  • Post-Tonsillectomy Bleeding
    How likely is it?
What can be done about it?
    How do we avoid it?
    Summary:
Virtually no blood loss in the operating room.
Around 3% chance of bleeding at any point during the 2 week healing process. If this happens, bleeding will usually stop with ice chips, ice water.
Around 1% chance of requiring cauterization or stitch. Sometimes will require trip back to the OR.
During the 2 weeks post op, do not fly on airplanes, go out on boats, or venture too far from health care.

Dr I just want to say thank you for posting these videos. I watched them prior to my surgery and have used them for reference during the recovery period. It helps alleviate some of the anxiety of the process knowing things I am experiencing are normal. Tomorrow I am at day 7 from the operation. Pain got worse today as I think the scabs started to fall off.

I was part of the small, unfortunate percentage. I experienced arterial and venous bleeding at the same time. One was happening lower down my throat, which was awful. I went into surgery and stayed at the hospital for 2 nights while they monitored my recovery. All is well now! No more infections. Although my experience was VERY traumatizing, I would do it again. Thanks for the great video!

Today is 12 days since my tonsillectomy. It was by far the worst and most painful experience of my life. I don't know if I would do it again if I could go back in time... Up until day 5 I did nothing but take pain killers and sleep. I tried to eat solid food because my Dr told me to. He said it would help remove the scabs and prevent bleeding. Early morning day 7 I took a pain pill to help with sleep and started choking. Ended up spitting out lots of blood, then the bleeding would calm down and then start again. This happened three times in about 45 minutes. Then I started throwing up blood and went to the ER. The whole ordeal was quite scary. I spent the day in hospital and now I'm almost 100% recovered, only a small amount of pain, no need for medication any more.

Dr. Liu, I wish our ENT had properly explained the risks more clearly. I feel like being in the 4% and then the 1% took us completely by surprise. On day 10, my then 6 year old daughter had an arterial bleed and was swallowing the blood as it was at the back of her throat and naturally had nowhere else to go. The paramedics couldn't see anything but we transported her to hospital via ambulance anyway. She was given zofran as her stomach was tender. Little did we know what was happening at that time. Two hours later she began vomiting copious amounts of blood. She was rushed to the ER. After transfusion she was fine but absolutely traumatized by the experience, as was I. I cannot encourage parents enough to keep close watch of their children. Yes, it's rare, but it happens. If bleeding occurs you need to access an ER immediately.
Thankfully she is fine now. 9 years old, no longer traumatized, and strep free for 3 years. Prior to that she was sick every month and it was so taxing on us all. The benefits of removing her tonsils and adenoids have been numerous.

Unfortunately was in the 1% on day 12, which was surprising to emergency staff as this is considered rather late. Woke up in the middle of the night choking on the scab/ blood, lost a lot of blood and spend an hour or two in the hospital coughing up blood clots and blood which was awful and it wasn't for stopping. Ended up having to go and have the additional surgery, and spend the night in hospital for monitoring purposes, 4 days post op from this, recovering well and experiencing very little pain.
Interestingly, the side that bleed was the tonsil that I often had the most problems with pre-operation, causing it to be heavily scared and was the most painful post surgery.

My sis was that one percent, scariest moment to see my sis like that but I had to look brave for her, the worst part was, was the night she vomitted blood and woke me up for it, my parents thought to give her cold things like ice cold water, popsicles, things to make it stop...it did but the next night after I had slept with her in the living room to watch her it happened again, even worse that time so they took her to the emergency room but they wouldn't let me come considering that I hadn't slept since I was watching her the night before(staying up on my phone beside her) so I stayed home but yet again stayed up; to text them for a reminder if she was alright cause they had to take a bucket and a big cup of ice and water to slow down the bleeding while they were driving. I then waited for a reply to see if she was alright, which I later got but I think the most frightening part about those nights was when I would be standing by her while she was hunched over the toilet bleeding from her mouth like a slow water faucet while I was shivering but still keeping my composure. The doctor had told my parents that if they didn't take her to the hospital that night than she would've died to bleeding since it wouldn't have been semi controlable anymore. She was really pale for a couple days but I just thank God for watching her during the procedure that she went through the second time and to thank him for her being here.(It was an artery that was bleeding btw)

