There is plenty to go wrong with the x-ray, sometimes it just doesnt work, like in sniper elite 3.
The reason why many games dont do x-ray is because x-ray doesnt deliver good aftermath when its the main focus. Many people would prefer seeing their guns ripping everyone to shreds, than see a cutscene and then the brain matter just disappear like it was never there.
