  • Longevity is at the top of everyone’s mind these days, and we can’t talk about longevity without mentioning movement, mobility, and safeguarding against falls and fractures. What lies at the heart of these endeavors? Our capacity to build and sustain lean muscle.
Today on The Dhru Purohit Podcast, Dhru sits down with Dr. Gabrielle Lyon to discuss why building and maintaining skeletal muscle is critical for preserving our healthspan, lifespan, and metabolic health. In this episode, Dr. Lyon explains the power of a muscle-centric approach to weight loss and shifting body composition.
She shares how prioritizing building muscle has helped countless patients reclaim their health and speaks on her strategy for consuming enough dietary protein, strength training, and meal planning and how to incorporate them into your life in a sustainable fashion.
Dr. Lyon is a Washington University fellowship-trained physician in nutritional science and geriatrics and is board-certified in family medicine. She is the founder of the Institute for Muscle-Centric Medicine and has a private practice that services leaders, innovators, mavericks, and executives in a number of fields.
In addition, Dr. Lyon also works closely with special-operations military personnel. Her goal is to change the paradigm of medicine from obesity-focused to muscle-centric and the way we think about health and medicine with high scientific integrity. Her new book, Forever Strong: A New, Science-Based Strategy for Aging Well, is set to be released on October 17, 2023.
In this episode, Dhru and Dr. Lyon dive into:
-What happens when women over 40 don’t prioritize protein and lifting weights
-The relationship between satiety, protein, and weight loss
-Muscle-centric medicine vs the conventional approach to weight loss
-Why muscle is critical when it comes to longevity
-What contributes to skeletal muscle loss
-How Dr. Lyon inspired Dhru to start resistance training and eating more protein
-The best way to measure lean muscle mass
-Optimal body fat percentage for women
-Measuring strength and establishing a baseline
-Minimal viable dose for resistance training
-Modifying our diet and lifestyle to match our environment
-Top reasons why people don’t prioritize protein
-How much protein and carbs to aim for per meal and day
-How Dr. Lyon meets her protein goals
-Top mistakes people make when increasing protein intake
    -Structuring meals around protein
    -Dr. Lyon’s take on carbohydrates

So glad to have found you, Dr, I'm a soon to be 80 yr old stroke survivor who went from a prognosis of having less than a week to live with a hemorrhagic bleed in the thalamic region of my brain & complete left-sided, head to foot paralysis, to 85% totally recovered in 12 wks @ age 62. I walked out in 3 months while looking past individuals who had come in with me around the same time. Many were depressed and had not even begun to rehab yet. Ironically my dad, uncle and kid brother all passed exactly at age 68 from cancer, stroke and Covid.
I used to be a distance runner, which we, having yrs of self discipline had cultivated into ways of learning to work past all sorts of injuries. That was going to be the plan, to move purposely into working the muscles, tendons and ligaments, with no time left to allot for feeling sorry for myself. Actually it was the nursing staff who seemed depressed. No one was talking to anyone.
The very first night in hospital I commandeered a wheelchair with one good arm and leg and tried to go to the bathroom by myself at 2 am in the morning. I had it made too, Got onto the john just fine, but that's when I began tilting like the Leaning Tower of Pizza, I had no feelings on the left that anything of humanity still existed there, and so I fell ignobly onto the floor, after which I crawled back and waited for the night nurse to help me get back into bed. She came in shortly, and panicked when she saw I was missing, then spotted me laying there at her feet. Suddenly we both burst out laughing, but then she left!
Not for long. She returned with a bunch of nurses and they all had a good time laughing at my idiocy, and me with my scrunched up face laughing along with them at myself. But here's the rub. What they also had done was given me back my humanity. In their eyes I was a real person again.

Bravo! Great interview. This is my first time hearing Dr. Lyon, but not my last. I'm 66, in phenomenal condition and health, and study the topic daily...always looking for something I missed or simply forgot about. I was a gymnast as a kid, and learned to read my body from an early age. It's very important for older people to assess where they are prior to each workout rather than mindlessly pushing weight (which is a good way to get injured).

Now, I'm 8 months carnivore and I don't use the rails. Up and down stairs without fear. After listening to several Dr Lyon interviews, I'm going to start exercising. Just need to find the best routine for me.

