Day 3 TRL laser - whew - vinegar soaks wow makes sense!! I’m gonna start today. Thank u for sharing very informative. Disappointed my dr wasn’t informative and expected me to ask all the questions. My procedure was very painful as numbing wore off ! So painful my dr could only go one swipe in a couple areas as I just couldn’t take it. Honestly only God got me through this !! Thank you again!

I am having the full deep Erbium laser procedure in two weeks. I have already had my pre-op. I was given one page of instructions. This video is so helpful. Thank you. This was not explained to me as fully as you just did. I have all my medications and the Aquaphor and the Cetaphil. I need to get the sunscreen. I will be returning to work (work from home) 12 days after my procedure. I am hoping I will be "presentable" by then -- you know, Zoom meetings. Thank you for the thorough discussion. Dr. Joe, your videos have been so helpful! On Day 3 of using Vaniply, my face and neck erupted with oil-filled boils and raw areas. My Drs office instructed me to discontinue the ointment, up the vinegar soaks, and I've taken Benadryl and Tylenol for pain and itch. They did not feel I was infected or need antibiotics, and said they've seen a few cases with severe reactions like mine. After the boils cleared, I tried the ointment again on Day 6, and my skin developed red bumps everywhere. I discontinued the ointment again, even though I'm very dry. It's now Day 8 and most of my peeling is over except the neck which had a light laser but is the slowest to heal and has very raw patches. Have you had patients with skin too sensitive for ointment?

The same thing happened where both the ointment and cleanser seemed to be too thick and locked in the heat, causing pain and blisters by day 3. At Day 5, the nurse recommended I switch to Cetaphil cleanser and Aquaphor ointment and I healed up quickly. I’m the rare case of skin not tolerating Vaniply. Thank you posting such helpful videos!

Great videos, part 1 and 2! Thank you for being so detailed and informative! That is very useful and important information but also very hard to find in most cases. I have been searching for after care for many days now and your videos were the first that answered all of my questions! So now I have a concern. My Derm, who is GREAT, does need to update his post-op instructions he admitted. We were emailing afterwards with a few questions and one was about washing-when 24 or 48 hours after- because one thing was said and another was typed out on the instructions. It was determined to do this after 48 hours and keep the aquaphor on. I was also told I could move to a Cetaphil/CeraVe type moisturizer by day 3 or 4 and I did. I emailed about that however because I felt like my face drank up the moisturizer and would be dry almost 15 minutes later. It was an exhausting process. I have moved to alternating (aquaphr and CeraVe Moisturizer) to give my pores a break but to also keep my face hydrated as much as I possibly can. I am also washing 2-3X a day. I am concerned though during the 1 and 3/4 of the day (this includes two nighttimes) that I had taken a total break from the aquaphor and the periods where my face was dry as a bone for times up to an hour at a time...way longer when sleeping---only because I would moisturize and then it would evaporate into my skin quickly and with 5 year old twins sometimes I just did not have that second to reapply 15-30 minutes later. Now I am using a sample that my Derm gave me, Resurfix. It is pretty much a nicer version of aquaphor, I guess? I see what you are saying with keeping it "greasy" for the first week. But do you think I may have jeopardized the optimal results with having those couple of days without "grease" a.k.a. aquaphor?

This is a Godsend for those that went to Groupon laser spas. 🙋♀️ I am day 5 crusting gone but underneath is burns on those places. Red burn skin then my healthy skin beside it- fraxel didn’t go there Are these burn scars or will they get better and maybe crust or peel again Yikes

Great question. I would say yes, in the winter months, but we do a lot of school teachers etc in the summer. If you live in Miami, there is not winter, so sometimes you have to do it when you have the time.

