That was a fake tongue. Silicon tongue that you just bite on and let it hang out of your mouth. That really is the actress in the closeups. The stunt performer was only used when she rams the wheelchair through the doors into the burning room.

Comparing the books is like those brain game picture puzzles except Satan appears if you find all the differences. Then again I've never found all the differences on my own in any of those puzzles sooooo.....
