feel great. Following a ketogenic nutrition plan of whole otganic natural foods. THOU SHALT NOT EAT BARCODES! Love that from you! I have subscribed to your channel today as soon as I discovered you! Metabolic rate is a key. Think of the things that boost it: fasting (first 2 days), cold therapy, low insulin diets, exercise (HIIT and movement in general). What lowers it: long term calorie restriction, inactivity, staying in the temperature confort zone, high insulin diets.

Doctor, when I am watching your videos I actually feel as though am back in my uni classroom! btw, I didn't study medicine, but you are rather captivating lecturer, especially how you explain things. I have only now started to watch several of your vids, and I find them so interesting. However, how is it that you are recommending the eating of fruits and vegetables? That is so controversial here. I was surprised when you said that, i thought you would have said to limit fruits and eat very low carb vegetables, because that is basically how I eat, without refined foods and sugars and more meat for the last 7 months and the results have been remarkable! I lost 14 kilos. I am now at the lower end of my BMI (19.5) .
Thank you Dr. Bikman!! Very few podcasters can articulate with such clarity on this complicated subject (without being overly long-winded & I appreciate your humour). Most doctors are too arrogant, lazy or misinformed and would rather push pills as the easy solution. I have always had a healthy weight, with high metabolism, low blood pressure, zero alcohol and exercised all my life focused on cardio. However my A1C in the pre-diabetic stage and therefore must be skinny-fat! So I have cut sugar, added a lot more fat & protein. My carbs are now mainly from vegetables and fruit. I started strength training to build more muscle in addition to my cardio routine. My battle with blood-sugar is still ongoing and all I can conclude is that I have been suffering from high insulin levels for years. I will have my doctor order the tests you suggested. Love all your lectures and will recommend your channel to my family and friends.
I'm getting better sleep and I am low carb except when I drink EtOH, but I am NOT giving up caffeine! That hot liquid gold brings me joy in the morning. Sucks that it triggers that kind of response, but isn't it also loaded with antioxidants? I'll hedge my bets, but just try not to drink too much. As an aside, I have watched hours upon hours of podcasts on low carb topics, carnivore, etc. Dr. Bikman, props to you as you make complex topics completely understandable and, more importantly, interesting. You and Dr. Zoe Harcombe are the best speakers I've watched on youtube by far. :)

I use CGMs occasionally to track how various foods affect my glucose. But you don't mention these in how to diagnosis/assess your Insulin Resistance. How does the CGM curve differ between people with various degrees of T2D? Does the post prandial peak get prolonged? How quick should it go back to normal? What is a baseline normal level?
Last time I checked it was 4.9.. I should check it again because my A1c was almost up to the limit..🤔 My triglycerides are now at 49..and my HDL is 68..glucose..88
Yes, half my height is my goal for my waist measurement. I have a fabric 'black belt' with a marker of that goal sewn on, and clip that I move to show my current progress towards that goal. It's another motivator. I only make progress if I am Carnivore with water, anything else I bloat and gain. I have a goal to work towards the Lion Diet, even PKD, but at the moment the variety of priming opportunities with carnivore is great for me. I was thrilled to find my BP back in the normal range this morning. Six weeks strict, though not 'perfect'.