I shouldn’t be telling a doctor this but here goes. I am a recently retired cop and my last homicide was just something else to say the least. I had quit smoking for 20 years and I fell off the wagon after that incident. Good news is I weaned over to vaping recently and trying to wean off that too but my eyes can not unsee stuff! I don’t do much, I do have it down to a dull roar and I am wanting to get co2 done in March. I can absolutely do my very best to stop but if I can’t go cold turkey is this going to make my skin die off like it could with a face lift? Or just slow down the healing process? I count every time I vape and I have it down to an equivalent of about 5 traditional cigarettes a day. I’m doing my best to stop. Thank you sir for your time
Unless you have lung disease you can continue to smoke or vape that amount. No surgeon will ever tell you it is good and all surgeons should tell you to stop. Having said that we do operate on smokers unless they have pulmonary problems.
@@jniamtuiii thank God no lung issues yet I actually just had a lung X-ray. Thank you for your honest answer doc. My surgeon is reevaluating saying since my skin is clear with no issues maybe co2 is not a good option for me. My goal was to lift the cheeks and face as they are beginning to slightly fall south with normal aging. I was under the impression this could lift the skin and apparently it does not. We are going to do morph8 lower face (which I don’t think that is the best machinery out there) and ETwo under the eyes, cheek filler touch up and call it a day. Unless you can recommend a better option for skin rejuvenation. Lifting is a surgical procedure and I’m not quite ready for that.
@@jniamtuiii thank you for confirming. No wrinkles, well maybe some fine ones on this over 60 skin but I do use Retin A and have a very good dedicated skin care routine. You are in Richmond and I’m in Fairfax. Think I’m going to have to come see you in the future when I’m ready for something drastic, if ever. After 26 years of service out there I can honestly say waking up alive every day is a blessing! I just want to be in my best skin for as long as I can. Doc I have an interesting question and since you are all about skin care and co2 laser I thought you might know what is going on with my skin I am 12 weeks out from fractional co2, second time, after waiting six months in between. My skin was amazing until this 12 week period I have a circular red spot on my face and it kinda look like ring worm 🤦♀️. I sent my doc a pic and she thinks it might be fungus so I’m applying lotrimin cream today. It kinda looks like a chemical burn. Her esthetician seems to think it’s not fungal and to stop niacidimide and cereve pm and Trent for a week. Have any of your patients experienced this?
@ she didn’t culture told me to go to a dermatologist 🤦♀️ I didn’t, i listened to her esthetician and cut back on niaicidmide and Trent and it worked like a charm. Apparently it was irritation. Can you tell me is it normal at 3 months to see no crinkles under my eyes when smiling and other days they appear?? It’s driving me crazy. 🤪 one day look great next day not so good. I did ask if we can do another co2 after my six month wait period, it would be my third and she said no I have to wait a year She did do two six months apart. So why can’t I do another?
Dr. Joe, what's your opinion on Fraxel Laser for fine lines on upper lip , under eyes and brown spots on the skin in general (mostly on the upper cheek area). If you have any type of flesh colored permanent make up on your face will all lasers turn the area black? I had a scar revision 13 months ago after trying to camouflage the original scar with a light tattoo pigment (specifically made for use as permanent makeup...NOT regular tattoo ink). Well that does not work. The scar has lightened significantly but there is just a bit of an edge to it that makes it more noticeable. My dilemma is that I wanted to try some kind of laser perhaps fraxel but I believe I have some residual flesh colored pigment on one side of the scar and I'm afraid to do a laser procedure for fear of it turning the pigment black, hence giving me a worse problem....Please help......

I personally don't do much fractional resurfacing because I don't think it makes a big difference unless you are doing multiple treatments. Having said that it can be effective for superficial skin problems
@@jniamtuiii Seems like a big recovery for no results. My skin is good just need a little skin tightening. What would you recommend. Seems like the cheek lifting fillers need a little assistance 😂. No wrinkles on this old ass over 60 face just want more tight skin. Laxity is not enough for a upper face lift yet
I just had a co2 laser treatment with no after care from my doctor. I’m using all Cetaphil products, cleanser and moisturizer. I’m fair skinned and would like to use a timed moisturizer with SPF. CeraVe has one with 30 spf. Is it ok to use a tinted moisturizer spf with my fair skin, I’m 14 days post op.
You really can't shave for about 10 days, then I recommend a floating head (Norelco) type electric razor and shave in the shower after really wetting the facial hair. You can put shaving cream on 10 minutes before shower.
Hello Dr. Niamtu, I had CO2 fractional laser done on Dec 15. It is Day 17 and my skin is still red in some areas and has turned reddish brown in other areas. Is this normal? If so, how much longer do I expect to wait to get a skin with a uniform tone?
Thank you for your immediate reply. I don't know the settings used but what I know is that it was deep rather than superficial energy setting. Today, its Day 18. On Day 7, I was so happy with what the results may be; skin was pink and peeling and I thought the skin would blend in the next couple of days. On Day 18, skin is still red with patches of reddish brown.
This is scary....did you heal correct or did you stay with red brown skin I JUST HAD MINE DONE I am red and I am afraid. ..itt won't go away. Pink is not as red as I am...

@@pnevlud so many variables that I can't answer. Laser recovery depends on depth of treatment. My patients are alseep,so I go deep. I think deep laser is more demanding than profound. Your doc needs to recommend treatment based on the condition of your skin damage.

