  • Puberty is an awkward time for just about everybody, but for transgender teens it can be a nightmare, as they grow overnight into bodies they aren't comfortable with. In a heartfelt talk, endocrinologist Norman Spack tells a personal story of how he became one of the few doctors in the US to treat minors with hormone replacement therapy.     Dr. Spack is my endocrinologist - he saved my life, in many ways. Showed me and my family my happiness would be possible, reassured them when I needed top surgery by reinforcing the necessity of this. He's brilliant, smart, and has truly saved so many kids. I feel really blessed to be where I'm at now thanks to the knowledge and resources provided by him.

      I really hope that I will be able to get someone just like him in the UK. He seems like a really great guy who totally gets it and is doing a lot to show the world that being trans is OK.

    Glad this came up. Now I can show my parents that there are people out there like me and that it's not that uncommon. I love TED for this very reason. It seems like everyone that talks is reading my mind, and I'm only 15.

    I just want to give this man a hug. I'm not transgender myself, but I'm non-binary, and it puts me in some of the same social circles as trans people. I think if people would just MEET trans people, realize that they're people, and not the walking stereotypes you see on tv, then they'd start paying more attention to these issues.
    It always makes me happy to see someone who GETS IT. Trans people need to be treated as human beings the same as the rest of us, and if we refuse to treat them with basic human dignity, then who are we to call ourselves civilized?
    Also, people who cry over giving children hormone blockers need to remember, as he said, IT'S STILL REVERSIBLE. All it does is buy them more time to become CERTAIN that they're transgender, without puberty destroying their body, until they do start to give them hormone treatment.

      One question (sense you seem informed on the subject); if halfway through puberty someone were to take the hormone blockers would they be able to revert the past affects of puberty?

    I'm so glad I made it through my teen years without irreversibly screwing up my genitals. Those hormones can really mess with your head. Fortunately it's mostly settled down now and I can have clear thoughts and I'm definitely much more comfortable in my own body.

    Regardless of my feelings about transexual/transgender issues, I deeply respect this clinician. An old school therapeutic-relationship driven therapist. A renaissance man with a practical bent but with boundless empathy for all his patients. Well done sir!

      It's not what YOU said about your child, it's what a doctor, nurse, or midwife said about that child. The medical person said "It's a boy" or It's a girl".

      to SEE FOR THEMSELVES whether the child is a boy or a girl. Kids with gender dysphoria, if allowed to go tru puberty naturally usually turn out to be gah or lesbian. Happy gay or lesbian peopele who do not life long surgeries and very serious medical interventions. But some homophobic people encourage the trans because they don't want the child to be gay.

    i am a 21 yo Male to Female Transgender and i am just starting my transition to female because i fear the judgement of others, back in 2001 i attempted to take my life 6 times because i felt like i was being ripped apart with my body being male but knowing in my heart i was meant to be female since then ive attempted to take my 8 more times, my depression got so bad i could leave my room without breaking down in tears, im just hoping the world is ready for my true self to shine

    The guy's not a brilliant speaker, but I have the utmost admiration for the work he and others like him do. It saddens me so much to see people so afraid of anything different from what society tells us is normal, and shunning young kids instead of helping them realise their own potential. Then again, people do tend to be angry at things they don't understand...

      I’m a psychotherapist and have worked with gender dysphoric youth for over a decade. It’s a false duality to suggest that, just because someone doesn’t immediately and automatically affirm this mental health disorder, they “hate” that person. Quite the contrary. I have nothing but love and hope for these young people. Sadly, the answer is not and will never be (for 99% of trans-identifying) bodily mutilation and chemical castration.

