Fun fact: in Japan you have to handwrite your résumés in pen-- so if you mess up even one stroke you have to start from the very beginning again and have to physically write multiple copies if you apply for multiple jobs.... now consider the fact that the job market is very tight in japan, so this guy had to handwrite anywhere from 20-40 applications perfectly with this long-ass name.

「院」とか「殿」とかつくと戒名みたい……と思いながら見てたら、最後の３文字でのけぞりました。
おそらく「入道」までが戒名で、最後の4文字は俗名でしょう。
戒名が付いているので「△□○」は諱ではなく仮名と思われますが、それにしても変わった字ですね。
何となく戦国時代以前の名前っぽいですが、源平両方の字が入った名前にも複雑な背景がありそうです。
最後の3文字がなかなか来なくてのけぞりました。

The person's family name actually is the first 3 character name... the one that's 14 characters long is actually their personal, given name. In Japanese, the family name comes before the given name. For the sake of illustration, pretending I was Japanese and not a foreigner, my name would actually be ordered as Howard Kat with my family name coming before my given name-- but in English, we put our given name first followed by the family/surname.