After my CO2 I sat in front of a fan till the heat went, did some cold washing, and the next 2 days I put loads of coconut oil on to keep the skin moisturised and went through a jar lol
Dear Dr Niamtu, I am treating 10 week old scarring that resulted from an aggressive fully ablative erbium and fractional co2 to the eye area. The skin is thickened from the eyelid crease to below the brow and has a very obvious demarcation line before it drops down to the normal skin level. Can this demarcation line be blended in now with a fractional laser or do I really have to wait a full year for it to maybe flatten on its own? I have recieved 3 lots of vbeam and cortisone / 5fu injections which have helped mostly the tiny scattered scars under the eye. But this thick area remains and I'd like to make it less obvious now if possible. Thank you

I have very fair skin. I had mine done a couple of weeks ago and I’m all peeled and have pink skin. It’s also extremely dry, especially under the eyes and I already know makeup will not look normal at this point. I’m using a growth factor moisturizer and vanicream moisturizer and face wash. I’m wondering if anyone else experiences this super dry skin and how long to expect this? I’m starting to get nervous about when my skin will start to feel soft again and look normal with makeup.

Had a fractional co2 laser done with tech. At my dermatologists office. Day three. Still miserable no shedding. Very pink. And under chin left side hurts like crazy. I have calcinosis caused from trauma of an accident. And scarring. Getting really scared and freaked out this red brown waffle face is a burn that will scar. Wish I knew how to up load a pic.

I have not seen that problem with Vaniply, but some people can't tolerate ointments, sounds like you are better now.

I recommend my patients stay out of direct sunlight for at least a month, then use a phyiscal sun block

I shouldn’t be telling a doctor this but here goes. I am a recently retired cop and my last homicide was just something else to say the least. I had quit smoking for 20 years and I fell off the wagon after that incident. Good news is I weaned over to vaping recently and trying to wean off that too but my eyes can not unsee stuff! I don’t do much, I do have it down to a dull roar and I am wanting to get co2 done in March. I can absolutely do my very best to stop but if I can’t go cold turkey is this going to make my skin die off like it could with a face lift? Or just slow down the healing process? I count every time I vape and I have it down to an equivalent of about 5 traditional cigarettes a day. I’m doing my best to stop. Thank you sir for your time
Unless you have lung disease you can continue to smoke or vape that amount. No surgeon will ever tell you it is good and all surgeons should tell you to stop. Having said that we do operate on smokers unless they have pulmonary problems.

My surgeon is reevaluating saying since my skin is clear with no issues maybe co2 is not a good option for me. My goal was to lift the cheeks and face as they are beginning to slightly fall south with normal aging. I was under the impression this could lift the skin and apparently it does not. We are going to do morph8 lower face (which I don’t think that is the best machinery out there) and ETwo under the eyes, cheek filler touch up and call it a day.
Unless you can recommend a better option for skin rejuvenation. Lifting is a surgical procedure and I’m not quite ready for that.

@@jniamtuiii thank you for confirming. No wrinkles, well maybe some fine ones on this over 60 skin but I do use Retin A and have a very good dedicated skin care routine.

After 26 years of service out there I can honestly say waking up alive every day is a blessing! I just want to be in my best skin for as long as I can. So any and all skin rejuvenation treatments is always welcome 🤗. And might I add your skin looks AMAZING 🤩

Doc I have an interesting question and since you are all about skin care and co2 laser I thought you might know what is going on with my skin
I am 12 weeks out from fractional co2, second time, after waiting six months in between. My skin was amazing until this 12 week period I have a circular red spot on my face and it kinda look like ring worm 🤦♀️. I sent my doc a pic and she thinks it might be fungus so I’m applying lotrimin cream today. It kinda looks like a chemical burn. Her esthetician seems to think it’s not fungal and to stop niacidimide and cereve pm and Trent for a week.
Have any of your patients experienced this?
Hard to say, on my patient I would culture first, maybe steroid cream, maybe biopsy, you doc should know what to do.
@ she didn’t culture told me to go to a dermatologist 🤦♀️ I didn’t, i listened to her esthetician and cut back on niaicidmide and Trent and it worked like a charm. Apparently it was irritation.
Can you tell me is it normal at 3 months to see no crinkles under my eyes when smiling and other days they appear?? It’s driving me crazy. 🤪 one day look great next day not so good.
I did ask if we can do another co2 after my six month wait period, it would be my third and she said no I have to wait a year She did do two six months apart. So why can’t I do another?
Dr. Joe, what's your opinion on Fraxel Laser for fine lines on upper lip , under eyes and brown spots on the skin in general (mostly on the upper cheek area). If you have any type of flesh colored permanent make up on your face will all lasers turn the area black? I had a scar revision 13 months ago after trying to camouflage the original scar with a light tattoo pigment (specifically made for use as permanent makeup...NOT regular tattoo ink). Well that does not work. The scar has lightened significantly but there is just a bit of an edge to it that makes it more noticeable. My dilemma is that I wanted to try some kind of laser perhaps fraxel but I believe I have some residual flesh colored pigment on one side of the scar and I'm afraid to do a laser procedure for fear of it turning the pigment black, hence giving me a worse problem....Please help......
I just had a CO2 Laser on Tuesday, I was told it was Ablative...and I think it was the smart skin machine? Maybe? IS this as ablative as your "old school" CO2's that you perform? Your patients pic look amazing!!!!!!
Not if it was fractional. The laser I describe in this video is fully ablative and full coverage, not fractional.
Dr. Joe, I'm having CO2RE on face and neck next week. Are your instructions and timetable the same or different for the CO2RE (Syneron CORE) laser? I will take an anti-viral and have 30 min. of numbing cream before the procedure. Thank you.
If you are doing awake, then it is a lighter treatment, but the instructions are the same. Good luck.

