    Sports Hernia Diagnosis
    What Is A Sports Hernia?
    A sports hernia is tearing of the transversalis fascia of the lower abdominal or groin region. A common misconception is that a sports hernia is the same as a traditional hernia. The mechanism of injury is rapid twisting and change of direction within sports, such as football, basketball, soccer and hockey.
    The term “sports hernia” is becoming mainstream with more professional athletes being diagnosed. The following are just to name a few:
    Torii Hunter
    Tom Brady
    Ryan Getzlaf
    Julio Jones
    Jeremy Shockey
    If you follow any of these professional athletes, they all seem to have the same thing in common: Lingering groin pain. If you play fantasy sports, this is a major headache since it seems so minor, but it can land a player on Injury Reserve on a moments notice. In real life, it is a very frustrating condition to say the least. It is hard to pin point, goes away with rest and comes back after activity, but is hardly painful enough to make you want to stop. It lingers and is always on your mind. And if you’re looking for my step-by-step sports hernia rehab video course here it is.
    One the best definitions of Sport hernias is the following by Harmon:
    The phenomena of chronic activity-related groin pain that it is unresponsive to conservative therapy and significantly improves with surgical repair.”
    This is truly how sports hernias behave in a clinical setting. It is not uncommon for a sports hernia to be unrecognized for months and even years. Unlike your typical sports injury, most sports medicine offices have only seen a handful of cases. It’s just not on most doctors’ radar. The purpose of this article is not only to bring awareness about sports hernias, but also to educate.
    Will you find quick fixes in this article for sports hernia rehab?
    Nope. There is no quick fix for this condition, and if someone is trying to sell you one, they are blowing smoke up your you-know-what.
    Is there a way to decrease the pain related to sports hernias?
    Yes. Proper rehab and avoidance of activity for a certain period of time will assist greatly, but this will not always stop it from coming back. Pain is the first thing to go and last thing to come. Do not be fooled when you become pain-free by resting it. Pain is only one measure of improvement in your rehab. Strength, change of direction, balance and power (just to name a few) are important, since you obviously desire to play your sport again. If you wanted to be a couch potato, you would be feeling better in no time. Watching Sports Center doesn’t require any movement.
    Why is this article so long?
    There is a lot of information on sports hernias available to you on the web. However, much of the information is spread out all over the internet and hard for athletes to digest due to complicated terminology. This article lays out the foundational terminology you will need to understand what options you have with your injury. We will go over anatomy, biomechanics, rehab, surgery, and even the fun facts. The information I am using is from the last ten years of medical research, up until 2016. We will be making updates overtime when something new is found as well.     How Do You Know If You Have A Sports Hernia?
    Typical athlete characteristics:
    Male, age mid-20s
    Common sports: soccer, hockey, tennis, football, field hockey
    Motions involved: cutting, pivoting, kicking and sharp turns
    Gradual onset
    How A Sports Hernia Develops
    Chronic groin pain typically happens over time, which is why with sports hernias, we do not hear many stories of feeling a “pop” or a specific moment of injury. It is the result of “overuse” mechanics stemming from a combination of inadequate strength and endurance, lack of dynamic control, movement pattern abnormalities, and discoordination of motion in the groin area.

    29/07/23 started feeling pain in my left groin and the next day also my right side hurt like hell then I came across you guys page since then I followed through Your procedures and today am feeling more better you guys are the bomb

    Great info. Although I’m a physio self-diagnosing is still hard sometimes and this really helped me consolidate my thinking around a current injury.

    Thank you for the great explanation in this video. I had a bad ski accident on March 21st where I went over backwards on a very steep tree run and was backflipping down the mountain and came to a slamming halt about 50 yards from where it started. Very lucky that I did not hit trees. Broken leg, concussion, and I believe impact to the groin area as well as immediate strain in that lower pelvic area. Everything down there got very black and blue which has gone awa,y but I believe I may have a sports hernia as well as groin strain. I could hardly move without pain in that area and it has gotten slowly better to the point where it is still painful but no where near where it was. I really want to get over the hump on this as my leg will be healed soon and want to get back to golf, pickleball, etc. Wanted to see if you recommend starting some rehab at this point and if so I will search your videos to get started for appropriate videos.     Thank you for these videos, I've been having groin pain coming on and off, every time I go out for a walk after 5-10 minutes my groin starts to hurt, I've had this problem since , 2019 as I said coming on and off and it's really etettednto bother me at this point, but I can't seem to understand the problem behind it, this channel has been of help to me though, thank you guys!!