I surely could use it as I feel I am one of the rare cases with complex fat & insulin issues that has made it impossible to lose weight. At 53 I feel like a sick 73 ( my mom’s 74 and she moves at a better pace and is fitter than I am!) I’ve been skinny before pregnancies , always eating healthy and exercising and maintained a good weight until last 15 years or so where weight just kept creeping on. Tried every possible thing I came across and cannot lose weight. Things that worked in the past stopped working. My aim was to get diabetes under control and I felt that if I do this then inflammation, obesity would be reduced. Nafld,Diabetes 2,Lipedema,High cholesterol , fibromyalgia, joint pains. On 3 months carnivore no weight loss to date, and I’m applying all lipedema self care as well but no improvement, still hopeful but not sure what to try next. I'm late in watching this, but man,..I'm so grateful. I have been struggling with sleep seasonally over many years. Recently, I was told I was insulin resistance. So, I have changed lifestyle, food and exercise. But have not been able to win lower insulin substantially. I have just completed 6 months of Ozempic. That seemed to help reduce abdomen fat, but along the my sleep was adversely affected. Now, I'm deciding to stop Ozempic. I take many supplements but my sleep is really bad at the moment. My blood work is apparently great on the 16 tests I do every few months. I am not sure there is a link between Ozempic and my poor sleep currently 🤷♀️
Thank you for this clear presentation I have insuline resistance determined by a HOMA IR ratio and have reversed a T2D since 6 years with a low carbohydrate diet and IF, going on to effortlessly lose 30 kg. My triglycerides are very low but I still consider myself as potentially insulin resistance since a high carbohydrate meal leads to hunger pangs.
I have also seen blood pressure go up since being Keto/Carnivore. Would be good discussion to see what others have done to get it down when diet fails. One woman said she took high dose niacin to control bp spikes.

I am a bit confused on one thing in todays talk. you mentioned FFA will increase with low insulin - which is a good sign and decrease with increased insulin (and this is logic) however what I understand from the literature that in the presence of insulin resistance there will also be an increase FFA ! below is paragraph from a publication that I want you to comment on it please. "Insulin suppresses lipolysis efficiently in lean insulin-sensitive humans, but less so in individuals with obesity and type 2 diabetes . This adipose insulin resistance results in excess free fatty acid (FFA) delivery to other tissues, which may contribute to ectopic fat deposition and insulin resistance (lipotoxicity) . Therefore, adipose tissue insulin resistance may be an early metabolic defect in development of whole-body insulin resistance and precede insulin resistance in skeletal muscle and liver (3).
Could you make a video explaining why blood glucose goes up when not eating anything or eating zero carbs (eggs or beef for example), when exercising (and should those spikes be avoided), and what we should do when blood glucose is “stuck” above normal range when you’re already doing carnivore. Wondering what is going on in the body in detail like you are able to explain.
I became insulin resistant when I was young and active and working out 5 days a week. In the 80's when everything the experts where telling us about nutrition was wrong.
Dr. Ben, do you do consultations? I’m struggling my insulin remains high after 8 years keto, I have done extensive fasting and lost 100+ lbs only to gain it back on carnivore. I am sensitive to protein. High fat is hard to stick with though. My insulin is 20-40 units. After a 10 day fast it only drops to 6 units. How can I fix my disease? Doctors don’t know anything about insulin.

Thank you so much for the fantastic presentation! I have a few questions, plus a request. The first question is regarding artificial sweeteners - what effect on insulin do they have and is it specific to the type of sweetener i.e. sugar alcohols vs stevia vs monk fruit vs sucrolose vs aspartame vs allulose. What is the best indication of moving the needle back into insulin sensitivity. I ask this because when I was doing keto (and fairly keto adapted) I’d find that sometimes I’d I ate a few carbs my blood glucose would remain high for much longer than I would have expected. It wasn’t a rapid drop down like I imagined. That seemed to indicate very poor insulin sensitivity. Lastly a request- can you discuss the Ray Peat ideas. I was happily doing keto a while back and came across a Dr Mercola article about why he gave up keto style eating and it all had to do with Ray Peat. It was very complicated (and honestly discouraging) and I’ve been trying to put that information into a framework that makes sense… I would love to hear a simpler explanation for whether there’s merit to his ideas and how (or if) it makes any sense metabolically. Dr. Bikman, my fasting insulin level is 12.16, blood pressure is near perfect 120/80. My glucose is 85, a1c 4.8. I weigh 295 pounds at 6' tall. Stomach measurement x2 is way over height. My triglycerides divided by HDL is 1.44. I have "low sugar episodes" frequently. Shaky, dizziness, brain fog, very hot hot flashes. Advice? Ive had the best luck with keto for losing weight. And i try to add intermittent fasting but i often get weak and tired and lethargic. Do i need to do 2 meals a day with little to no processed sugars until insulin level goes down and the try fasting longer? Thanks. Great info!
Dr Bikman is amazing! I just dont know why my glucose hovers in a high range or maybe its not high? During the day its always 95-120 range. that's fasted, between meals, after meals, etc. I eat only protein and fat and veggies. I'm very lean (low body fat for a woman)and do resistance training / exercise daily. I feel great but shouold i worry my glucose is never low like i see all the othe keto people talk about? Is it higher because of muscle mass??

