Here's an idea: Hot challenge charades (Hot Takes). The performer has to eat a hot pepper or take a swig of hot sauce. They then can't have any relief (milk, bread, what have you) until the guesser guesses right. If time runs out before a right guess, the guesser has to do double the peppers or hot sauce. It could also work for Pictionary (Hot Lines).
In a team setting each round could include a predetermined number of prompts. Any unguessed prompts are how many peppers/hot sauce swigs the team must consume. Or the difference between the teams is how many the losing team must consume.