It happened while i was sleeping and after reading the comments, i realise it happened for quite a number of people while in the middle of sleep too. Probably because while sleeping you went too long without hydrating your throat. It's good to wake up every hour to drink water. When your throat gets dry it tears the scabs and causes bleeding. Some use a humidifier to keep the air moist, it helps too.

There are many blood vessels in the nose/mouth/throat that can pop and bleed. This is more likely to happen when there is inflammation (like an illness). It is entirely possible that this is what happened and it never happens again.
And it's also possible that there is another explanation. An ENT might need to do a thorough evaluation and possible scope to make sure there isn't anything else going on.

Day 7 and I woke up this morning because of the blood going down the back of my throat, I kept swallowing it which made me feel really nauseous while my mom was on the phone with my ENT. I threw up and the only thing in my stomach was water and A LOT of blood. Throwing up made it bleed more and when I started to gargle ice cold water it started to bleed even more because I wasn’t gargling it for long enough. Eventually it stopped bleeding but 1 hour later I threw up again and the blood started pouring again, quickly stopped it with ice water. Really hope it doesn’t bleed like this again or else we’ll probably have to go back to the ENT.

Doctor told me the next day they suctioned about a cup in that hemorrhage alone. It was wild. I had to stay at the hospital for two days and now I am terrified for when these stitches fall. 1)do they form on the surface of the tonsil and are easy to remove?
      Or
      2) do the stones form in a hard to reach cave like area formed between the tonsils and the surrounding tissue?
You can find this out by using the tip of your tongue to navigate towards the tonsils and to try to feel for tonsil stones or just understand the tonsil surface and its geometry/topology/shape and size.
If your case is of the first type then a slight change/alteration in habits related to oral hygiene can help, that is - 1) avoid rinsing your mouth with water upto atleast 20 minutes after brushing just spit the tooth paste out and that's it (only use drinking water to rinse your mouth not tap water, because tap water can contain excessive mineral compounds such as calcium and magnesium)
      2) you can apply toothpaste to the tip of your tongue and then use the tip of your tongue as a tooth brush to clean the tonsils and the surrounding area.
      3) tilting your head to the sides while gargling(to gargle the tonsil area) but be super careful.
If your case is of the second type and bad breath is really troubling you (which is normal) then tonsillectomy is the only permanent solution, but do find someone who is experienced in this... I assume you're from India- so the tonsillectomy operation would cost around 30k to 60k depending upon the hospital, location, method used etc.
Just make sure that you have a week off post surgery, let the surgeon know two weeks in advance that you're willing to go ahead with the operation and ask him/her about things that you should avoid leading upto to the day of the operation (there are certain medications that you need to stop taking weeks before being operated)..
      Hope you find this helpful
And p.s if you're worried about whether removing tonsils could hinder your throat's immune responses then don't worry as there have been studies done regarding this you can read online which suggest that there is no evidence of a significant clinical effect of a tonsillectomy on the immune system.

@@akashawasthi8129 thank you Akash 🙏🏻.. I truly appreciate your response and time taken out by you to write this!! … I fall in the 2nd category wherein my stones are hidden ( formed in the caves deep) and are visible only wen I try to press and scrap them out from the sides of the tonsils . It’s been more than 2 years now.,currently I’m taking dr batras homeop medication which is providing relief to some major extent , however I still hv to clean the tonsils every week which I hate .. hence I hv been planning to get tonsils out but I’m so petrified of the recovery process as an adult that I keep pushing it forward every year . An autoimmune disorder that attacks my blood capillaries which results in a blood stain looking rash on my skin annd during a flare up I have an increased chance of bleeding internally easily. I most likely an going g to need a tonsillectomy, would that be considered a higher risk for bleeding in surgery? Or even after the surgery?