Thank you so much Dr Bikman, I bought and read your book and listen to most if not all your podcast, it took me only 6 months to see the reverse of pre-diabetes. My family doctor insists that diet and exercise will not work, and it is in our gene, (I am Asian 76 years old) she made me take metformin and ramipril, literary blackmailed me to take it. I took it for 7 days and I dump the meds into the garbage. I follow the 4 pillars, control carbs, prioritize protein, don't fear fat and the last one is fasting. During the last 6 months my weight from 148 pound down to 130 pound, I don't have mood swing, I have lots of energy throughout the day, and I am a happier person, and the world looks so much brighter. I didn't know I was in the fog until I am out of it. Blessings.

Thank you. Due to my hyperinsulinemia and warning that I am in pre-diabetes mode, I joined the keto diet 10 months ago. While my hyperinsulinemia is still not fully cured, I lost 25 pounds and reached my A1c of 5.2. Lots of ‘goodies‘ also happened (for example, no more need for blood pressure pills!). At age 85, I‘m also visiting a gym every second day. Life is great!

Finally, straight talk that makes total sense! I wish medical practitioners would learn to speak in this common sense manner. When I asked my doctor some questions on the topic of insulin resistance, I was told I wasn't intelligent enough to understand and that I should do what I was told to do. Needless to say, he is no longer my doctor!
If they only cared enough to learn the lasted science it would be an improvement! I know it e they are incredibly busy but must are trapped in the tradition of treating the symptoms rather than looking for the root cause and actually CURING the problem. Wish I could find one off these!

i have very low body fat for a woman (about 11 percent) and am quite muscular. I find my glucose hovers between 95-120 all the time during the day. There's no way im insuline resistant- i eat only protein , fat, and a few high fiber veggies...my last insulin test was 4 so its low (before that it was 2.0). Do you think the strength training is the cause and is it harmful?? I wonder about all this and its hard to find people who address this and give reason.

Least 16 hours per day. There is a bread recipe on utube made with physillium husk, tastes good and has no carbs plus will keep you full and regular.
@shellygradin8635 increase your fat if you're hungry. I've failed at 20gms of carbs until I increased fat use say .. butter in coffee. Sometime I even eat a bit of bitter if I am hungry. It stops the hunger but won't give you the carbs. Hope this helps you

Eating fruit liberally kept my insulin high. Only when I completely cut it from my diet and went carnivore did I reverse my high insulin and I lost over 100lbs. I'm now 14 months strict carnivore and at 64 feel amazing with a fasted insulin of 4 and a HA1C of 5.1.

Nice video. I learned a lot. I became carnivore 6 weeks ago and my glucose was 329 at start. Now its 120 with diet alone. I'm thinking a little exercise and I will get it below 100.

I have enjoyed reading your book, Why We Get Sick", and always gather knowledge from listening to you! I am 76 and always had desk jobs but recently, purposly ,took a strenuous job for four hours a day where i lift, push, pull and stay on my feet. I have noticed an ability to do this when I am eating strict Keto as opposed to getting food from carbs. I am so glad that I can keep the insulin under control and appreciate your excellent work in this area to guide us!
I’ve been low carb but always crept in enough carbs to keep me out of ketosis and fat burning so on January 1st went carnivore. Day 3 the inflammation disappeared, day 6 brain fog (KETO flu) disappeared since then have been steadily building muscle and burning fat but have not lost weight. Going for a DEXA scan the end of the month to get a more accurate BMI also gonna get a OGTT I believe the reason I haven’t been able to lose belly and man boob in the past is my fat is still extremely insulin resistant as going no carb has proved. Looking forward to getting ripped by summer at 58 years young.

