I had a friend with the last name of "Newman". I always greeted him with a disgusted look and said "Hello, Newman". He would respond just like Newman did to Jerry. We did that for many years. I don't think Newman hated Jerry at all, otherwise he'd have surely responded with 'Hello, Seinfeld', in an similarly distainful manner. Yet Newman always called him 'Jerry', and mostly quite cordially - in fact Newman's 'Hello, Jerry' are much more varied and nuanced that Jerry's 'greetings' - indicating that it was just depended on what mood Newman was in then. I think the real subtlety of the joke is that Newman is somewhat used to people taking an automatic dislike to him (and why not, he is portrayed as an odious man), but that he's learned to live with it; he never complains about it, or even try to ingratiate himself to Jerry, he is just who he is. That actually shows strength of character; afterall, it's really not anyone's business to concern one's self with what other people think about you... that's their business.

Sometimes, you have a friend that you really love and is great to hang out with. But then that friend has another friend that you just can't fucking stand, and the feeling is mutual. But you keep the peace, even though you hate each others guts, because you don't want to risk your mutual friend having to pick one and choose the other guy. And that's Jerry and Newmans relationship in a nutshell.
Several years ago we went out on my brother's birthday. He'd been teasing our little niece a lot, and she was perpetually irritated w/ him, so when she saw him at the restaurant, she greeted him w/ "Hello...birthday boy" in the same tone Jerry used. Cracked us all up!

If it's happening to others, they just think it's funny or, at best, aren't bothered by it at all. Only very rarely do they actually try to help. That's part of what made this show funny.
