Love it how I've completed ME3 twice, now on my third playthrough and I'm still seeing things I haven't before. I always play paragon, can't play renengade no matter how many times I try.
That's easy. First look at every decision you can make and decide what you'd do as Paragon. Now, knowing the most paragon choice, ask yourself "what's the worst possible thing I can do here?". Now, if your goal is to get to the Catalyst with the most dead, you'll need to do a little research to find out how to do certain things like survive the suicide mission with minimal crewmates, particularly ones who you can get killed by doing nothing in ME3. For my Engineer, those two were Jack and Morinth. Jack becomes a Cerberus slave if you let Grissom Academy expire, and Morinth becomes a Banshee on earth no matter what. Some highlights of my campaign! Told Garrus to fuck off when he wanted to join in ME1. Romanced Ashley (yes, I consider this a renegade action because well, I think she's kind of boring), killed Wrex on Virmire and got Kirrahe killed. Killed the Rachni Queen. Killed all the colonists and that asari on Feros. Let the council die and installed an all human one. I still installed Anderson as councilor because no matter how I play, Paragon or Renegade, fuck Udina. ME2 I sold Legion to Cerberus, treated everyone like crap, and romanced Jack. Sided with Morinth, obviously. I ended ME2 with only myself, Morinth, Joker, Jack, and Mordin alive. Wait, Mordin? Well I also decided, from a roleplaying perspective, the one person this particular Shepard would give a shit about is Mordin. My Shepard is a crazy engineer, and us crazy engineers gotta stick together. And it's very hard to get it such that Mordin, Jack, and Morinth survive the suicide mission. The key is that characters who are loyal and are left to hold the line die last. ME3 everything got worse. Treated Ash like crap on Mars and never visited her in the hospital. Never got Javik. Never did Grissom academy. Sided with the Geth over the Quarians, let the fake Rachni queen join the war effort. Sabotaged the Genophage (thus keeping Mordin alive and dooming the Krogan) Killed Ash during Udina's coup. The group photo I took at the end of Citadel was about the saddest one I ever saw. When I got to Earth, the only one I could say goodby to by holocomm was Mordin. When I got to the citadel, all I could choose was destroy, and since I had such a low asset score, it blew up the galaxy. Final death count: Wrex, Kirrahe, Feros Colonists, the Rachni Race, the Citadel council, Garrus, Tali, Jacob, Miranda, Kasumi, that mercenary whos name I forget at the moment, Grunt, Samara, the SR2 Cerberus crew including Chakwas, entire population of that Batarian colony that blows up in Arrival, The Krogan Race, The Quarian Race, The Geth Race, Morinth, Jack, Legion, Vega, Liara, EDI, Joker, SR2 Alliance Crew, Planet Earth, the Reapers, and virtually all life in the Galaxy. I bet Mordin's last years were an absolute blast because that guy loves to solve problems, and boy oh boy did I give him a juicy one. My Shepard wasn't the good guy. He was the Reaper's Final Boss as they tried to save what they could for the next cycle. And they got game over. If we ever get a Mass Effect sequel that lets you import a completed ME3 Save, THIS is the one I'm going to start with. Now, there is an important exception to the "what's the worst thing I could do" rule if you're going for the "mass relays explode, everybody dies" ending of ME3, and that's the fate of the collector base in ME2. If you want to get the highest possible death count, you MUST destroy it, even tho this, IMO, is the Paragon choice. Reason being blowing the base up means the first option you get in ME3 is the Destroy ending. If you save it, Control is the only option. Another thing is to make sure your readiness rating is as low as possible. No idea if you can even raise it anymore, but I remember it took a long time to decay low enough for me to actually get the absolute worst ending.
At 3:24ish, I actually got a different line. Instead of "I've seen the way you look at each other" it was "and how many times have you thrown her on the bed and peeled off her uniform?" I'm guessing it differs depending on whether or not Shep and Liara have slept together yet

One refers to Liara being in love with Shepard (gender does not matter) but she did not have the courage to confess (when we are what we tell her we want to have something with her), and the other refers to the fact that we already have one Romantic union with her and is telling her that we already had an intimate moment XD.
