I never have those issues with my household iron and I've done decent amount of personal htv projects. I will say though that I've heard that you should use an iron with holes just around the side (which is all I've ever had) and not through the full face of the iron (which is what you used). Because the more holes make both the pressure and temperature more uneven
HTV and Iron-On are the same product :) HTV is the universal term, Iron-On is the term that Cricut adopted to call their brand of HTV. Great question!!

Like adhesive vinyl? That is not the same as HTV and you definitely should not apply heat to that
Using a House iron doesn’t work as good as a heat press but it will work just not as well as a heat press
