- “Service book is a contemporary record in minute details of a person’s official’s career”.
A service book is maintained for every Govt. employees including Gazzeted Officers
No need to maintain service book in cases where they are recruited for purely temporary vacancies not likely to last for more than a year and not eligible for permanency.
Service Book must be maintained in duplicate for a Government servant from the date of his first appointment in Government service, by the Head of Office (HOO) in which he is serving and transferred with him from office to office.
Service book of a government servant will contain following particulars.
Signature or left thumb impression of the government servant will be obtained in the presence of the Head of Office or authorized officer.
A photograph of the government servant has to be affixed to on first page the service book. Cost of the photograph will be borne by the Department. This photograph should be changed after every 10 years.
Medical examination on first appointment.
Verification of character and antecedents
Declaration about marital status
Declaration of oath of allegiance / affirmation to the Constitution
Community status: Whether belongs to SC/ST/OBC
Whether recruited under ExSM or PH quota or sports quota, compassionate appointment etc.,
Date of birth in words and figures.
Education/technical/professional qualification at the time of first appointment and those subsequently acquired.
Details of family members in Form-III
Nomination details for GPF/DCRG/CGEGIS
The following statements / declarations are required to be kept in the Service Book with the entries attested by the Authorized Officer:-
(i) every step in the Government servant's official life; prescribed format should be maintained for every government servant separately.
(ii) every period of suspension, interruption, etc., with its period;
(iii) penalty / reduction to lower post, with reasons;
(iv) certificate of annual verification of services; and
(v) GIS entries
(vi) The fact of availing Leave Travel Concession for himself and/or for his family.
Entries made in the Service Book should be attested by the Head of Office or such other officer duly authorized in this behalf.
Service Book must be maintained in duplicate for a Government servant. One service shall kept in office and 02nd handed over to employee.
In January each year, the Government servant shall hand over his copy to his office for updation. The office shall update and return the same within thirty days of its receipt.
If the Government servant's copy is lost by the Government servant, it shall be replaced on payment of Rs 500.
Every Head of Office shall initiate action to show the Service Books to the Government Servants concerned every year and to obtain their signatures therein.
When a Government servant is transferred to Foreign Service, the Head of Office must send the Service Book to the Accounts Officer who will return it after noting in it, the order sanctioning the transfer, the effect of transfer in regard to leave admissible during Foreign Service and any other particulars which he may consider necessary. Similarly, on the Government servant's re-transfer to Government service, his Service Book must again be sent to the Accounts Officer for noting all necessary particulars regarding foreign service and return to the Head of Office.
The service book of the official shall not be given to the official on retirement/resignation of service etc.
Service book of employee is transferred from parent department to present department on technical resignation
HOO shall be responsible for custody of Service Book
Declaration and pay fixation orders shall be pasted in Service Book
Alteration of Date of Birth(DOB) (note 06 below FR56)
No request for change of DOB is made within 05 years of his entry in to service
It is corrected if it is clearly established that a genuine bonafide mistake has occurred
Date so altered would not make him ineligible to appear in any school or university in which he has appeared or for entry into govt service .
