      most urologists don't have an idea about it either. Now most professionals are realizing that it's a muscular skeletal condition, not urological.

      @@BS-uk2jr the problem is finding out WHAT is causing it first. CPPS/Chronic Prostatitis is like cancer in that it is not just ONE disease or condition causing the same symptoms. It's a grab bag term used to cover the symptoms, and the cause can be multi faceted. For some it truly is bacterial that has never been caught, but for most people it is trauma. Basically your pelvic floor going into hyper tension because you had a back injury, a spinal problem, or your pelvis is twisted and rotating causing stress down there. Many many people get much more help with a GOOD physical therapist who will work with you and find out what is causing the tension in your body, and take care of that, thusly eliminating the need for your pelvic floor to go into hyper tension.
      I spent 7 years not knowing WHAT caused it, until I re-injured a 10 year old back injury and suddenly I got a MASSIVE spike in my CPPS. This clued my physical therapist into the fact that it was muscular-skeletal in nature and with some guess/test/revise methods we worked on lowering problems from my back injury, and worked on pelvic floor release excercises that worked for me and it's been reducing as my back injury goes down. The trouble with each individual person is finding out WHAT is causing your pelvis to feel like it needs to go haywire.

      Some people like u get better with exercise and other get better with only supplements.
      Some people get better by cut down sugar bread rice meets.
      How I can know? How I fix my problem
      Most days the symptom decreases with no reason and back with no reason

    Thanks for the video. I went through this last year and now having issues again as of now. My symptoms are exactly what you described on the chronic non bacterial. Sitting is almost impossible. I see a urologist next week. My family doctor gave me some antibiotics. It seemed to help for the first 7 days and then the perinium started hurting again as well urination. Last year my urologist had be to a CT scan and put a scope through my urethra to the bladder since the found red blood cells in my urine under scope. However, both CT scan and scope showed nothing of concern...which is a good thing too. Maybe I will order a doughnut so I can sit while at work.

    Dr. Eric thank you v very much. I have been scouring the U tube to find information about my condition of chronic non bacterial P which is exactly the same as you have described. Unfortunately, I have taken a course of antibiotics for one month, the pain has not gone away but my PSA levels came down. Since the pain is there, I have to try the treatment you thankfully mentioned in your clip. I am grateful to you and may ALLAH bless you

    I'm getting worked up for this. My urologist called it a dx by exclusion so getting an ultrasound on my kidneys, bladder, testicles, etc. Had multiple negative urinalysis in past month for everything and normal blood work including PSA. I live in Florida but my symptoms started on my last day of a snowboard vacation at altitude in Colorado. Maybe the 4 days straight of snowboarding and my weak back and core muscles led to a pelvic floor dysfuction. Who knows. Just very odd how this all started at 10,000 ft of altitude on the 6th day after 4 straight days of snowboarding which I only do once a year. Great video

    Thanks for making this video Dr. If you would take the time to answer my questions I would would be grateful 🙏. Here is my case history. Over the past 2 years my psa had been rising every six months average of increase .7 when my psa went from 2.2 to 3.5 I had my first mri which was deemed a level 2 and radiologist saw T2 weighted image looked consistent with chronic prostatitis. Then about a year later my psa went from 3.5 to 4.8 and had another prostate mri which showed in the radiologist point of view again T2 image consistent with chronic prostatitis. Plus a nodule that grew a little so now I was deemed category 3 and went for a biopsy which was negative however on one out of 16 biopsy they saw 3 atypical HGPIN cells no big deal. 6 months later my PSA was unchanged. Fast foward to last week my psa was 5.6 however I believe it was contaminated because I ejaculated within 12 hours of the test. I am going to retake the test in 2 weeks with no ejaculation or exercise 1 week before test. I don't really have any symptoms of chronic prostatitis however my frequency has increased a little. My question for you is can I safely say I have chronic prostatitis because of the mri findings which is responsible for my increased psa and if I ejaculated about 12 hours before my last psa if you had to guess ballpark how much would my psa have risen because of that?. Is a weighted T2 image reliable for chronic prostatitis on mri. I am not really worried as the biopsy last July 2021 was negative. I did order that herbal supplement you suggested on another video. Thanks a million Doc.