Wow! How inspired I am! I am 62 yrs old and the last couple yrs feel so doomed about aging. I don't look my age, or act my age lol (everyone tells me that).. But i feel absolutely miserable and can't get motivated to eat correctly, exercise or dust!
I have always yo-yoed with my weight my entire life. I am about 5'8 and carry my weight pretty well... Long story short I have lost and gained 80 lbs plus several times throughout my life... My point is, I DEFINITELY KNOW THE DIFFERENCE BETWEEN feeling good with exercising and being at a ideal weight...compared to very little exercise and feeling fat as hell! Especially the older I get!
Maybe I have been depressed and don't realize it, because since I have turned 60, gradually gained weight, I haven't seen a doc in probably 4 yrs even though I need too... I tend to ignore everything piling up around me, and feel as if I am self sabotaging..
I have had 3 total knee replacements. Beginning at a very young age compared to the average age. My first knee replacement was when I was 35 yrs old... Looking back at my life, I feel as if I have always had some sort of serious bone and joint issues, but no doctor ever got to the root of it..
Now days, even though people around much don't realize it, my bones and joints ache from my toes to my ears... I could start at each joint and describe some issue that I lay awake at night sometimes and wonder why this or that feels like it does..
During the bouts of me eating healthy, almost religiously, I have always ate from all the food groups, protein probably the least amount.. I always focused more on a balanced diet and portion control.. Along with exercise... I weighed and measured every single thing I put in my mouth when eating healthier and trying to lose weight.. 5 yrs ago I lost 80 lbs in 8 months by weighing, measuring and journeling everything I put in my mouth... Basically the weight watchers point system has ALWAYS WORKED FOR ME, and I have always felt it's very well balanced..
The reason why I have never tried a protein based way of life, is because I feel like foods grown from the ground have to be more important then eating animals.. I guess i am way more pro fruits and vegetables, eggs for protein, then I am basing my entire diet on protein alone.
Sorry this got so long.. But basically I probably could have simply said, maybe I should be more open minded to trying a protein based diet, because I definitely love meat and high protein foods.. One of the things that always made me veer off the WW based point system was the fact that I love meat and cheese and it was I was missing the most...
Maybe some of my bone and joint issues are because protein hasn't always been my biggest focus... Now days I eat horribly... Everything I eat is something easy to grab, along with a glass of wine to wash it down...
Some of this is because my husband has always been able to eat anything he wants and has worn the same size clothes for the entire 20 yrs of marriage.. I find it easier to eat what he cooks or to cook what he likes then it is for me prep my own food and eat totally different then him... He drinks while milk with every meal for God's sake!
The man has never worried about anything he eats, but he also doesn't eat tons of sweets or sits around over stuffing his face... Except when he does sit down to eat, he is always done before I even make my plate, and he usually has double the amount of food I eat... But i am the one miserable, fat and unhealty...😡.. I am really happy I ran across this podcast, I know I need to help myself, because noone else can... I to never leave the house without makeup, jewelry, and took pride in my appearance... Its probably been 4 yrs since I have put on a drop of makeup, jewelry, did my nails etc.... Depression, lack of protein, laziness??? I am not sure what my problem is..
But for some reason your comment is inspirational to me! Ty

I really like Ben Bikman’s approach - eat protein and fat, in nature they both come as a perfect package and both are essential (amino acids & fatty acids). We need fat also for our hormones, sustained energy, fat soluble vitamins, etc. Protein isn’t the only component to longevity and health.

I’m in my early 40s, & for most of my life have been a yo-yo dieter, who lost the same 5-10kgs over & over again, by focusing on keeping my calories & carbs low + doing cardio. I’m slim, but once I hit my forties, I got loose, baggy skin with terrible cellulite on my legs. Since aiming for 100g protein per day + adding 3 x 30min dumbbell TH-cam training sessions only 2 weeks ago, I’ve been astounded at the results! I’m eating more, feel less hungry, have lost my terrible sugar cravings - & have seen my body tighten & stomach flatten in ways I never thought possible! Plus I’m loving the resistance training (which I’ve never even tried before in my life). Highly recommend women over 40 giving the approach recommended in this video a try 😊

Hmm...DR. Attia says -yes' to Rapamycin?' I don't think so.
Never heard him making that recommendation, at least.
(Being that... there are no long term safety studies on it - for humans - as yet).
Although there was series of safety studies on Rapa in the 'Dog Aging Project.', and I think it passed this safety study with flying colors..
But I don't see where Dr. A would recommend taking something like this that has v. few human safety Studies. It just does not sound like something he would do, imo.
And you do know, that taking Rapa - for any reason, would have to be by prescription, right?
Additionally, you are aware that Rapa interferes with the mTOR pathway(?) - which logically (but not necessarily scientifically speaking (bc it's an unknown), means that it's interference with mTOR would probably interfere w/muscle protein synthesis - but again, no one really knows how that would play out.
      I just dont recall if
Dr. A is taking Rapamycin. I know he was taking prophylactic Metformin, but quit after some new studies arose.
By your stating that Dr. A says 'yes' to Rapamycin are you inferring this bc he's taking it(?)...and is he, do you know?
I can't recall whether I heard him say he IS taking it, in a recent interview I listened to when the Rapa subject arose. I want to say he is, but I may be 'remembering' that incorrectly.
I know that Dr. Brad Stanfield has a keen interest in Rapamycin as a bio-longevity tool, (do you follow him, btw?)
But he is holding out for more studies of its
safety/efficacy for healthy humans to use prophylactically, for longevity.
He actually has a fund going on Patreon to help him finance his own RCT's on humans, for Rapamycin.

