ruined king was noxian king which wife was sick and he sent kalista and hecarim to look for cure. they found blessed isles. Kalista didn't wanted to tell the king about this isles so hecarim did it instead of her. when they came back the wife was dead and as hecarim told him about blessed isles that here they can revive her, he didn't counted on that this would unleash the evil trought the isles. kalista had to help the king but she refused and king ordered hecarim to kill her, then he helped him. her spirit actually was evil and turned the holy water into the black mist. it killed all the noxians even the ruined king, but hecarim and his stead turned into one. and thats how heca and kalista became heca and kalista. The end.
I don't think he did it because Kled doesn't really have a lore. I'll summarize it, He got every single title in the military, stole a big chunk of land from Noxus. He will kill anyone who steps on his land. I don't remember how he got Scaarl though