At day 8 I bleed all through the night coughing up blood but because I was so tired I didn’t realise until it had been heavily bleeding for hours. I ran into my mums room and we went straight to the hospital. From that hospital they sent us in an ambulance to a specialist hospital where they had an ENT specialist surgeon. I got put into a ward for a night and left the next day, however because of the severe loss of blood I felt extremely week for about five days after. Overall though it wasn’t too bad I think it was worth it for the amount of time I had my tonsils out.
The only extremely bad thing was the fact that while I was at the hospital in total I had at least 16 tubes of blood taken out of me to test for diseases and get my blood type in case I needed a blood transfusion

I feel like I almost died from this. I ran out of pain meds, the doctor wouldn't give me more so I took ibuprofen like an idiot. My bathroom looked like a murder scene and blood was gushing out of my mouth for hours in the middle of the night. I lived alone and couldn't get ahold of anyone to go to the hospital. As my bathroom garbage can started to fill with blood, I estimated maybe a quart and a half, I was becoming disoriented and had it in my head that I couldn't call 911 because it is so expensive. I was the 3% last night! I woke up coughing and choking on bright red blood. I fell asleep with a hot water bottle on my ear due to earache so I can only imagine the heat thinned my blood and caused this. I swallowed most of the blood but I did spit a fair amount into a cup, so that was about an inch or two or bright red blood. It was terrifying! I put a bag of peas on my throat and drank cold water which stopped the bleeding after 10 minutes, my throat looks okay now no active bleeds anymore. I’m scared to eat and drink now! @@VALPARAISPECIAL Hello! I’m not really a doctor but what helped me was placing a frozen bag of peas (or anything cold really!) over my neck and throat, this then constricts the blood vessels and stops bleeding. It’s scary lol so I feel for you. I think after it happened I was really wary about what I ate for a day or two as I didn’t want to knock the clot out of place and have it happen again. Try cold yogurt, ice cream and mousse as this is soft and cold, definitely keep away from anything warm near your neck and throat and if in doubt contact your surgeon :) It also helped me to not talk or cough/anything that would make it move. Hopefully there’s light at the end of the tunnel for you!

The doctor said that will cause stuffiness afterwards. Are my worried unfounded or valid?

It is very common to do these procedures at the same time. It does not change the bleeding risk in any significant way. A humidifier in the bedroom will definitely help with some of the dryness issues but is not absolutely necessary.

I just had a tonsillectomy 5 days ago. I’ve had a lot of pain but it’s manageable.
I’m concerned because I clear my throat ALOT due to acid reflex, and even though I’ve tried my best to not, I’ve done it a few times during post op recovery. Also, one of my tonsil beds has a bright red streak that looks like a cut. I haven’t spit out any fresh blood tho, only old brownish blood. But should I be worried about the red streak? I’m just not sure if it’s normal or not.

It's hard to know from your description. Certainly, fresh blood would be concerning. Other times, a slow drip could collect blood in the stomach and cause nausea. If you vomit up brown/black "coffee grounds" appearing material, it likely means there is ongoing bleeding. Also, blood further down your intestines can cause cramps, loose stools, diarrhea that can be black or "tarry".

I was told it was because of an infection and there was nothing i could of done about it.