Dr. Bikman, thank you for explaining so well on how to lower insulin resistance. I started a completely carnivore diet 24 days ago and already have found results in so many ways. I find it so easy and satisfying. I am looking forward to my next visit with my doctor in three months (I never thought I would say that!). I really wish she had suggested either keto or carnivore to reduce my insulin resistance and to lose weight but instead she said, I needed to eat 5 or 6 small meals every day of about 1600 kcal. to keep my metabolism revved up and get our and walk 60 minutes each day. Sounds good but constantly grazing does not work for my body. Keto worked for me but I do not like vegetables so I switched to carnivore and absolutely love it.

Great presentation. Been in this space for 3 years eating animal meat and eggs. Over the last month after picking up "Why We Get Sick" and now "Insuin IQ" I've picked many metabolic health tips.
Thin fit guy that has always had borderline high blood pressure. Started intermittent fasting 18/6, reduced healthy carbs (quinoa, beans, etc...) and my blood pressure immediately dropped. I have always exercised, got 8-9 hours a sleep, monitored stress but reducing my insulin was the only thing that fixed my blood pressure. Wish I knew this info earlier in life.
I have to say that everything this man says is just phenomenal!

My food pyramid = bacon, dark choc and coffee! On a more serious note, Ben has a way of helping me to understand why my health is what it is. I've been Type II over 20 years and with the METFORM and Glimep not working much for the past two years, my endo kept suggesting Mounjaro. After 6 months on the liquid gold, I've yet to experience the mind control benefit but I swear by using a CGM. My Dexcom\Libre is the one prescription that has actually helped me control what I eat and how much. At 6'2" and 200lbs. you would not suspect my A1C was over 9 but my endo said the GIP in mounjaro will make a big diff. I say all this because I have to say that learning from Ben has become the third leg on my bar-stool ( I mean foot-stool ) I don't drink or smoke. Compared to online classes that cost thousands, Ben's classes win HANDS-DOWN ! Ben - If you start offering one-on-one 10 minute video calls - I may have to put it on my shopping list. Hi Dr Bikman. Thanks I've watched quite a few of these and find them so interesting. I'm a retired medical doctor. Now 56 and menopausal and do lots of ex exercise. I generally have a high protein diet, with lots of and vegetables and daily fruit. I do eat meat, eggs, yoghurt, keffir and intermittent cheese. I'm lactose intolerant so I do eat pulses, nuts and seeds. I make most of my own food and don't eat processed food. I don't eat refined carbs day to day. I have 2 rows of 70% dark chocolate every day have 1 cup of 50:50 caffinated coffee in a morning and 1 cup of decaff in the afternoon. I make my own wholemeal bread, have brown rice, quinoa, freekah. I exercise daily-, stretching plus some resistance excercies ,walking on the moor uphill for 30 mjns-1 hr or interval running uphill (only 15 mins), now twice a week at the gym lifting weights and I mountain bike 1-2 a week 5 -20 miles . Long distances are on an E bike as I have long COVID. Since I've been menopausal I struggle to keep my blood sugar up . And have ended up having to walk my bike uphill. I saw an endocrinologist as everyone around me was concerned about my condition and how much I had to eat. After my own research and the advice of the endocrinologist I now have dextrose drink on the bike, dried mango or sweets, and snack bars when I'm on the bike. I did ZOE testing with a continuous blood glucose monitor in response to different foods, and my blood sugar control.is good , clearing fats not so good but by cholesterol, triglycerides hdl/ldl ratio is good and my bp/pulse are low. I had a 16 hr fast and my blood glucose was 4.4 but I felt out of it all day, struggled to concentrate on my work. On my bike I have to have banana/malt loaf before I set off. I sense I don't seem to create glucose easily but I'm fine if I have a steady supply while I ride. I'm now playful on my bike and have endurance like I did before menopause. I think the thing that I've tried and struggle to stick to with a busy life, dog and teenager is reliably eating my last meal early and therefore get to bed on time. I can get easily sleep deprived with long COVID and have to have a lie down in the day if I've gone to bed late/not been in bed for long enough. I am prone to increased fat storage around my abdomen which varies. My waist has increased by 6 inches since menopause although my BMI is fine, and 2x my waist is just under my height. Although I would dearly like it to be much less. I think that late meals and inadequate sleep may be contributing to this. I feel much better when those two things are in check. I have watched your sleep/insulin resistance episode.