    I'm 14, and for almost my entire life I was comfortable being a girl. Once all the "magic" started happening, I started hating myself. I always kind of thought that is was just the regular self-consciousness that everyone felt, but later on I realized that I felt fat because of my breasts, and I got quieter because I hated how my voice sounded. About eight months ago I realized I was trans, and everything kind of aligned. I'm not completely out, and it's in no way easy or happy, but I know who I am now.

      hello! this is op six years later lol. i don’t remember watching this tedtalk or leaving that comment but i saw your reply and for some reason i feel like responding.
      i actually came out a few months after that comment, and while it definitely wasn’t an easy or painless process, i’ve been out ever since with my family’s support. i’m 20 now and in college, and i don’t remember the last time someone called me my deadname.
      again i don’t really know why i feel compelled to give this update, but i did stay strong, and things did turn out okay

    I'm straight but have gay friends and family. No transgendered though. If it doesn't hurt anyone then I say let people be happy. Props to this guy for understanding that idea, we don't need a god damnit box to put people in we need open mindedness for us to go forward and to help those that need it

    Thank you Mr.Spack. As a Transgender, I believe things matter is how we respect ourselves and how well are we blending ourself into different sort of society. With respect on ourselves, others and also knowledge, I think there is no issues. Only for those who are not open up on this matter, we just need to clarify to them and respect their point of view.

      We should all take it as commonplace to experiment, really.
      Why do genders have to be rigid and difficult? If numbers can be imaginary, something as dynamic and wonderful as biology and neurology can't be solid, but fluid. Plasma even. Hyperstates. Non binary.
      Even the rigid boy and girl labels we have today are already a mix of a lot of things. People are just scared when it comes to new, unknown things.

      ​@@jason32708you seem to have an odd almost metaphysical notion of happiness. It would be impossible for me to be happy if I wasn't able to live my gender identity the way it's supposed to be. I felt real happiness and joy only after I realised I was trans. All the days prior to that were awful, and sure, I felt some occasional joy that lasted a couple of seconds, maybe a few minutes, but then I would remind myself that I should kms. Haven't had that thought in so long now, and it's because I feel good about who I am, my actions, thoughts and words feel honest and genuine. I am able to think to myself "it's pretty neat to be alive". And all of this while I'm a very lonely and isolated person; I have little contact with family, I have a pen pal overseas but no friends irl... But I am able to experience happiness nevertheless, because I'm living the dream I had since I was little, to be a girl

    It's amazing how much the science has changed. The Dutch protocol has since come under harsh criticism for treating kids too early and streamlining them to surgery. This promotion of gender surgery is the default cure for homosexuality in places like Iran. The doctor here is extremely creepy in that he boasts about how someone "looks" and doesn't really address how the transkids end up "feeling" on the inside. You can look like a boy, but not think like one. And I'm a gay man. If I was 12 years old I am sure I would have told my parents that I was supposed to be a girl - based on what everyone around me told me.

    I'm not against this, but skeptical. What if you misdiagnosed yourself as transgender? There isn't much known about transgenderism and HRT. This isn't a game, the stakes are high and the changes irreversible.

      that's what a good therapist is for, physical changes take a long time to be irreversible and are very gradual. plenty of time to change your mind, and if it's a phase, there is really no harm done. Phases tend to be over and done with quite quickly when it comes to kids once going to the doctor regularly is in the cards. Nobody jumps right into surgeries, and almost all regret is specified as a result of negative treatment by others, not actual regret about affirming their gender.

    First off, I'd like to just add my own life experience to the idea that an early transition is CRITICAL. This video is the most concise and well put "crash-course" on treating transsexuality in youth I've ever seen.
    Now, I know the importance and the benefits of an early transition, and this is mostly because I was fortunate enough to be able to transition at 16 years old. Although my experience varies from those in the video, my (relatively) early start has kept me from homelessness, depression, and a whole host of other experiences that are a common burden of trans people - throughout their lives. I cannot stress enough how fortunate I am to have evaded so much negativity and struggle in my life due solely to my appearance matching my sense of self...and what other people want to make of it. I know from seeing first-hand, people in my position simply do not have the same chance to succeed when they're visibly transgendered people. That is why it is essential that adolescents who exhibit clear signs of gender dysphoria be treated appropriately, instead of being left to develop further into someone who they're not.
    In my case, I had to basically fight tooth and nail to get a chance at treatment. My parents shared many of the opinions you see on this page that show a willful ignorance instead of attempting to understand transsexualism. It is a very rare occurrence, not some feature of "the gay agenda" or however it might seem to a disillusioned few. In any case, I saw that my repeat visits to doctors and psychologists throughout my early teens would never resolve in the treatment that I knew that I needed, because my parents didn't agree with their diagnosis. So I was left to treat myself. I had to sell things that aren't legal in pursuit of money that later funded my hormonal transition.
    Every doctor and psych I went to told my parents the same thing that I knew: I was trans. They just simply didn't want to face the facts. For that reason alone, I think it's important that people are educated more about this disheveled facet of humanity, so that trans youth can develop into successful members of society. I think we can all agree that it's much better to prevent a tragedy than witness the waste of one's youth and innate human potential.