TLDR had to be cauterized 3 times in the OR and once in the office.
I’ve had my tonsils out about a month ago The fourth night after getting them out and a relatively easy recover so far I wake up with a thick feeling in the back of my throat get up go to my sink spit some blood and it just keep coming out like a facet get ice water. My mom drives me to the hospital 15 minutes away five minutes into the drive I stop bleeding. We get to the hospital go to the ER they take blood and do there thing. I thought it was going to be no big deal I figured it was normal for a little blood. The doctor comes in and says they aren’t equipped to handle it if I bleed again to be able to stop it considering it a small rural hospital and if I bled again I would have to be flown to a hospital 90 miles away. They tell us they got an ambulance on its way to transport me to another hospital 60 miles away, I get there stay there for observation a couple hours then right before they were getting my discharged papers ready, I feel the warm thick feeling in the back of my throat. Bleed like a facet for thirty minutes start feeling weak they are getting the OR ready. I finally stop bleeding after a good half hour of using suction and spitting of blood. They prep me for OR and send me back. They cauterize i come out and about 5 minutes later still under I start bleeding once again I get sent back for another cauterization. I wake up with blood on my back and everything. They send me home me being annoyed I had to restart the recovery all over again. The first go around I had no pain for the most part. I ate soft foods and drank plenty this recovery did the same thing. another 4 and half days roll around I fill my mouth fill with that warm thick feeling I go to grab a dirty towel and spit into it yell to my mom and let her know I’m bleeding (I’m 17 btw) We get a bag of clothes ready rq and hope in the car and go straight to the hospital an hour away the bleeding haults within ten to fifteen minutes. Starts again maybe 30 minutes later for only a minute. We go to the ENT doctors office and the same one who performed the Tonsillectomy he uses the spray and match things to cauterize the same side for the third time . They send me home annoyed again I was annoyed about it all. Come eat soup and told to follow a soft food diet which I already was since the beginning. Being two now a week and a half(9) out of the first surgery hoped I be healed but no there I was now getting pain from the last two recatuzation . The next day morning rolls around and I get a metallic taste and constantly swallowing I go to the trash and spit no blood. Lay back down the back of my throat feels full get back up and spit no blood, my spit i really mean let the saliva drip out my mouth. Lay down for a couple minutes boom blood but a small amount yell for my mom, and with the bags packed yesterday still being in the car go up to the hospital this time . 45 minutes on the way there I start gagging and getting cough up blood clots it stops by the time I get to the hospital, go to the ER waiting spit up a little bit in there finally get taken back no bleeding so they admit me for the night and bam no bleeding get discharged in the morning feeling like a hundred bucks get to drive around a little bit that night it’s a Saturday When I get out 11 days from the first surgery and two days after office match sticks. I return to school and work on Monday after being out two weeks was only planning on missing a week but turned out to be two so far. I get through the week being on a soft food diet till Thursday a week after office match stick thingies. Still taking the food light I get through the weekend till Sunday morning 19 days after the tonsillectomy and 10 since the office sticks.i wake up at 3 in morning with blood coming out getting it to stop immediately me and my mom go sleep on the couch thinking it was a one and done situation. Evening time rolls around I’m out cruising I get that warm gushing feeling in the back of my throat that I know to well by this point. I Catch a ride home rq call my mom to let her know I’m omw I get dropped off cup in hand with little bit of blood not too much me thinking damn this sucks not again, my dad being home this time ( works outtta state) gets us in the truck and hauls ass to the hospital I start bleeding worse and worse so five minutes out of a town 30 minutes between my house and the hospital my mom calls the EMT to meet us somewhere., we park where they tell us, and wait about 5 minutes they load me up my mom in the front and my dad going to meet us up there. The bleeding stopped by the time the ambulance arrives they take my vitals and we begin going, they see how much blood I’ve spit up in a little trash can, I start bleeding again not to sure when as It became a blur I remember going in and out of consciousness and having the emt rub my chest to wake me up from what he said could of been a seizure, I wake up with blood all over me asking what happened still in a wierd haze. Before slipping out of consciousness all I remember is my head getting a real cold fuzzy feeling real relaxing like and zooming in on what felt and looked a big projector which displayed my dream that I forget within seconds it felt long, he said I was out for 10-15 seconds max. The sirens go on and they start rushing fast we finally arrive at the same hospital I get rushed to trauma one where a big group await me. The next hour or so long is fuzzy to me. I remember getting in there not bleeding then started bleeding. They tried getting blood work I threw up blood at this time and don’t remember the for a little while longer i then remember the same fuzzy feeling again and I lose vision which my mom chalks up to be stress and panic, I don’t remember after that till I get poked with an iv which I hate they start giving me fluids I change gowns and sheets and await before the send me to the OR I remember doctors and nurses coming in and saying is that all from you just on the floor the estimate I lost a liter of blood which is about 20% of my blood volume. Sent to OR came out good. Told to be on soft food and no school and work for two weeks not out of school a month for a simple tonsillectomy. Writing this 10 days since the last incedent no more blood and hopefully stay that way

