I am incredibly amazed that the doctors picked up on what was happening with the chemical reactions. To learn those specifics, then remember them, then remember them in an applicable situation is very impressive.

I imagine emergency rooms have protocol to follow with poison cases, which are found in the computer. Maybe some doctors understand the chemical reactions behind the symptoms and treatment, especially if their specialty calls for it.

It was probably the friend who had the sense not to eat a bunch of hot sauce in a competition.
Apparently the way to deal with spicy foods is frozen key lime pie. The cold helps soothe. The fat coats your tongue, and the capsaicin is fat soluble. The acid from the lime juice breaks down the capsaicin. And the strong sugar flavor competes with the signals from the pain receptors in your tongue to drown it out. Also, holding your nose when you eat helps dull the sensation some too.

His friends made me very, very happy with their abundance of good sense and making wise choices again and again. After seeing several ChubbyEmu videos where people who could have said, "Yeah, [patient] ate/drank [stuff] before [problems]" but didn't, it was very nice to see people who didn't have their heads up their asses, leading to time needlessly being wasted on figuring out what was wrong.
Yeah luckily they were actual friends and saved his life instead of being making fun of him for his stupid decisions and letting him die.

The unusual thing in this case isn’t the irresponsible use or consumption of medicine/food/nonfood, it’s the quick and measured response from RJs friends. It’s always sad hearing the stories where they would’ve been fine if immediately treated but no one figured out they needed help until much later. Bravo to them!
I do wonder what would have happened if RJ took the bottle with him to the bathroom, rather than leave it on his chair for his friends to find. I mean the fact that they acted quickly here makes me think they wouldn’t have forgotten about him or anything, but it might easily have been an extra 20 minutes before they realised something was wrong.

A while ago when I was in the hospital I (briefly) had to have an NG tube placed and it really irritated my throat so they gave me the same kind of throat spray. It didnt really work so I kept asking for more but they had a very strict schedule of how often i could have it, and they wouldn't leave it in my room, I had to ask the nurse for it each time. I got fed up and asked my dad to go down to the pharmacy/convenience store and just buy some for me, but they didnt have any. So thank you to Johns Hopkins for being so "mean" to me because I totally would have been dumb enough the down the bottle

Jean-Baptiste Lully was a composer and conductor. Back in the day, conductors didn't use those little sticks you wave around. They used a staff that they'd bang on the floor to indicate the beat of the song. During one performance, he accidentally hit his foot. The wound turned gangrenous, but he refused to have the limb amputated, and he died. Not quite stubbing your toe on a grand piano, but pretty close I think
also the title "this is what happened to his brain" would probably apply pretty well, because he died after the infection spread to his brain
I stubbed my toe before taking a wee and passed out.

But in our case, we got sent to a suspected OD case because they thought a guy drugged his date, turns out she was trying to deepthroat him and kept spraying the stuff down her throat before becoming altered. I know this case has been altered for obvious reasons, but mad props for the friends to do the right thing and call an ambulance/tell the ambulance crew what happened.

@@Aramil4 For patient privacy. Dr Bernard cannot just take the exact events and explain them on youtube, because that would violate HIPPA. (Or the Health Insurance Portability and Accountability Act.)
@@Aramil4 each of the stories are changed slightly so that it has no biases and dosent violate HIPPA protecting the medical records abd privacy of the patient. doctors use these all the time for case studies and learning.

I don’t know how other viewers feel, but I love how thoroughly you explain the chemistry behind what’s happening in the patient’s system. I have a background in chemistry and medicine, so the additional information helps me understand what is happening to the patient. Dr. Bernard explains the chemistry in a way that it can be understood by anyone (I think?).
A sign of high intelligence is taking complex information and simplifying/summarizing it so others can understand.