    Already at 2:00 in - Why would anyone define someone's sexual "orientation" at birth, and what is wrong with defining one's "anatomic sex" ever?? Everyone with brain mass between their ears knows that when you say "It's a boy" or "It's a girl" at birth that you are talking about the physical characteristics of the baby which do exist and are definable by the definition of "boy" and "girl". This is not discrimination, this is stating pure facts of biology.

    Children who become convinced that they are trans are the children who will NEVER have an identity/sense of self. Once they have transitioned, and realize that didn’t achieve the feeling of comfort with themselves that they expected it would, AND permanent damage has been done to their bodies which will make it about impossible to find a serious romantic partner, well, things get really bad.

    So much for “do no harm” - these endocrinologists are making so much money off these children. I’m unsurprised the vast majority of endocrinologists refuse to treat them. It is only those in specialist gender clinics, which have already come under for child abuse and have been successfully sued by the grown children’s whose bodies have been permanently mutilated. They are very negatively seen in the medical community.
    I hope the parents of these abused children who have taken experimental drugs that caused permanent irreversible damage to their bodies to sue these charlatans. Certainly, remember their GMC number. It’s dangerous medicine. The modern day lobotomy.

    TED Thank you for making this important discussion available for viewing on TH-cam. It cannot be restated enough. Transgendered individuals have one of the highest suicide rates of any group. One has to wonder how many suicides, in cases where the reason is unknown, were as a result of unexpressed & / or untreated transgenderedness.

    It's disturbing hearing the laughter and clapping from the lemmings in the crowd. When he talked about the terrible bullying Jackie endured, and then went on to talk about how his Frankenstein experiments on him helped, I was reminded of a minister in the Israeli cabinet suggesting a curfew on women to reduce the rapes happening at night. Golda Meir retorted, saying there should be a curfew on men. He puts the cherry on top when he says "She has more natural self than half the other contestants." In other words, he's saying men make better women than women.

    My question: You clearly outline the difference between homosexuality and transsexualism, explaining that they are two completely different things (which I wholeheartedly agree). However you compare the two as the same thing in relation to 'escaping' from the DSM which seems like bit of a double standard to me, am I wrong?

    I wish the best towards the movement for transgender rights and the use of the title Mx. (Transgender Title instead of Mr. or M(r)s.). I also agree with him that the Dutch method for hormonal therapy at the beginning of Puberty is the most effective way to naturally alter a person's gender. However... I do not believe that these treatments should be allowed for minors under the age of 18 for ethical reasons within the boundary of the law. It opens a door in which the recipient of the treatment could possibly regret their decision to convince their parents or guardians for the procedure later in life once their brain is fully developed. It also opens a door for potential abuse, such as a parents who may have wanted a boy request treatment for a gender change against the possible wishes of their daughter. Or another situation may be where parents may request the gender change of their son to become a girl in order to win prize money for some gender specific contest. (The whole point is to what extent can altering gender of minors open the door for any kind of abuse).

      +Jason Short Your comments are very refreshing. After spending time on a TH-cam channel filled with people who hate the transgendered with some people professing concern for us when their true intent is to have a world where no one is trans (in fact, they don't even acknowledge that some can be trans by labeling all a mental illness), it's nice to see some concerns that are truly thought provoking brought up in a loving context, with no hatred at all. Yes, I'd be concerned that waiting until 18 if a child truly is trans could result in a loss of some benefits but I think these issues are worth taking into consideration by the treatment community. Just like those who are in politics, looking at all possible consequences in response to actions that are taken is worth it. There's always room for improvement.

