Travelling to Maldives doesn't require an RTPCR report but I would suggest you check once with your Resort before traveling. And do check latest guidelines of traveling back to India as well. You need to fill Air Suvidha Self Declaration form for each and every family member while traveling back to India.

I won't recommend you to use CreditCard or Debit Card for international transactions as Bank will charge you extra for each transaction.

I travelled in May month and Waterville cost me around 15000/night. I don't remember the exact cost of activities as there were really long list of those, but some rough estimate I can provide. Scuba diving : ~24000Rs/person Snorkeling: If you are renting a kit then it's ~3-5$. If you are doing with a trainer then it's ~15$. Kayaking: Complimentary There will be tons of packages available for more reasonable prices. You can club your activity accordingly.
I would personally recommend the Maldives Where you can relax and longue but Bali is more of an adventure destination. Hundreds of places to visit there, It will be a quite tiring trip.

Yes and No. Yes, If you are more into exploring the places and city life, because if you are going to private island resort then there won't be much to explore but only the island property. No, If you more into beach life and water sports, because you can do plenty of water activities. The experience you will get in Deep Sea Snorkling and Scuba Diving stands out. It's unmatchable.
