They lyrics in this song were to compare how serial killers and himself have crazy thoughts in their head and relationships with girls have been his downfall to emotional distress, up to this point. People didn't like the different voices he used to do slant rhymes. He even admits in an interview that he was crazy drugged up during the making of this album and just did the voices to make more words rhyme. He finished it in sobriety and added this song to the album at the very end. This song was him flexing his lyrical abilities as a sober man. It was after this he became "woke" Em and started working on Recovery as a sober rapper. I loved this album. Still do. If you listen to it from beginning to end, its a great "crazy" album.

The Relapse album was probably his most thematically creative. Most of it wasn't about anything in partucular other than conveying a psychological horror vibe. It's psychotic horrorcore rap.
The flow is crazy. He's mentioned it in 2011 Bad Meets Evil "The Reunion" 'Don't act like you don't like them them accents, I rap tight" he's talking about Relapse and especially this song. He's always been dark and twisted due to his childhood trauma and has a thing for murder and serial killers and horror so he went in on that this entire relapse album.

They had a mini amount of popularity, unfortunately because they were the most 'normal' looking. It's just a quick nod to them, because he used to do the major pop culture stuff, like going at Superman that got paralyzed. It was just some shit that he always does that goes over 90% of people's heads cause Em is a strange dude. Especially since this was the album where he mentioned strange shit like this, and serial killers a LOT.

That whole album is essentially just a rhyme flex. He says some things with substance but a lot of the songs have whole versus that don’t actually pertain to anything. It’s just a flex with words. Many people regard this as his worst album but in a way it’s one of his best in terms of lyrical ability. He beats words into a mold that will rhyme with what he wants

He's not talking about anybody specific when he's talking about the twins. Listen to the lyrics again, he said "conjoined twins" when he first started talking bout them, and then began talkin bout how he was gonna seperate them...😂 Ems sick man.
He was tho. There's a famous conjoined twins out there and when em wrote this they were in the news and stuff. They had been studied and did like a documentary or something. Sooo em just went sideways like he occasionally does and dives down the rabbit hole and does his crazy rap shit. So this album he was high and then getting sober and then relearning how to rap. He's always been dark and twisted due to his childhood trauma and has a thing for murder and serial killers and horror so he went in on that this entire relapse album.
Oh okay... I kinda figured it was, but wasn't sure. I'm aware of his horrorcore rap style, I'm actually really into that genre based out of detroit. Other rappers like king gordy, and esham were a huge influence on em back in the day.

No problem man... King gordy, and esham are underground. Esham actually created the genre "acid rap" which is basically what we call nowadays horrorcore. He's the other half of "Natas" which was him and his brother, they rap about devil worshipping, exorcisms, etc. It's not for the weak minded. King gordy, bizzar, d12,em, insane clown posse, twiztid, blaze ya dead homie, amd many more were also inspired by Natas/ E.S.H.A.M. (East side hoes and money.) The creators of that shit.lol
