Unfortunately, it is frustrating that the method for the double threading is in no way shown, so the beginning of this stitch is a mystery for those of us less familiar. Is there no knot? Are you using two loose ends or are they connected? etc. as it is all cropped out too :(

My thread gets twisted while I'm casting on and then makes a giant know so I can't pull the thread through. Is there anything to do about this?