    I have this since 2013. Im 28 now. First 4 years was hell. I improved a lot. What helps most is managing my anxiety and avoiding inflammation causing things, like not much sleep, overtraining, overeating and using substances or alcohol... My cause is mycoplasma/ureaplasma. Took many antibiotics but it keeps coming back on and off.

      I’m 28 as well had this problem since I was 18. However in my situation you add that I leak semen all day not a fountain but small amounts throughout the day. Underwear would alway be wet and sticky.

    After living with it for over 8 years, I can submit you hit all the important aspects. I've watched every video
    On TH-cam and have seen at least a half dozen urologists. One performed a cystiscope like you mentioned.
    Eventually, through my own research, I have a way to manage it. Tamulosin, Prostagutt, and no sugar or
    Alcohol.. Lastly, lots of vigorous exercise. Thanks for your presentation and analysis

      @@scoot0boy like every thing which surrounds health , higher heart rate.. sweat.. taxing the muscles allows fluid exchange and blood flow to every cell in your body. One of the issues of enlarged prostate, not being able to complete evacuation of the bladder. ** Have you ever had water issues in your house and not flushed the toilet? Build up of rancid urine is the result. By exercising sweating..drinking massive fluids , you are cycling more urine in and out of your bladder. Plus , getting blood flow to the prostate. It's a difficult organ to get blood flow.
      Also, inform your partner you will be getting it on with her twice per week. Don't eat process sugar.. keep all sweets at a minimum.
      Watermelon is the worst. Salads ..meat and fish. Tamulosin and Prostagutt.. Follow my guidelines, and live long and prosper.
      Just Say No to any procedures in the hospital

    I had all the classic symptoms of CPPS. Painful urination, painful ejaculation (when I could get an erection), Penile pain and frequent urination. After suffering with this for over 4 months one morning during one of my very painful urinating sessions I passed a 6.25mm kidney stone! And just like that all symptoms vanished!

      How did you not get an ultrasound or some test that can detect the kidney stone? Ofc, my guess your doc was one of those who doesn't know how to use common sense?

    Awesome video. Very informative and spot on re: CPPS/Chronic Prostatis. Another HUGEEEEE symptom you didn’t include is lower back pain. I suffer with the lower back pain, urethra, and pressure in the testicles/groin. It’s been a little over a month and a half, I’m 26, and yes …. Antibiotics will not touch chronic prostatis/CPPS …. namely Levofloxacin or Doxy. It’s definitely going to be a waiting game.

      @@sleepyyheadd if there's no bacteria found, Go See a pelvic floor physical therapist. Pills are not going to fix your problem. Start doing pelvic floor stretching for men here on TH-cam while you wait for your appointments. But a Pelvic Therapist will be able to find your problem if it's muscular. Good luck 👍

      Did you get it fixed ? My symptoms were hot/needle-like pain/sharp/itching of the urethra. No urine or blood came out with anything, so mine might be non-prostatitis. A guy on youtube commented saying he did pelic stretching which cured him. What were your urethra pains like ?

    Hi Doc, I was testosterone therapy for my low number until my PSA came back at 2.91. So my doctor took me off of it because he said if there is cancer in the prostate testosterone would feed it. So I came back in 3 months and it was down to 1.93. In October of 2022 I caught a UTI where they found E. coli in my urine. In March of 2023 my PSA came back at 3.41. And just last week in September 2023 my PSA came back at 2.61. Now the urologist wants to do a biopsy. I have many of the symptoms you mentioned. Should they be ruling out everything else first before just jumping to a biopsy? Thanks! I’m 54 year old African American male.

    When taking tamsulosin for cpps , is that Rx short term ? Meaning 30 -60 days ?
    This started out as frequent peeing constipation. And 2 bladder infections. With chronic Epididymitis that I’ve had for probably five years.

