You can either abbreviate it or use Microsoft Word to write it down then attach it to the document (as exhibit A), or add it to the additional document section on your application. I hope this helps 👍🏾

I wouldn’t say it has a specific “full meaning” but according to the Canadian government, the document is referred to as IMMIGRATION DOCUMENT NUMBER 5645, which is also known as “Family Information document” I hope that sheds some light to your question.
