The thing in the beginning is not a knife, it’s a harmonica that belonged to the girl who died. So basically he was given 2 choices, he kills the girl or the children that she teaches music will be killed. He chose to kill her but he didn’t actually shoot her because he couldn’t bring himself to do it. The girl then knowing that he can’t shoot her, gets the gun and shoots herself. Also, the girl is mute.