    Hi doc and greetings from sweden! I wish I had a doctor like you over here.. struggling with bacterial prostatitis for 5 years now. I keep getting infections and the doctors give me antibiotics. Ive done xray, ultra sound, cystoscopy, and the one thing that is ”wrong ” is that i have a cyst (?), a pea sized ball in my prostate. I want to remove it, since im not getting well, but the doctors say that its only water, and that if we would remove it it Will fill up with water again. The bacteria is almost always e-coli. Between the prostatitis ive had several bitesticle inflammations, with e coli bacterias. I had to do surgery one month ago because it was so infected and my ballsack got filled with some fluid. I also learned i had a cyst on the bitesticle that they burnt of.
    I wish it was ”only” the pain, but all these bacterias and antibiotics makes it so hard to keep on. I dont know what to do.. i have a daughter coming in october, so that and my gf is the only thing kereping me alive. Im 29y/o and it all started 2016.
    Shoutout to all the brave men fighting this horrible disease! You are not alone.

      This might help or got E coli is bacteria might comes up from your guts. Try L-Glutamine pills for gut leak maybe that will help.

    Doc! I guessI have this CPPS or Non-Bacterial Prostatitis. I was diagnosed last year with prostatitis took some meds but still I have this dysuria pr burning urination and also I experienced pelvic pain most of the time. Can you give me some tips on what to do Doc?

    I had a case of prostatitis when I was 25--it felt like a had a tennis ball between my legs, hard to urinate. The doctor massaged it and I thought I was going to die it hurt so bad. He put me on some medication that caused me to urinate orange for awhile, but then it cleared up. That was in 1969 and I've never had any trouble since then.

      I feel like I have this im 20 and went to the doctor I thought it was a uti I took a urine test and the doctor said there was bacteria... then they send it to corporate and say they didn't spot any bacteria. But now that I took the antibiotics for uti I still feel like a tightness and its hard to start peeing and emptying the bladder... and alot of dribbling and it does feel like a tennis ball between the legs.

    Can you exercise while having pain? I'm very afraid of lifting heavy weights or doing strenuous exercise while having pain in groin and scrotum.

    I had exactly CPPS with the symptoms you describe plus low back pain, tried a lot of stretching and supplements, but what actually help and make me free of symptoms are doing Ketogenic diet and intermittent fasting, like after 1-2 weeks of carb free the symptoms went away.. 3 months after i had Abdominal USG again and the doctor said my prostate is in normal size and not inflammated like before.
    The problem is i got too skinny and feel tired (maybe i dont do keto in the right way), so i start adding healthy carb like red or black rice.. i eat whole food only and limit my other carb intake.. but i feel the symptoms are back but in a mild intensity.. so im sure the culprit is carbs.. im still trying on my diet modif, maintain regular exercises, stress management.. btw i strictly avoid caffeine like tea, coffee and chocolate, also avoiding milk.
    My suggestion is, if you are capable.. avoid carbs.. trust me this works, if u dont have any problem like underweight this will works wonder.. hope it helps.. God bless ❤️

    Thank you for your videos as I find them excellent, they provide lots of relevant information about my condition. I have chronic prostatitis, previously, after the antibiotics did nothing, eventually, my condition improved to the point it went away for years. Unfortunately, with the advent of Covid, working from home, sitting at a computer terminal probably for far too long, my symptoms have returned. I have pain in my perineum now, which is quite uncomfortable. I went to see a urologist who prescribed Alfuzosin, which created side effects such I got off it. I'm taking saw palmetto, getting up from my computer often, drinking less coffee, working with a personal trainer twice a week. Is there anything you can suggest as well as other natural herbal supplements I can take?

      PSA will naturally fluctuate within a 1-or-so point range. When you add in an inflammatory condition like chronic prostatitis, those swings may be more severe. With advancing prostate cancer you will typically see a consistent upward trend in the PSA rather than a bouncing up and down. I usually describe the normal PSA pattern as a saw-tooth pattern. In your case, if there's ever a true concern regarding the PSA, a prostate MRI may be helpful.

