Pancreatic Mass | Hepatic Metastases | Portal Vein Thrombosis || Ultrasound || Case 275
Clinical Features:
A 70 years old male patient came with
- Upper abdominal pain
- Jaundice
- Weight loss
Ultrasound Features:
- Irregular, ill-defined, heterogeneously hypoechoic pancreatic head mass.
- Dilated main pancreatic duct, common bile duct with mild intrahepatic duct dilatation.
- Inhomogeneous hepatic parenchyma with hyper & hypoechoic space-occupying lesions of variable size & shape in both lobes.
- Low-level echoes in the main portal vein and its right & left branches with no luminal flow on Doppler.
- Sub-millimeter floating echogenic foci casting comet tail artifacts in gallbladder lumen.
- Moderate ascites.
Remember:
In case of abdominal malignancy, specially hepato-biliary ones, check the portal vein & inferior venacava carefully to exclude thrombus which may affect the prognosis.
