Once you do the puzzle in the vault room you should get the corruption spreads II. Then go to the enclave in savathun's throne world and put on the intrinsic trait on Outbreak and it should unlock it.

I'm confused. Because I tried to do this and my doors are all locked. And yes I flipped the switched.