I strongly recommend calling your surgeon's office ASAP. If you aren't sure, going to the ER is always the safest route. There's no way to predict what may happen next. We usually want no bleeding at all.
Bleeding is a risk that can happen at any point from the OR to the time where the last little bit of scab is gone (typically 2 weeks post op). We don't like any bleeding. Sometimes a little bleed is all that there is. Other times, little bleeds happen before a big bleed.

I’m 6 days post tonsillectomy but I’m very scared too eat things (only ate ice cream last night), as my tongue is still very swollen and I can’t open my mouth too wide to fit a spoon, I also have a very sharp pain that comes and goes when I swallow, and i don’t know if it’s normal or not, just some advice would help

    • @GeneLiuMD
      @GeneLiuMD  5 ปีที่แล้ว

Tongue swelling, tightness of the muscles, pain with swallowing are all typically expected after tonsillectomy. They can in rare situations be symptoms of something different. Usually not, though. If you are concerned enough, you're best bet is to reach out to your doctor's office.

I am a member of the 1% club. I’m about 22 days out and awoke to excessive blood. After 30 minutes of gargling with ice water and Afrin, I continued to bleed.
I went to the ER and was immediately prepped for surgery. I was cauterized and sent home.
I have a few questions and would love any responses...
Do you still feel like there is drainage after the cauterization? I have rinsed my mouth, but do not see blood. However, as I’m sitting here I feel like I have blood drainage on my throat.
Is the recovery for the cauterization the same as the tonsillectomy? Same or similar pain!
Will there be scabs fall off from the cauterization, which could possibly cause more bleeding?
    Should I expect more bleeding?

I'm on day 11 now and yesterday I had a small amount of blood in my saliva for like 10 min. I was so scared, but by reading the other comments of people who experienced bleeding, it seems that in my case ithe bleeding was way less severe. i think I've just eat too solid yesterday and this made the scabs fall (maybe). But is it still dangerous?

Bro I was one of the 3 out of 100 post op people day 6 I stared spitting up blood and my doctor said that if there was any bleeding that I had to go to the er and it was an emergency 5 mins after it started I went to the er I bled at the hospital for a good 3 hours before they stopped it with ice water. I'm praying it dosent happen again they said if it happened again I'd have to go back to the er and I don't want to do that.

I actually bleed yesterday, the day before that and twice the past night, luckily it stopped after 4-5 Minutes by us just spitting it out and putting ice packs on my neck as well as eating ice afterwards. Im on day 10 now and I hope I am done with the bleeding now, it is actually really scary, even when it stops. I guess in my case its random whenever my body decides to get rid of scabs, because it was always in the evening or at night, haven't been doing anything all day and at least 3+ hours after I last ate something. I really hope tonight were my last scabs coming of. im done with the bleeding tbh :D

Does anyone know anything about taking steroids post op? I’m on day 4 and my doctor put me on course of steroids on day 2 because my throat was so swollen that I was hard to even take sips of water. I didn’t want to stop drinking water cause I know how it’s important. And I chocked a few times trying to take the pills. So I called him and he prescribed me methylprednisolone to bring down the edema and to help with the pain, but now I’m freaking out because I heard that steroids can delay healing and may cause bleeding. Anyone please tell me what you know about it?? I mean it’s hard to think my doctor would prescribe it to me if there were risks of bleeding but that’s what Google told me and I’m freaking out. And I’m only starting day 5 this morning ! 😥

Bummer. Did you talk to your surgeon about it? Just make sure that you keep him/her in the loop. And if the bleeding is at all significant, you may need to go to the emergency room. You can ask your surgeon for how and when to notify them and what to do if it happens again. Good luck!