    Thanks Sebastian for a great video and nailing down my symptoms to a tee. Sports Hernia! I'll check out your other videos to see what I can do to fix it. If I was in Costa Mesa, I'd come and see you. But I'm in Yukon (beside Alaska). LOL
    I'm an active, soccer playing 66 year old. Had a hip replacement almost a year ago and the results have been amazing! BTW, the hernia symptoms are on the good old hip, not the bionic one.
    One day I got a little carried away with doing planks and tried a new variation where you rotate your body and lift one arm to the sky, then rotate the other way and reach way under your body. I really pushed myself with this movement, even though it was slow and controlled. Since that time, about a month ago, I've noticed all the symptoms you mentioned. I suspect I'm going to have to stay away from soccer for a few weeks. Up to now, I've continued playing twice a week and I can feel that it's more painful during and after the games.

    Thanks for making these videos. I’ve been experiencing a little tenderness in the groin in the days after my hockey games. During a game yesterday, I felt a pop and intense pain down where the adductors meet the pubic bone. I needed to be helped off the ice. Unfortunately today, the pain was so bad I could barely get out of bed. I’m thinking I’ll give you guys a call as I’m in OC too. Budget and crappy insurance may prevent that though, so thanks a ton for making these pro bono!

      I felt that, it will get better, what helped me when I had that during soccer was resting the first couple days then slowly stretching more and more with the following days , but make sure not to stretch during the first few days so it doesn’t make it worse . Hope that helps

    Thank you SO much for this explanation (in layman’s terms). I think I strained something in that (rt) area which I thought was the beginning of appendicitis. My muscle soreness wraps around to my lower back. I think I’m just scaring myself. Ie torso twist machine at the gym.

    Very good info thank you. I felt my groin pull playing soccer a year ago. The groin pain cleared but since migrated to lower ab pain and weakness. So I can't do crunches or v sits and now both groins feel very weak. I'm very slow to turn or cut. Any ideas? Thank you if you have any suggestions.

      Glad you liked the info. We have a lot of exercises and stretches that work well for this type but they are recommend specifically as a "movement medication".     Hi, great video. Much appreciated! I have had a pain down left side of groin / bottom of abs / pubic bone for 2 months now. Think I got it from a violent cough or sneezing (used to hold them in - never again). I have since been on 2 ski trips and am training 8 times per week (4 weights, 4 HIT jogging on spot) with no problems. Only pain I feel is when I either try to do situps (stomach won't let me) or try to sit up in bed (get a pain when I try to do this so use my arms to assist. Does this sound like a (inguial?) hernia or groin strain? Additional: There are no lumps to be seen or felt. Hoping to fix whatever it is with exercise rather than surgery. Going to doctor this weke for an assessment. Any help very much appreciated.

      Sorry to hear about that. I responded to someone in here recently that sounded like they had a similar story. The facts remain, that most groin pain is a problem with biomechanical faults, movement compensation from old injuries (in other regions) or load management issues. All of there are very solvable with the correct program of exercises, stretches and happen changes.
      I can’t speak for all facilities but in the last 5 years we have only had to send 4 people for surgery for groin pain. That’s a very low number considering how many we see.

      Ya we have seen a lot of people who had a surgery and have had the issue come back within months. We tend to use rehabilitation that seems to work very well. Hip mobility, core stability and repatterning movements. Keeps people back in their activity as well!

    Hello thanks for this content it’s helpful to me at the moment. I’ve been lifting heavy cans of paint this week and my right side groin area has begun aching a lot. I’ve had a mesh implant to repair both left and right side hernias many years ago. The pain I felt today was almost like the mesh patch had moved. It was quite uncomfortable. Could you tell me what if any issues have arisen for people who have had mess repairs for example is it possible to have a 2nd hernia at the same sight even after a 15 year old mess repair.

    Thank you. I hurt my groin last week squatting. It hurts pretty bad but there’s no bruising after 6 days. I have my mri tomorrow and ortho Friday so I’m hoping I don’t need surgery.
    One thing I’m confused about is when you said if you sit and it hurts worse to stand up it might not be either of them. What would it be if that’s the case because that’s definitely my symptom?

    I’ve gotten CT scans both with and without contrast and ultrasound but all negative still dealing with pain above the pubic bone area and groin pain in that hip crease. Feel it doing sit ups ,I can’t go on a jog and the more I’m active the more it flares up.
    Have been dealing with this for almost 6 months.
    Initially started from heavy squats. Did squats one week about 2-3x groin got super super sore seemed like it took forever to heal and then the hip pain revealed itself.