I have type 2 diabetes and being the knowledge nerd I am, I have been devouring info since my diagnosis last year, and managed to lower my A1C to a pre diabetic level with diet and no meds. I still have a ways to go because I’d like to continue to reverse my insulin resistance. I find you very knowledgeable and you share info in a concise and digestible fashion. While I’ve also appreciated other keto and carnivore experts, I sometimes find their generalized recommendations and promises a little suspicious when they fail to integrate gender, age (I’m 59) or insulin resistance into real consideration. Hi. I'm in a similar situation but can't lower my A1c even though my fasting insulin and glucose is always normal.
My Hdl is always above 85 (maybe too high) and my triglycerides under 60mg/dl but ldl always over 200.
So good markers but my blood sugar always takes too long to come down after a meal.
Also maybe stress increases the blood glucose.
I urinate a lot at night.
I only weigh 49kg so no weight issue. And I walk a lot.
A puzzler. Doctor is pretty useless.

Dr Bickman, please help the young people suffering from long covid mitochondrial damage and odd changes in blood glucose, fat and muscle loss, etc. We are desperate for help from a mind like yours.
Why not just try a ketogenic diet and see if it helps ?
I am. But my case is mild compared to my 19 year old daughter and the many thousands whose bodies have been hijacked much more severely. There muscles are wasting, their minds are altered, their cortisol, insulin, glucose, thyroid, sex hormones, virtually all hormones, guts, vascular, central, peripheral, autonomic, and sympathetic nervous systems are all highly involved. Yet their tests are random, spiking oddly at odd times. I just bought her a CGM without insurance to try to track better. It is more like mitochondrial or glycogen or glucogon or insulin cell disease, like the mitochondria only receive or processes ketones or sugar or glutamine intermittently, what seems randomly. It is not just micro clots. But I truly think Dr Bickman could figure this out. These people are suffering daily, and fading fast, disabled and reaching death. My daughter is beautiful, smart, motivated. She was active, strong, healthy. Now she can hardly walk, has lost excessive weight, can't sleep half the night, utter fatigue most hours of the day, brain fog, random anxiety. Her basic bloodwork is usually normal, but at times slightly off in random odd ways, then normal. The typical doctors don't have time, lack the heart, mind, curiosity, or care, or are lost, or in denial (caused by governmental issues similar to what caused our country's sugar problem). Honestly, I am going to start begging Dr Bickman to please, please, please help. He is equal to this extreme challenge. For such a time as this....

Briefly, my Naturopathic doctor recommended low calorie and high protein and cardio exercises. The absolute opposite of what I have learned this year as to how to address health issues caused by poor metabolic systems. I have been fasting 18/6 for 6 months now and have lost 21 pounds. Feel great. Following a ketogenic nutrition plan of whole otganic natural foods.

Metabolic rate is a key. Think of the things that boost it: fasting (first 2 days), cold therapy, low insulin diets, exercise (HIIT and movement in general). What lowers it: long term calorie restriction, inactivity, staying in the temperature confort zone, high insulin diets.