    I'm still a little skeptical about the subject of working on teens. I'm pretty sure puberty is awkward for most people and the "Pinocchio turning into a donkey" thing doesn't necessarily apply to only people with gender-identity conflicts. In the same way, couldn't a teenage female who's self conscious around other developing females and wants to be more feminine be treated by inhibiting her testosterone? And yet we encourage them to be happy with their bodies. The same could be said about males. I may just be really off base, but I think teen is too young to treat for an identity they are still in the process of developing.

Do teenagers really know who they are and what they want at that age? Or are they different and need time to be comfortable in their own skin? At the age of 13, like that Miss England semi-finalist, a person may not have a good grasp of who they really should be.

      I'm glad you think that you would totally accept it, but I really find that hard to believe after talking to a lot of people that are actually going through it, and how they find it not something that is just easily to accept.
      As for the teenager, totally agree, and that is why what this guy is talking about is less invasive than what is currently being done in most countries. This process seems to delay the child from having to choose until they are 16 for hormones. The 13 year old UK girl was an exception for being given the hormones to to getting the blockers later than the average person would have. By most people know what gender they are going to go by later on in life.

      Andrew Quan If you actually talk to transgender people of different ages, you would find out that their gender dysphoria has followed them throughout their entire lives and can only be treated by allowing them to be their true gender. I'm a MtF transgender who transitioned at age 27, but I've had gender dysphoria since age 5 and have suffered depression all my life. Since I've transitioned, that depression has completely disappeared. Blocking puberty hormones with medication has no harm to the child, but allowing an unwanted puberty causes irreversible and expensive to correct changes to wreck the body. Since I went through a male puberty, I will always have to deal with having limited breast growth, a male bone structure, and an adam's apple and male voice pattern. These are aspects of puberty that emotionally damage me while I was going through puberty and even afterwards.

      you should look up Dr. Zuckers work on this. He treated almost 600 patients with gender dysphoria with therapy and "watch and wait" policy - not puberty blocking drugs and surgeries. He had 80% success in female patients and 97% success in male (meaning their feelings of gender dysphoria resolved)

    this video made me cry really really hard. if my parents had just listened to me so much could have been avoided. i could have been given the childhood ive missed out on. instead im 15 and suicidal and suffering every day because of my body. i dont know if ill even get on t until im 18 because my parents are mormon. things really feel hopeless for me but its so incredible what medical science can do.

    Think about it. You cis-identitied people get to live in your natural homes, but if I try to live in my natural home I get arrested. I don't even want to think how the Russians would treat me if I went to visit Moskow.

Just think about it this way: You're a man, and this is not just defined by your genitalia but also by your psychological state.       The problem is, as this very video described, waiting until they're adults is bad. Once you've let puberty run its course, irreparable damage has been done. Hormone therapy is MUCH more successful if you do it before puberty has taken effect. That's why they use hormone blockers to hold off the effects of puberty. Would you propose they hold off puberty until a person is 21?
      I'm not an expert, but I would imagine that regardless of what physical and developmental side-effects that could have, the societal effects of being a prepubescent adult could be fairly traumatic.
      As the guy with the diploma in this sort of stuff said in the video, by the time they start giving a teenager hormone therapy, they are HIGHLY unlikely to change their minds. Its not like they just hand out hormone therapy to random teenagers, they make pretty damn sure they're certain before they do anything irreversible.

    I wish that this treatment was available before I went through puberty, then maybe I wouldn't be as much of a freak.. There are physical triats that I have that I wish that I didn't, so that I can be passed as a biological (looking) female and not just a transgender female (who is 6 foot +, with broad shoulders and a receeding hairline and little breast)... I hope the whole world embraces this treatment for it is sometimes hard to go through life being called names... If transgender teens can go through life without having to face the pain and trouble like I, and other like myself, life will be a little bit easier!!!

    I'm not hating but i feel this man is just enabling young people to do this at a young age since their brains aren't fully developed and can't make an informed decision that will affect their entire lives, every speaker just talks about their success stories but what about the ones that regret what they did and cant go back?. I think if they want to modify their bodies they need to have a full understanding of what they're doing, also their parents should stop to think if that is the right thing to do or just what the kid in its childish mind wants.