      Not sure your age, but if I was in your shoes and I had chronic Prostatitis on top of a low grade prostate cancer, I'd be taking a hard look at a radical prostatectomy. That's just me though. Life is too short to live in pain forever, and I wouldn't want that cancer in me, no matter what grade.

    I have a reccurence of Cpps lately.i have all the synptoms youve mentioned...is it possible that stress is tue main culprit of the reccurence?the gp just gave me an antibiotics for one week but i dont have any infection in my urine but the music threads levels is high..what does it mean?

      Stress can absolutely be a trigger for CPPS.

    My doctor literally did the opposite and prescribed ciprofloxacin (the worst antibiotic possible) even after establishing that my urine sample had no sign of infection from two different source exams. I then come across this video that repeatedly stated NON-BACTERIAL prostatitis cannot be treated with antibiotics. I am a basketball player and I realized I just began a course of ciprofloxacin which affects tendons and leads to ruptures. Is it too late to stop taking it?

      Absolutely, those can be signs of non-bacterial chronic prostatitis. They can also be signs of BPH or urethral stricture, so evaluation by a specialist would be advised.

    I have given up trying to resolve what seems to be some form of chronic prostatitis. Have had varying symptoms since 1997. Main problem is mostly frequent urination, especially during the night. When the infection is especially noticeable, my urine has a weird pencil eraser smell to it. Processed foods and sugar often worsen condition. My G.P. is never concerned unfortunately.

      I've been dealing with something similar for about 3-4 years now. It comes and goes but never seems to fully disappear. It does seem to get better when fasting particularly reducing carbs and sugar and drinking lots more water. I've also found that oregano oil helps immensely but still figuring out the exact dosing to hopefully kill this infection once and for all.

    I have all the symptoms of chronic prostatitis to a T..... I had a PSA of 5.5 and the urologist wants to have me do a biopsy. The DRE was normal with a slight enlargement. My question is why wouldnt he exhaust all the other measures that my symptoms could be and instead go straight to a biopsy. I had no infection tests or anything like that, I did do a 2 week round of cipro which dropped my score to 4.5 but then it rose again, wouldnt that indicate infection??. Also I would really really like to know and cant seem to find the info anywhere online, just how much of a rise in score will ejaculation or heavy exercise within 24hrs of a psa test actually raise it. I can find that those activities can skew a test but not by how much... Thank You

    Hi... Am 29,i was diagnosed with acute bacterial prostatitis with positive urine culture showing e coli growth...My prostate was enlarged weighing 67.70 gms but after 14 days of antibiotics course my prostate reduced to 22 gms, should I have done psa... My urine culture is still positive

      Quitting coffee definitely had a huge impact for me. I get the pain probably 1 or 2 per month if any and it doesn't last more than a few seconds.

      I have the same symptoms and have been dealing with them for 3 months antibiotics help but I still have bad days. They are going to have to run more tests soon.
      I'd recommend you see a doctor even though it's expensive.

    I'm 22 and I'm showing all the symptoms, I keep getting sharp pains in my genitals and pain in pelvic area like right above my pupic bone, everytime i go for a pee I get a sharp shooting pain in my urethra and it takes me longer to start a stream of urine my frequency of going for a pee has also definitely increased also I get pain in my bum from time to time and it's only really relieved when I'm standing up walking about also I have IBS if that is accossiated with it... could you give me some advice doc does this sound like CP/CPPS?

      If your urologist can’t find anything obvious then it could be a muscular problem. Strengthening and stretching your hips will help massively

    Dear DR ERIC .
    I have blood clot and blood in my ejaculation. Psa normal. Symptoms=weak stream and frequent urination. NO Fever , just a little burning sensation in prostate and penis.
    This has occurred 4 times. It comes roughly every 1.5 yrs. Each times it lasted for about 2 months. I didn't take any medication. It came, lasted two months , then disappeared with no medication.
    But since eight yrs ago, i started to notice that my urine stream was weak. Could this be bph, or prostatitis.? I am now 50yrs old. Thank you .

