You need your -
1. Passport
2. Flight tickets, and
3. Hotel booking voucher
to fill this form.
Fees
A S$30(INR 1900) non-refundable processing fee is payable online using a Visa or MasterCard credit/debit card.
Processing Time
Your visa application will be processed within three working days (excluding the day of submission). Some applications may take longer to process.

You need to fill that in one form only. But consult with your VISA agent as well.

Either way is fine, but you need to apply VISA through an authorised agent only so you can share your details and they will fill that on your behalf

but I recommend filling this digitally, as it will reduce the chances of mistakes and once filled, you can take print, paste your photo, sign and submit.
