Causes may vary, but the most common is a failure of the oring seals that separate the oil and fuel paths between injector and chamber wall, this can be due to wear, or to incorrect installation (like deforming them when mounting, or pinching them when inserting the injector in the engine).
We are sorry if you had a bad experience. We note that "nozzles" do not have any opening pressure by themselves, the NOP is calibrated by the shim and pin acting on the spring above the nozzle, if you replace the nozzle you MUST recalibrate the injector, including those shims as needed. As an altrernative, we sell "nozzle groups" which in the case of Caterpillar C7, include the nozzle, bush, spring, and shims, and come pre-calibrated with the correct NOP. We note that also in this case it is ALWAYS necessary to check the complete injector on the test bench and recalibrate accordingly if needed. Having the correct NOP just gives a starting point.
We cannot advise on engine troubleshooting as we only focus on the repairing of injectors themselves. Please pay special attention to the orings, and that they slide in without getting pinched, tretched or damaged during installation. Damaged orings may cause the problem you described.

bad orings can cause oil/fuel mixing, so replacing them is a good practice.