      Children as early as age 3 can suffer from gender dysphoria and depression because of it. It's been very clear from all the cases that have surfaced over the years that gender identity isn't something someone chooses. The only choice in the matter is whether they come out of the closet about it. That is of course dependent on their social environment allowing them to do so. I'm a MtF transgender and I recall as early as age 5 being mentally a girl. I didn't transition until I was 27, because everytime I tried to come out as a kid I was ignored and shunned by those who were closest to me. This is the kind of rejection and stigma that pressures people to be closeted and adds to the suicide statistics. Also having to go through a male puberty was traumatizing and expensive for me to correct later in life.

      Daniel Aparicio That's part of why, as Speck states, the "real" hormones usually aren't given at age 12 or 11. Blocking hormones are used instead, and they're not just given to help trans children, but also to ensure that they truly feel the way they feel before moving onto to the opposite gender's hormones. It's touched upon in the talk a little.
      Children who feel the way they do deserve to have their voice is heard, especially if they feel more comfortable, happier, and better as a result. Children identifying as trans, in my opinion, absolutely deserve to be believed for their feelings. They know when they don't feel happy and/or complete, even if they don't quite know what to do about it yet.

    I really dislike the idea of transgender period. In Thailand males do a sex change because they want to be able to survive because it is a third world country. And they don't really have a choice unless they had some sort of job or income to sustain themselves but in first world countries I don't understand why is there a need to do a complete sex change especially during an early age. Most of these kids don't even have a good enough reasoning to do a sex change. And even the majority of adults that go through a sex change regret the fact that they did it. Also, you can never have kids or go back. It is a permanent decision that you have made. Also, if you ever date or get married, are you going to tell them that you went through a sex change. And the majority of people won't be able to accept it. If you don't tell them and they find out they will most likely never forgive you for not telling them. Only a small percentage truly accept gender change.

    First I have to state that I am a proud man! A he!
    However I do understand that our body may work against of our mind and soul, and I guess (even if it's not ordinary) this fact, that there are transgenders is only highlights more how awesome human beings are! I mean there are genders and social norm around them, which is good, but there are and always will be cracks, where PEOPLE may fall into. The beauty (at least one of them) which lies in us is that we don't have to stick with the way that fortune/destiny/God gave us.
    In the society where I come from, the general expectation from man and woman alike is that they are capable of tolerant behavior and mutual understanding. We don't have to accept each other... but please do accept the fact that we are all people! Human beings. So that all of we have the right to have a peaceful, meaningful and happy life.
    One of our greatest duty is to help each other!

    Too bad some people nowadays really take transgender issues to the point where both sides of the argument have emberassing amount of extremists. Radfems trying to make the movement extremely anti-trans and siding with actual conservatives just for the sake of being hateful? Transgender people threatening violence if you don't want to date them? Perverted cisgender men pretending to be trans-women to get into women's restroom? Really weird code-names for genetal issues, because not all people that have vaginas are women, which just offend more people? Sounds like a fantasy, and yet, here we are
    I'm trans myself, have been out since I was like 12, and since I'm in a pretty homophobic (transphobic as a logical bonus) country I had no way to take puberty-stopping drugs, and later had a really restricted ability to even get diagnosed as trans (yes, it is considered impossible to diagnose something like that under the age of 18 here. While every psychoitrist I had when I was 16, no matter how transphobic they were, at the end of the first session had no problem adressing me in my preferred pronouns. Why yes, look at me just being childish and following a trend without thinking). And I'm happy to say that even these kinds of restrictions have a plus - I'm now really confident in myself, to the point that I can safely say that I am a man, and transphobic people will believe me without any questions, even though I haven't started hormonal therapy and at this point I don't think I ever will. I'm happy with myself, my life is great, and I wish this on every person that struggled with gender dysphoria as much as me, while I was going through puberty and becoming more femenine every day while trying to convinse people from my really conservative family that this suiсidаlly depressed and anxious mess of a person is a man
