And it's very pessimistic, as I have taken the lower end of DNB seats available, lower end of number of repeaters, and not taken certain branches into account, and also not considered those who would take admission via inicet. But no harm in being prepared! I do think DNB seats will have much much lower closing but expect all the good institutes to be closed within the rank range. And there's DNB stray vacancy round too where the ranks might go upto 15-20k for many of the upper clinical seats in less preferred institutes! Few lower clinics seats will also go upto 25k in stray vacancy round. So cheer up! Because of combined counselling, there's going to be very less seat blocking in state R2, R3, so there's an advantage for you there. So instead of seeing this video as the absolute, just think of it as one of the possibilities. You may take into account all the factors that I missed and then derive the cut off rank yourself also ;)

I don't have much idea about it, as in general I never hear people say they want to do a 6 year course. Just rely on previous years R1 rank cut off and don't depend on further DNB rounds.

Md should be possible in your state counselling provided there are enough seats. DNB you should get easily. Which state are you from?