Doctor, when I am watching your videos I actually feel as though am back in my uni classroom! btw, I didn't study medicine, but you are rather captivating lecturer, especially how you explain things. I have only now started to watch several of your vids, and I find them so interesting. However, how is it that you are recommending the eating of fruits and vegetables? That is so controversial here. I was surprised when you said that, i thought you would have said to limit fruits and eat very low carb vegetables, because that is basically how I eat, without refined foods and sugars and more meat for the last 7 months and the results have been remarkable! I lost 14 kilos. I am now at the lower end of my BMI (19.5) .
Thank you Dr. Bikman!! Very few podcasters can articulate with such clarity on this complicated subject (without being overly long-winded & I appreciate your humour). Most doctors are too arrogant, lazy or misinformed and would rather push pills as the easy solution. I have always had a healthy weight, with high metabolism, low blood pressure, zero alcohol and exercised all my life focused on cardio. However my A1C in the pre-diabetic stage and therefore must be skinny-fat! So I have cut sugar, added a lot more fat & protein. My carbs are now mainly from vegetables and fruit. I started strength training to build more muscle in addition to my cardio routine. My battle with blood-sugar is still ongoing and all I can conclude is that I have been suffering from high insulin levels for years. I will have my doctor order the tests you suggested. Love all your lectures and will recommend your channel to my family and friends.

I'm getting better sleep and I am low carb except when I drink EtOH, but I am NOT giving up caffeine! That hot liquid gold brings me joy in the morning. Sucks that it triggers that kind of response, but isn't it also loaded with antioxidants? I'll hedge my bets, but just try not to drink too much. Thank you Doc Ben. I also love your Instragram posts on all the papers you read and share. I have been preT2D, better now a low carb and IF. I use CGMs occasionally to track how various foods affect my glucose. But you don't mention these in how to diagnosis/assess your Insulin Resistance. How does the CGM curve differ between people with various degrees of T2D? Does the post prandial peak get prolonged? How quick should it go back to normal? What is a baseline normal level?
PS My TG/HDL ratio is 1.55. HbA1C has dropped from 48 to 39
So important to get these tests and see if we can resolve insulin resistance!

Yes, half my height is my goal for my waist measurement. I have a fabric 'black belt' with a marker of that goal sewn on, and clip that I move to show my current progress towards that goal. It's another motivator. I only make progress if I am Carnivore with water, anything else I bloat and gain. I have a goal to work towards the Lion Diet, even PKD, but at the moment the variety of priming opportunities with carnivore is great for me. I was thrilled to find my BP back in the normal range this morning. Six weeks strict, though not 'perfect'.

I surely could use it as I feel I am one of the rare cases with complex fat & insulin issues that has made it impossible to lose weight. At 53 I feel like a sick 73 ( my mom’s 74 and she moves at a better pace and is fitter than I am!) I’ve been skinny before pregnancies , always eating healthy and exercising and maintained a good weight until last 15 years or so where weight just kept creeping on. Tried every possible thing I came across and cannot lose weight. Things that worked in the past stopped working. My aim was to get diabetes under control and I felt that if I do this then inflammation, obesity would be reduced.
Nafld,Diabetes 2,Lipedema,High cholesterol , fibromyalgia, joint pains. On 3 months carnivore no weight loss to date, and I’m applying all lipedema self care as well but no improvement, still hopeful but not sure what to try next. I feel if anyone can help me it would be you Dr Bikman 😊🌈🙌🏽

I have been struggling with sleep seasonally over many years. Recently, I was told I was insulin resistance. So, I have changed lifestyle, food and exercise. But have not been able to win lower insulin substantially. I have just completed 6 months of Ozempic. That seemed to help reduce abdomen fat, but along the my sleep was adversely affected. Now, I'm deciding to stop Ozempic. I take many supplements but my sleep is really bad at the moment. My blood work is apparently great on the 16 tests I do every few months.
I am not sure there is a link between Ozempic and my poor sleep currently 🤷♀️

I have insuline resistance determined by a HOMA IR ratio and have reversed a T2D since 6 years with a low carbohydrate diet and IF, going on to effortlessly lose 30 kg.
My triglycerides are very low but I still consider myself as potentially insulin resistance since a high carbohydrate meal leads to hunger pangs.