      Sorry to hear that. It’s more common than you’d think. Most people have a lot of imaging done which is great because it rules out serious thing…. But most people we see have intense groin pain with negative imaging.
      A logical reason why imaging is often negative is because the problem is a issue with movement mechanisms. Mechanics and loading strategies are not see on images because they are still images.
      A trained eye can see all of the info you need to design a program to follow.

    Is another sports hernia symptom a feeling of pressure in one testicle?
    I was deadifting heavy singles and as soon as I put the bar down I felt a very minor “tug“ on my right testicle. I didn’t think much of it and continued to train three days a week on a heavy light medium schedule for four weeks. I stopped training after I felt a sharp, horizontal zing across my abdomen. Not long after I started getting some soreness in my right oblique and right adductor. I just started training again very light, but still feel a little adductor sensation and testicle pressure occasionally. It’s been about seven weeks since the first incident.
    Testicle and pelvic ultrasound show no sign of hernia. Thoughts?

    Hi,
    I went to a physician about 30 days ago.
    I was diagnosed with a groin strain, put on medication for the past 30 days. Methypredisolone (steroid pill) for 6 days and meloxicam for 30 days (painkiller pill).
    My injury has gotten better, but I am still able to feel the strain/slight pain when making sudden movements like flipping my hips to run to my left or right (I am a football player).
    Should I go back to the physician to get more medication? Or should I just rest a little longer?
    Like I said, it’s gotten much better, there’s just slight pain/soreness still when making very sudden movements

    Hello I have pain left inner thigh but also slight pain in right inner thigh after being on my feet a lot. I know you said both sides would be a groin strain, but I don’t have bruising. Could I still have a groin strain? I know I tweaked it with a twisting motioning during a hockey game

    Would you feel a mild lower tear down slightly into the top of the testes, by the top of the scrotal sack? Cuz I’m pretty sure I’ve had this before, but I just want to be sure, and I wasn’t doing anything too physical that I know of. Pretty sure it’s healed itself before…

    I was squating in the gym exactly a week from today and I felt something in my right groin. I've never seen swelling or a lump. Just a little pain when I move my leg around. I was doing deep 3 second pause squats which probably wasn't the best idea but Would consider this just to be a strain? I know sometimes It takes a while for a hernia to present its self.

    Today I did a trap bar deadlift, and I do push myself on every workout. I was hitting again a new small personal record of 130 kg which is exactly 10 kg (22 lbs) more since the precious workout. On the second rep (out of 6) I felt exactly on the place you are showing at 01:09 (inguinal ligament) a sharp pain. I would describe the pain as something small inside was contracting/trying to shrink itself while making me feel sharp pain. The pain was the strongest exactly after I just lifted the weights off the ground (while being slightly bend in the knees and almost parallel to the ground from the waist up, and when I was standing straight up with the weights in my hands the pain was almost gone. I finished the rep, and pushed consciously through the pain as I was not sure what it was for 2-3 more reps but the pain didn't go away, and that I took as a sign to stop completely my whole workout. Any suggestions or ideas what it might be? I should also say that I feel like I didn't have the proper form this time while doing the exercise. I feel like the weights where so heavy that it was difficult for me to have a tight core which is crucial for a deadlift. My current body weight is 85kg (190 lbs).

    Can maybe hernia belt help with this? I went to an ultrasound and they discovered a hernia but a really small one, and they even had problem finding it (they found it after a while), doctor said i should do some exercises to strengthen that area and that i should wear a belt. Also he said that i should try kt tape when playing soccer, i tried it but it doesn't really work. And you explained literally all the things that happened to me, groin pain over time not from one particular moment. When resting for a few days it gets better, but it always gets back when i start playing again, such an annoyin injury. Also i feel it on both sides but on the left way way more, on the right side is almost none but it exists, and on the left side after time through activity pain is going down through the leg. Last year i was going to the gym and i think that started it all.

    Hey! So I had this feeling that my groin had to pop all the time or kind of crack. Whenever I would get it to crack, it would feel better for a little bit. Anyways, I went and saw the chiropractor and after the first visit, it went away. The second time however, it hurt my groin. And I have been stuck with this pain ever since. I’ve been doing rehab and now my groin is just extremely tight with extended movements. Any suggestions?