      If the culture was positive then it was infection. 5-7 WBC simply indicates inflammation, not specifically infection, though it can be from infection.

    5 years ago I experienced clear watery discharge from penis , burning sensation in urethra and frequent urination ....Doctor told me that this might be some STD like gonorrhea..........He gave me antibiotics which i used for a week but after one week i started feeling severe pain in testes and swelling.....I changed the doctor and the new doctor did tests .including urine culture, VDRL and HIV test .......Thank God all tests were negative ............I was then given an injection and antibiotics for one month ...I cured everything and was fine.......But now in 2023 i started feeling mild pain in testes and sweollen veins in scrotum which comes and go ........and pain spreads to lower abdomen , back ....and even thighs and legs ............and i feel better when i exercise.,..,..............I have repeated the tests before taking antibiotics but all were negative........Antibiotics arent working and 25 days have been passed .....
    Is this prostatis or some STD like gonorrhea?..If so why bacteria doesn't show in urine culture ?...please help...

    I hate this condition. I've had it for more than a decade, on and off. I wear diapers due to this because I need to urinate often and sometimes toilets are not available. Sometimes ejaculation is painful.
    I've had since I was 23. Medication helps -until the symptoms return.

    Prostatitis sucks! I've had it several times in my early twenties with crazy high fever and just sick in bed. My doctor said sometimes you just get dealt a bad hand and end up with the infection. It seemed to happen after I was very sexually active and then abruptly stopped due to a relationship ending. Fortunately I haven't had an infection now in over 20 years coincidentally as long as I've been married.

      @@therealgist I was extremely sick with flu like symptoms, pounding headache, body aches and a very high fever 104 degrees, and my balls, taint and back ached bad too. The doctor put me on Ciprofloxacin (big horse pills) 500 mg twice a day for a month. I was also told to ejaculate daily once I felt better. The fever broke within a couple of days and within a week I was starting to feel better. The doctor said the prostate was like a sponge and when you nut it would help by squeezing some of the infection out and it is also why you are on the antibiotic for so long. Now I had a severe case most likely caused by bacteria but I guess there are dudes who deal with it chronically which would suck.

Probably the regular sex of 3 or 4 times a week in a long term relationship VS being young and horny and banging 5 times in a day in short lived relationship. I meet someone and that is all we would do for a a month or two and then break up. Banging 5 times a day became jacking off once or twice every couple of days and the abrupt change in nuttting frequency messed me up.

    Hey guys, do red meat increase your symptoms ?? When i eat red meat i began feeling bad.
    I can be fine for some days, but whenever i eat red meat my symptoms come back again. Is this chronic prostatitis ? Or is another type of problem ?

    My feeling to the answer to understanding the cause of most cases of CPPS is metabolic. I also would say that CPPS visits persons with a significant _slowing down_ of their real base-metabolism. As an allegory, according to Dr. Navaz Habib, most people don't breath the way they instinctively breath their first year of life. It is corrupted with an American milieu and diet very quickly...

    Can chronic prostatitis be triggered via an STI such as Chlamydia? I experienced all the symptoms mentioned, and the doc diagnosed me with chronic prostatitis after a rectal examination. To cover all basis he asked me to get an STI/STD test done and unbeknownst to me I tested positive for Chlamydia. He said it was the root cause and that the bacteria spread to my prostate to cause inflammation. After an ultrasound my bladder and kidneys were fine. I am also experiencing Haematospermia and unsure if this is directly in relation to the Prostatitis. After 4 weeks of anti biotics I can say no more burning during urination, although it still hurts to ejaculate and at times I see a small amount of blood in my sperm during masturbation. I'm not sure if all the mentioned has been a domino effect due to the Chlamydia, but thought I'd share as for a long while I thought I was alone with this issue. I love the video Doc, please shed some light if you have the time!

      After i cured mine, i never thougjt i had kidney stone passed... I thought it was due to my ghonorhea.. So bettet to check what cause recurence of new symptoms
