If one of your team tries to hide an error, it points to low levels of psychological safety and trust. YOU OWN THAT!
As the leader of a high performance team, you are responsible for EVERYTHING which affects team performance. Deal with the individual who tried to hide the error with as much empath and sensitivity as possible, but make sure you raise the levels of psychological safety within your team.