      @@AJdada815 it was often a loud pop, yes. It wouldn’t usually be painful when it popped though. I did get the issue sorted out (I didn’t go anywhere it just kind of healed on its own lol)

    I have had this injury for longer than I should have. I have pain on both sides of my lower groin with continual pain and a lot if popping in my pelvic region. I can not cross my legs while sitting down nor can I stand with my legs spread too far apart. Its been several years now. I am in constant pain,. Please advise or assist

    Hi! This is going to be long winded but hope it won't put you off. I have had snapping hip syndrom for a long time, probably since around 2011. It did not cause pain but after a period of sitting I could make it pop pretty loud when I over extended my hip, anyway. Fast forward to 2018, I was in the gym in January deadlifting, next day after the session I felt weird in my abs, and looked weird too. Almost like my abs switched off (I had some serious 6 packs before this happened), and I also had a very slight bulge on the right side of my lower abdomen. I got it checked and doctors said it was a very minor inguinal hernia that only protruded into the inguinal canal when they made me cough. One of the doctors said it's better to have surgery cause it won't heal on its own. In June 2018 I had a nonmesh open surgery. I recovered pretty fast but funny enough, that slight bulge around the right of my pube never went away. I continued doing sports. This year I purchased a 5 kg macebell and started swinging in January. I went a little overboard with it because I really enjoyed the movement. (this training tool is very hard on the abs). Basically trained every day with it for a week, until one day I felt that old weird sensation in my abs that I felt years ago after the deadlifting, nothing too bad though. I gave myself a day off from the macebell. Then I had a session with slightly reduced intensity. Next day the feeling in my abs were a little worse, so I gave myself a day off. Following day I decided to have my normal bodyweight routine without the macebell, and I also stretched at the end. 24 hours later the ab pain came back with a vengance, but now my right side (obliques?) and my right groin was also hurting, with the odd painful sensation in my testicular area (not exactly my balls but something very close). This was 4 days ago. The pain didn't leave even though I only really feel this sharp pang when I get up from lying or sitting, and a bit when I walk. Still no typical hernia type of bulge visible anywhere. Does my story and symptoms sound consistent with sports hernia? Oh yeah I forgot I also had a minor rotator cuff injury like a month ago on the same side!

    today I was playing cricket and jumped for the catch which led me to bad landing and then could get up back again on my feet till very long and was painful in my groin.     Hello there. I am a footballer and around 6 months ago I firstly started feeling pain down my hips mainly on the right foot. After some months this was gone and when I started playing football again i suddenly developed groin pain which has reduced but haven't been eliminated completely. Everytine I play football I feel very sore in the groin area and it just doesn't go away. I am really wasting valuable time and I don't know what to do because doctors said rest will fix it however I tried both resting and some exercises like Copenhagen planks but the pain still comes back when i play football on high intensity. I would deeply appreciate any advice since I have no support from local doctors whatsoever.

    Hello I have had a an injury on my left groin near the testicle. I can jog but I can’t sprint at all. My belt line feels heavy and pressured at times and when I press down on skin below the belly button I feel a sharp pain. I belive it is a sports hernia but I’m not sure

    I was doing the vertical sit up machine at 90lbs. During a rep, I felt a ripple below my right rib all the way down to my mid-stomach on the same side. It felt like a combination of intense tightness/strain, spasm, pain and a weird sensation of something feeling like it was falling. I left the gym went home and iced. It happened Sunday morning and the right side of my stomach/ab area is extremely tight. Do you think this is more likely a strain of some sort or a hernia? I felt some tightness/discomfort midset and I hadn't worked out in a while. Any tips, advice or suggestions would be awesome. Thank you!

    This Helps. I used to dance/aerobic etc. I'm also a skateboarder of questionable age(!) Waaay ahead of 20s. I use my right leg for push velocity, always. And I find now pain and small bump on inner right pubis bone area. Man it hurts when I walk though! When I lie back it relieves. It can get naggy/prickly pain in day but no stick out lump etc. Is this still Sports Hernia or Inguinal Hernia? Have not gone to A&E/ER yet...

    I was at work standing and out of nowhere I had a sharp pain in lower stomach when I stood on my right foot then when I leaned left as I shifted weight it got worse … after an hour it went away .. there’s no protrusion but when I cough I can feel something move like sliding around in my public bone area … could it be a groin strain or hernia …

    Wow I think I found someone who is expert in this topic. Do you think i got sports hernia or groin strain? It start when i doing lunges with each hand holding 20kg dumbbell. I'm 182cm 81kg. This load is heavy for me moreover i perform this after squat. I just developed this lunges for about four months, before that i only do squat. I'm the one who thinks sit deeper is better until one sharp stinging feeling around my right groin/right upper pubic? hit me when i try to raise my right legs during lunges. It's been three months since i stop the lunges and continue to perform conventional squat instead. Usually even after fourth day of leg day i can feel fast weak pain everytime i raise my left leg by raising my knee as high as water level leaving my right foot stand alone to hold my body weight. If i abstain from squating for sometimes this weak pain feeling would go away but soon would repeat once i perform the squat again.
    Also sometime my right ball feel little bit ache but not pain enough to be called pain. But it's only last for one to two days only after leg day and it seems this feeling is gone recently even after my leg day.