Truly awesome! As a former EMT and MA, I can fully appreciate how hard some doctors really work to save a patient with no history of what is going on. The way he breaks it down is even easier for my 5 yr old to understand. We love your channel 💝. She wants to be a Dr though. I have been saving since birth. It's pretty amazing. I didn't think you could live with half a brain but you can... my youngest daughter had a hemispherectomy done at age 1.5 and she does have issues but overall is doing amazing. She had brain damage from hypoxia and a stroke during delivery..causing brain damage, epilepsy, cerebral palsy,etc.
The body is resilient. For us in the field, 5 minutes of complete hypoxia is typically the cut-off before encroaching brain damage. At 10, it's begun without a doubt, after that you're either a vegetable or dead. Preferably you're dead.

@@thaloblue I'm more curious why they thought it was within normal human skin tone that his skin was suddenly turning blue; only when they read the bottle that said the blue skin is dangerous, did they somehow realize "Wait... HOOMANS NOT BLUE! 'MERGENCY!" But even then, they appear to have failed to convey that he was poisoned with that spray to the doctors in a timely manner, as the case as covered sounds like the doctors were clueless when he came in on what caused his symptoms, tried several things, before discovering that there was a condition which, if they knew the ingested substance, would've been an immediate candidate.

@@oniryuuko You're very correct. One thing I've learned in all my viewing of medical content is this: We are all incredibly stupid. We will rattle off our favorite Chipotle order in extreme detail, but 24 hours later when the Chipotle gives us listeria we simply *forget to mention we ate at Chipotle.* And then when we are near death, one of our friends will happen to say "ew I still smell like chipotle grease" within earshot. So doctors spend 8-15 years studying the human body in order to try and save our lives within 5 minutes even though we've been in the ER for two days.
RJ has really great friends! To not only read the warnings and recognize the signs but do something about it.
There is a story of a girl at a party who got too drunk and ended up passing away in the bathroom, and her friends were having too much fun and figured she was just asleep or something for the whole night, never checking up on her. Ever since that story I've always been proud of friends that are willing to take a moment to check on their group even if it means ending the festivities.

Similar to the story you mentioned, there is an incredibly sad short documentary about a young man's death from alcohol intoxication at a frat party - you can find it on youtube it's called "BREATHE, NOLAN, BREATHE". That is an example of when friends don't do right by their friends and don't pay attention to warning signs. It's terribly sad, but I think it's a very important film.

There was a story on TH-cam with a drunk girl that got dropped off by her friends at her house but they never really checked that she got inside the house. She passed out for 9 hours in front of her house in the cold and ended up having her limbs amputated to save her life
They probably laughed a lot longer after seeing the bottle than this video shows. This video just needed to keep it short.

Me too, although "full recovery" doesn't mean he wouldn't now score lower on a cognitive test than before his mishap, just that his cognitive functions weren't affected enough to noticeably interfere with his life.

Professional singers sometimes used throat numbing spray to suppress the pain of a sore throat so they can perform when they're supposed to. But my daughter-in-law the opera singer explained that this is a bad idea, because a person can injure and possibly permanently damage their vocal cords doing that.
Yes, but not because of the numbing spray itself. Because the pain is supposed to stop over exertion, which could damage vocal chords.
Its probably the same thing like taking pain killers so you can make it through a physical competition without pain stopping you. The medicine masks the pain so you keep going and end up damaging your body because that warning is masked.

Yes. Pain is most of the time a message that something is wrong , broken bone , rotten tooth , burn , throat pain, muscle breakdown etc etc
Being able to suppress it is great if we aknowelegde the problem and work to solve the issue. Yeah you have a broken bone , but doctors made a radiography and put a plaster on it , the pain is not needed. But you know you should not force on it.

You know a guy is good when he makes Chemistry interesting to learn. I think practical cases like these during normal classes at high school would be crucial both for understanding why people study that and to peak interest on future doctors as well. Hell, I'm a graduated engineer and I want to be a doctor because of Chubbyemu videos, cause everything he says is interesting.
Exactly! And this is especially true of mathematics. Sure, 99% of what you learn the average person won't use, but there are use cases, and some of them can be game changing and fascinating and really spark that smoldering interest that just needed a little breath of fresh air.