I have also seen blood pressure go up since being Keto/Carnivore. Would be good discussion to see what others have done to get it down when diet fails. One woman said she took high dose niacin to control bp spikes.
Thank you Dr Ben for you efforts, I am a physician and learning a lot from your presentations. I am a bit confused on one thing in todays talk. you mentioned FFA will increase with low insulin - which is a good sign and decrease with increased insulin (and this is logic) however what I understand from the literature that in the presence of insulin resistance there will also be an increase FFA !
below is paragraph from a publication that I want you to comment on it please.
"Insulin suppresses lipolysis efficiently in lean insulin-sensitive humans, but less so in individuals with obesity and type 2 diabetes . This adipose insulin resistance results in excess free fatty acid (FFA) delivery to other tissues, which may contribute to ectopic fat deposition and insulin resistance (lipotoxicity) . Therefore, adipose tissue insulin resistance may be an early metabolic defect in development of whole-body insulin resistance and precede insulin resistance in skeletal muscle and liver (3).
Could you make a video explaining why blood glucose goes up when not eating anything or eating zero carbs (eggs or beef for example), when exercising (and should those spikes be avoided), and what we should do when blood glucose is “stuck” above normal range when you’re already doing carnivore. Wondering what is going on in the body in detail like you are able to explain.
I became insulin resistant when I was young and active and working out 5 days a week. In the 80's when everything the experts where telling us about nutrition was wrong.

Dr. Ben, do you do consultations? I’m struggling my insulin remains high after 8 years keto, I have done extensive fasting and lost 100+ lbs only to gain it back on carnivore. I am sensitive to protein. High fat is hard to stick with though. My insulin is 20-40 units. After a 10 day fast it only drops to 6 units. How can I fix my disease? Doctors don’t know anything about insulin.

I have a few questions, plus a request.
The first question is regarding artificial sweeteners - what effect on insulin do they have and is it specific to the type of sweetener i.e. sugar alcohols vs stevia vs monk fruit vs sucrolose vs aspartame vs allulose.
What is the best indication of moving the needle back into insulin sensitivity. I ask this because when I was doing keto (and fairly keto adapted) I’d find that sometimes I’d I ate a few carbs my blood glucose would remain high for much longer than I would have expected. It wasn’t a rapid drop down like I imagined. That seemed to indicate very poor insulin sensitivity.
Lastly a request- can you discuss the Ray Peat ideas. I was happily doing keto a while back and came across a Dr Mercola article about why he gave up keto style eating and it all had to do with Ray Peat. It was very complicated (and honestly discouraging) and I’ve been trying to put that information into a framework that makes sense… I would love to hear a simpler explanation for whether there’s merit to his ideas and how (or if) it makes any sense metabolically.

Dr. Bikman, my fasting insulin level is 12.16, blood pressure is near perfect 120/80. My glucose is 85, a1c 4.8. I weigh 295 pounds at 6' tall. Stomach measurement x2 is way over height.
My triglycerides divided by HDL is 1.44.
I have "low sugar episodes" frequently. Shaky, dizziness, brain fog, very hot hot flashes.
Advice?
Ive had the best luck with keto for losing weight. And i try to add intermittent fasting but i often get weak and tired and lethargic.
Do i need to do 2 meals a day with little to no processed sugars until insulin level goes down and the try fasting longer?

Dr Bikman is amazing! I just dont know why my glucose hovers in a high range or maybe its not high? During the day its always 95-120 range. that's fasted, between meals, after meals, etc. I eat only protein and fat and veggies. I'm very lean (low body fat for a woman)and do resistance training / exercise daily. I feel great but shouold i worry my glucose is never low like i see all the othe keto people talk about? Is it higher because of muscle mass??