I am watching your video at 2 am. My 4 year old son woke up just a while ago coughing on 7th day and there was bright blood on blanket as he wiped his mouth with it and then on tissue. We are concerned but he is very very sleepy as its midnight. He took Tylenol and slept again.
I called the emergency line and doc said its better I bring him up for monitoring. But he is otherwise asleep, no fever and it doesn't look like its a continuous bleeding but than of course it's difficult to tell.
I don't know to let him sleep and take him see doctor in the morning or take him now? They do wake up couple of times at night every day after surgery, this time there was bright blood streak. To me it doesn't look like continuous though and he has been drinking good fluids.
Not to ask for emergency service here (I am watching him) but does it look like normalish given its 7th day or concerning?

    • @GeneLiuMD
      @GeneLiuMD  4 ปีที่แล้ว

The most likely time period for bleeding post op is around day 7. And it is always still concerning because we can never tell if that's all the bleeding you will have, or if that's just the beginning.
What ended up happening? Hopefully you didn't need to do much else...

    • @randomite
      @randomite 4 ปีที่แล้ว +1

@@GeneLiuMD I end up staying up and monitoring him for couple of hours researching in the meantime. That was my conclusion too after reading that bleeding marks after 5th day is rather acceptable and it could be just a streak of blood not a continuous flow of blood but that was the real concern - which one is it!
He woke up fine in the morning and was better then the day before. I still called the original doctor and she said to bring him up for check up. She said there was no blood clot in throat so it looks fine but if it happens against bring him to ER immediately.
For benefits of other patients - The other two options she said were if were to 1) administer anesthesia again and really look inside the threat whats happening 2) (if I remember correctly) take him back to OT really look at surgical cite which sound like an overkill.
The bottom line is that it's hard to make a sure call if its really bleeding or just a streak but statics are favor of streak than continuing blooding when it happens after 7th day or probably even after 5th.

I'm on day 3 of recovery and I'm 40 they said I did very well at the hospital but now its like I can't stop salivating its so painful to swallow so I can't even get a good sleep because I have to spit all the time and even that is painful. I have a prescription for Tylenol 3's but I find they just make me 'loopy' and do nothing for the pain so I've been taking extra strength motrin every four hours and its helped a bit but I'm struggling.

My mom called the dr he told her to bring me to the hospital, guess because I was moving around by the time we got to the hospital blood was coming out of my mouth. They turned me on my side and rushed me to emergency surgery. The dr said there was so much blood in my stomach. I was 11 it was horrible.

A few possible explanations. Don't know if they had to do more on the left than the right. Asymmetry or one sided symptoms not unusual. Likely to improve with time. But certainly something you can ask your surgeon. He/she may have an explanation based on the details of your particular case.

Hi, I just had a second tonsillectomy. The first was by laser but they didn't remove all the tissue. The difference this time is that instead of just being white, there are what look like little blood blisters. Maybe they are clots? It just looks like a big drop of blood that doesn't move. Is this normal? I'm a little worried about bleeding and want to do what I can to avoid it.
    Thanks

    • @GeneLiuMD
      @GeneLiuMD  5 ปีที่แล้ว +1

Hard to say without pictures, but they could be the areas that were cauterized. They could be clot. The areas can always look white, yellow, brown, black, etc... If you are concerned, you can discuss with your doctor.