Not only does he make it more interesting, he also shows how/why chemistry is relevant IRL. The majority of high schoolers would be better served by classes in the applied sciences, which would focus on what's useful for non-sciences people (i.e., why you shouldn't hold kids on your lap in the car; why to follow the directions on the asprin bottle; why to finish that course of antibiotics; why peeing after sex won't flush sperm out of a female). The traditional, academic classes can be available for kids who want to go into the sciences.

Chem definitely isn't for everyone, but a basic understanding is very useful! Gen chem is tough because you get everything thrown at you. For bio you use organic which I find a lot more interesting than inorganic for the most part which you don't go into until later on.
I know how you feel. In high school, chemistry used to make me want to cry. I hate crying. Who wants to be that girl crying over their homework?? Plus, I already cried every single time I had to work on math. So instead I was fucking furious. I couldn't ask questions because I knew my attitude would make it sound like I was about to fight my teacher. I don't miss those days. Sure, I could eat three brownies without seeing them on the scale a week later, but I had to look at both math and chemistry on a regular basis.

It's nice to have an instance of friends who notice something's wrong AND bring the thing that might be causing it, I'm sure it helped immensely in this instance. Also, was not aware you aren't supposed to swallow that spray. Good thing I've only used it a handful of times, and only at the recommended dosage.

As a chemist in the pharmaceutical industry, when preparing aromatic amines by reduction of a nitro group, it's drilled into our heads to look out for the hydroxylamine and nitroso (+ other) intermediates, as these absolutely cannot be present in the API due to being known mutagens. I've not taken biology since I was in high school so I've never really thought about why they are so problematic, so this was an interesting case study for me on why exactly they're so bad!
Anything that will oxidize sulfide groups is especially going to be a problem but reduction of carbohydrates and radicalization lipids is a concern too. Proteins are the most studied in terms of ROS interactions as it can cause cross-linking which will miss-fold the entire protein. People are often surprised how damage to just a single residue can be significant but one you study how ternary structure forms, it makes sense. Just a single regular substitution can have significant effects on protein properties. And with a more sophisticated understanding of lipid signaling, it is understood that ROS are major culprits of inflammation.

I was too really surprised how he showed the orbital diagrams, normally i assume he would explain compex stuff, but i was able to relate to it all that i learned recently in school

edit: It came up in my sidebar again so I rewatched it and it turns out he actually said it had been on the counter for "at least a month". But like, still. Even stored properly in the fridge I'd expect they're only good for a few days. Maybe you actually found that video particularly scary because the guy died even though he barely swallowed any of it? That's an understandable response. And I don't want to give the impression that I'm celebrating his death in any way. But the labelling on food, medicines, and anything else that goes on or in your body is there for a reason. Ignoring that information can be harmful; doing so to such an extreme and senseless degree is bound to end in tragedy.
Growing up in the 1980s, mom would get out the spray bottle of Chloraseptic when we had strep or sore throat. Every time that I do organic extractions of DNA in the lab, that smell of phenol takes me back to that simpler time of Chloraseptic and chicken soup
I am just old enough to remember that when I would go to older relatives' homes as a kid, they would have old containers of "carbolated" everything, like Vaseline.
We really went crazy with the phenol in previous decades and before quaternary ammonium disinfectants. Lysol had a very distinctive smell of phenol, and I gather it was one of the more common methods for people wishing to do harm to themselves.
Reading the old usage suggestions for Lysol is shocking if you look at them today. I will leave it as an exercise for the reader.
One issue with Phenol-based anesthetics, is that they can injure the very tissues that you are trying to numb.
If used carefully, this can be minimized, but the Benzocaine-based products are generally safer(but, indeed can be quite hazardous if abused).

