Ishtar and for example has both her Rin-appearance and an original one that is an homunculus. Because of this, despite being the same entity, they are two completely different people. Same for Ereshkigal, which would be a rather unpleasant one if not for the Rin influence. This is the "canon" reason for pulling this. The actual reason is probably to justify putting characters like Shirou, Rin and other OG members of the cast in a roster of characters easily without having to worry about retconning stuff. Also, fanservice. It is a gacha game after all.
Well Rin, sakura and shirou cant become hero, but fans want them to became hero, so this is the logical way without breaking their lore, if you saying what about archer emiya, he exist but ppl prefer the shirou that still teen, dunno about other game that break their own lore, so exclude those from this game

@@Karuisdead oh yeah. Pseudo-servant are either too strong or too weak to be summoned normally. On top of it, there are the composite servants that are basically an amalgamation of multiple "characters" either summoned together or fused into one. Man, FGO really added quite a lot to the Fate-verse.