    Hi guys, I’m in England , I’m waiting for a scan in mid January , strained my right groin mid October , was able to walk as normal. End of November made a sudden left turn after walking briskly and then felt a pain in the right hernia area, since then pain walking and testicular pain no bulge, been 4 weeks still the same, any advice appreciated.

      Hey thanks for reaching out! These are all good questions. I think the best thing to do is have someone stress test the hip and the abdominal region. Sadly a lot of people don’t end up having this done because they are unaware that it’s possible to identify symptom generating structures with a specific physical examination. Imaging is very nice but it’s just extra information. I’ve met a lot of people who have completely normal imaging and still have pain and discomfort. If you’d like my help further let me know.       Hey that’s a good question. I have No idea how long you’re actually looking at. All I can tell you is that we we work with a lot of people with problems in the same region they have a reduction of pain within four weeks and often times they are we starting to return back to their full activity within the first few months. With that being said I have no idea what’s going on with you because I have not looked at yet as I mentioned the last comment.     Can a sports hernia lead into a nerve pain in the knee area? I have pain in the inner side of the knee and the doctor told me it was a block of the Saphenous nerve. I have always a discomfort pain on the groin area.

    I'm 36, played a game of basketball, and my groin feels fine walking around, but when I run, cough, or play pickup basketball again, it is a stinking pain in my pelvic area, groin, pubic area. I do not think it is an actual hernia. I've taken two weeks off from playing ball and any workouts.

      I'm goin to say it's a inguinal hernia, I trained for a half marathon and ran that followed by a summer league basketball...sad to say mine was inguinal, put it off for months thinking it would heal. No go. Ended up having surgery here recently to mesh the area. Still healing now and hope to get good results ro be able-bodied to hoop again.

    Hey my name is Jonathan and I do an extensive amount of running probably 3 to 4 times a week. Recently since July I have been trying to cut down my time in my mile to get a better time. I realize that on the left side of my really hurts a lot I thought maybe it was a some kind of hernia. And so I kind of kept on running my time got better and better but over time next morning or so just on the left side started to hurt more than normal.     Excuse me.. can you explain WHAT IT IS when you are in pain walking , "lifting your (my) leg to get "in" the car" & standing?
    Sometimes leg gives out ~ sometimes I feel the pain (go to) in my kneecap

      Yaaaa. That’s pretty common too. We have met a lot of people that feel weakness in the hip when shifting weight into the side as well.

      Because I have the same injury from a soccer injury
      And I've been to a few physios and osteopath too and noone can fix it
      3 years now
      I've done x-ray and MRI and to me the MRI seemed to show difference on the right side of hip but no doctor corroborated this

    What are your thoughts on the symphysis pubis aspect of the true sports hernia, and how this plays so heavily into pain (I've found this to be true in my situation, once the muscles can't fire due to severe pain, that joint just lacks support)? I'm talking about the variant in which the tears are at the aponeurotic plate.

      True hernias don’t tend to have symphysis pain. I think you’re referring to a sports hernia. This comes from overload of the symphasis due to a chronically poor loading movement pattern. Currently research shows the hip and the low back (if not functioning well) are major contributing factors, even if they do not currently hurt. Alex would you be open to doing a consultation with me?       @@Performance-Place-chiropractor I'm referring to the sports hernia, and have battled this condition since 2013 along with two failed surgeries for it, one of which just retore. My initial injury was traumatic injury, occurring during a jiu jitsu maneuver gone wrong. In my case, once the muscle integrity is gone or compromised (rather, there's too much pain to fire the abdominal muscles at all) the joint becomes so unsupported it ends up flared up.     I literally woke up with right testicle pain. I don't run or play sports. I jist lift weight being a trainer. Nothing like this ever happened. Went to a Urologist. Didnt scan me. Just palpated. He said I dokt have an hernia. Just tendinitis in the groin under the riggt testicle. Said ice and lay off the heavy training. Im still lost as to what it is ir what caused it.

      Incorrect. A good examine by someone who knows how to examine is all most people need. MRIs dont show all reasons for pain

    TY! The hips are very often collection points for toxins that have accumulated because organs of excretion are sluggish, and/or opportunistic bacteria, etc (inflammation) is attracted to weaknesses in the area caused by injury, weakening fascia, other musculature, bones. Often exposure to heavy metals such as mercury, and mold create inflammation in these areas and the stress of over exercise, sitting (a real culprit). Being under mineralized, poorly nourished causes severe stiffness, along with the natural detoxification channels of the body being greatly impaired.