Just wanna say that I’ve always had a hard time understanding chemistry, and I’m a nursing student. Referring to elements as “stable” vs “unstable” and calling the rings energy levels has been SUPER helpful
If you are referring to the rings in the molecules they are not really themselves "energy levels" they are more stable because the electrons are delocalised across the ring which resists reactions. If you see a ring (aromatic) you can mostly assume a higher stability.
its actually more correct to think of them as energy levels than rings. The concept of electrons orbiting the nucleus in rings is from a veryyyy outdated model from Bohr. Today we know that electrons exist in orbitals around the nucleus, and these orbitals are basically clouds where the electron probably is located. Different orbitals have different energy levels, so based off of the electron orbitals an atom has, we can predict how certains things will react together.
@@tsweetland7299 i think it's worth it to call it rings or orbitals during high school. learning the simpler explanations when i was 14~15 helped me conceptualise energy levels when i was 16~18. by uni, i agree that the terminology to switch and become more accurate.
with that said, i think this channel belongs to the first category, like an introductory level for casual viewers, and he's always meticulous about detail enough to explain his case but also keep it simple enough for randos who dont know about the medical field, hence why he went for the "ring" approach

Ok, on an atomic level the rings you talk about are "shells" or "orbitals", which are simply a 2d representation of where a specific amount of electrons can be at any point and time. Each shell farther from the nucleus, more electrons can exist in that shell.
On the molecular level, the rings refer to aromatics, which is a ring of carbon often having some alkenes in it. Benzene is a good example of this, it is made of 3 alkanes, and 3 alkenes.

I'm glad he made a full recovery, listening to the dire symptoms he was exhibiting I was beginning to worry that his story wouldn't have a happy ending.
It truly sucks to hear "a recovery" or "at autopsy" in these videos.
He once mentioned in one of the case that the one he presented includes the treatment possible for the case to be saved where full recovery could be assured. Meaning, the actual case didn't make it. So, there is a probability that other cases are also scripted to include treatments available, but in real life, some didn't make it due to lack of first aid, awareness nor quick response.
I wanted to thank you. Your videos are partially responisble for my doctor praising me last week. My main takeaway from your videos is that it is in my best interrest to give my doctor as much information as possible. My doctor started me on thyroid medication a few months ago and told me to take note of any changes I noticed. So I did, I sent her an excel sheet with a calender, everything I noticed was there with date and if relevant even time of day. I somewhat regularly take my bloodpressure at home just to know what my normal is, I bought a pulsoximeter when my dad (high risk) had covid to monitor him at home. When the doctor initially asked me to take my bloodpressure for 2 weeks every day she got a printed out calender with date, time and context on it for every messurement, sometimes multiples a day when I was feeling off.
In regards to this video: I'll have to watch it a few more times to completely understand it. Partially because english is my 3rd language, partially because I don't have the background to understand it easily. You do a great job with your videos!
Wow! Your English is really good for being your 3rd language. My 3rd is German and I can pretty much order falafel, say hello/goodbye and ask what time it is. I absolutely don’t know the German word for things like thyroid, blood pressure etc.

@Milla Cerne Hey there! The German word for thyroid gland is "Schilddrüse", the word for blood pressure is "Blutdruck". While the latter is exactly the same just in German words, thyroid gland in German means Schilddrüse (most direct translation: "shield gland"). I can't find any reason as to why it has shield in the name other than assume that the butterfly-like shape was originally also described as some form of shield-ish shape. I will update this once I've been able to ask the professor I'm working with :)

Fun Fact: The best way to fight spiciness is using buttermilk, ice-cream and yoghurt or lassi. Using water sometimes actually worsens the pain.
Worst thing to do is to numb the pain at tongue or throat level as it can cause high damage further in the digestive tract like ulcers since the amount of spicy food consumed might go unchecked.
I love spicy food. I can't imagine wanting to ingest an anesthetic just to eat spicier food than I could handle. It's the entire point of eating spicy food... To feel that burn!

I use this spray for its intended purpose. I recently purchased a different brand. It has a minty flavoring that if you spray too much, it becomes uncomfortably minty "hot," preventing one from using too much.
Kudos to whoever designed this formula. More than two sprays, you pay.
I always enjoyed biology in school but struggled in chem. I feel like I would have done so much better at it if I’d had your channel.