I personally don't do much fractional resurfacing because I don't think it makes a big difference unless you are doing multiple treatments. Having said that it can be effective for superficial skin problems

Seems like a big recovery for no results. My skin is good just need a little skin tightening. What would you recommend. Seems like the cheek lifting fillers need a little assistance 😂. No wrinkles on this old ass over 60 face just want more tight skin. Laxity is not enough for a upper face lift yet

I just had a co2 laser treatment with no after care from my doctor. I’m using all Cetaphil products, cleanser and moisturizer. I’m fair skinned and would like to use a timed moisturizer with SPF. CeraVe has one with 30 spf. Is it ok to use a tinted moisturizer spf with my fair skin, I’m 14 days post op.
I can't provide medical advice, but my patients can use such products if they are healed and not raw and don't react to the cream

You really can't shave for about 10 days, then I recommend a floating head (Norelco) type electric razor and shave in the shower after really wetting the facial hair. You can put shaving cream on 10 minutes before shower.
Hello Dr. Niamtu, I had CO2 fractional laser done on Dec 15. It is Day 17 and my skin is still red in some areas and has turned reddish brown in other areas. Is this normal? If so, how much longer do I expect to wait to get a skin with a uniform tone?
Hard to say exactly as I don't know what settings you had, but usually it takes 5-7 days before things begin to blend
Thank you for your immediate reply. I don't know the settings used but what I know is that it was deep rather than superficial energy setting. Today, its Day 18. On Day 7, I was so happy with what the results may be; skin was pink and peeling and I thought the skin would blend in the next couple of days. On Day 18, skin is still red with patches of reddish brown.

@@pnevlud so many variables that I can't answer. Laser recovery depends on depth of treatment. My patients are alseep,so I go deep. I think deep laser is more demanding than profound. Your doc needs to recommend treatment based on the condition of your skin damage.

After my CO2 I sat in front of a fan till the heat went, did some cold washing, and the next 2 days I put loads of coconut oil on to keep the skin moisturised and went through a jar lol

Dear Dr Niamtu, I am treating 10 week old scarring that resulted from an aggressive fully ablative erbium and fractional co2 to the eye area. The skin is thickened from the eyelid crease to below the brow and has a very obvious demarcation line before it drops down to the normal skin level. Can this demarcation line be blended in now with a fractional laser or do I really have to wait a full year for it to maybe flatten on its own? I have recieved 3 lots of vbeam and cortisone / 5fu injections which have helped mostly the tiny scattered scars under the eye. But this thick area remains and I'd like to make it less obvious now if possible. Thank you

I do all of my laser with sedation. Generally I laser the the seb hyperplasia as single units for smaller cases .
I did co2 fraxel laser ,fotona face laser 4 times, peeling,microneedling but nothing really changes...so frustrated😔 I'm in my mid 40s ,quite big pores and hyperpigmentation. I so wish you were here in Europe.

I have very fair skin. I had mine done a couple of weeks ago and I’m all peeled and have pink skin. It’s also extremely dry, especially under the eyes and I already know makeup will not look normal at this point. I’m using a growth factor moisturizer and vanicream moisturizer and face wash. I’m wondering if anyone else experiences this super dry skin and how long to expect this? I’m starting to get nervous about when my skin will start to feel soft again and look normal with makeup.
The laser is a true assult on skin and it can take months to hear. Keep in touch with you doc and their experience.

Had a fractional co2 laser done with tech. At my dermatologists office. Day three. Still miserable no shedding. Very pink. And under chin left side hurts like crazy. I have calcinosis caused from trauma of an accident. And scarring. Getting really scared and freaked out this red brown waffle face is a burn that will scar. Wish I knew how to up load a pic.