The resistance training/ weight lifting is super important. A year ago I started eating more protein but it made me gain more weight and it wasn’t good for me. Excess protein also turns into fat so it’s important to be mindful of the quantity you’re consuming. The key thing is that both resistance training and eating the right amount of protein has to be done at the same time or it won’t work as intended.

I'm a massage therapist of 15 years and a former aerial acrobat with years of strength, laxity, injuries and working with and training many.....I have been stressing strength for years and always give clients homework. Now at 51 I'm PASSIONATE about strength training esp for older women for hormones, insulin, energy conservation, time efficiency, bone health, mobility, and injury prevention esp for the changes women esp in menopausal years
I started eating more protein and for at least 8 years doing more strength exercises (but did strength in general for the last 30 years). Eating more protein hands down helps me retain muscle.
At 51 people think i am 30 ish and can still do pull ups and splits. I cant run due to inuries but i find other things i can do.
In some ways i feel stronger now

Is it wrong to think that it would be wise - during these conversations - that all carbs aren't lumped together? I've come to understand there is a huge nutritional difference between a potato and a piece of cake. BOTH carbs.....many that are new to these discussions only think of bread, when you say carbs.

I remember having a check up and the doc told me my cholesterol level was so low, not good for a young lady and he wanted me to go home and eat a package of hotdogs! Great interview from you guys! Thank you! I can’t wait for the book which I pre ordered. I feel a little upset sometimes about the misinformation about protein ( and exercise) I’ve read over the years as I might have had a better perimenopause and actually throughout my whole life as a female especially those tricky adolescent years. But glad I’m doing it now and the strength training feels amazing and truly like a protective armor when I had been getting ever more fragile physically and mentally and got to thinking it was inevitable.
I wonder if Dr. Lyons will also be able to provide some kind of course or possibility for nurses in the future as well as the one mentioned for physicians? (I’m a former geriatric nurse and hoping to return to nursing)

At 62 she got tired of feeling terrible and decided to eat more protein by going back to meat and started cross fit and got into the best shape of her life. Im mostly vegan and am strong, the point is she ate more protein and strength trained and gained muscle at a later age!

regarding satiety, I believe feeling fed/full/nourished with a basic food (and sense of tasty flavor) comes from mineral density. We eat foods from depleted soils, and food is less nourishing. Whether animal or vegetable, the richer the soil tilth the better tasting and more filling the food is. Animal protein is more likely to have the minerals needed for life, therefore its nourishing. I bought half a grass fed cow from my farm community and the meat was good, but then I ate grassfed beef from a farmers cooperative on Maui and it blew my mind how rich and tasty it was. same diet, different soil. I doubt breed has anything to do with the flavor. also experience the same thing with fresh grown veggies. Maui and Kona coast on Big Island have AMAZING soil and delicious island grown foods.

I hate that every diet guru just talks from their lense and don’t take in the account all the advice that we receive. I just listened to the podcast how we should be at more fiber. Explain to me how eating so much protein will not shift the microbiome towards more inflammatory state. My OD in the ‘90’s got me and my husband into a low carb diet. After he retired we got away from that. He wanted us to get 75 g protein per day. That seemed insurmountable, but having lamb chops for breakfast helped. I’m 5’2”, 173 lb, age 73. And I do work out. I’m a singer and need some bulk to make a bigger sound. So I’d rather not go down below 140 lb. I cannot possibleat 140 g protein. I cannot possibly eat six eggs at one sitting. Your plan is not even possible for me. I just started a modified carnivore diet, but unlike other women on carnivore, I can’t eat more than 2 hamburger patties at one sitting (plus a little bacon). My lifestyle is active for a 73-year old; I would never want a busy life like yours. And I will NEVER do HIIT. Or walk. In Chicago, it’s too hot, too cold, or too windy, which would give me a sore throat.
I work out with a really special trainer twice a week, with weights. On the same days I take a free class set up for seniors, either yoga, strength or Pilates. On alternate days I have two two-hour voice lessons-that’s my cardio. I move at my own pace and don’t pressure myself. I’m on bp med (Carvedilol) and I have a lot of stress in my life because my husband (76) still works at a very high-pressure job. His stress devolves on me. I have a cortisol tummy. I sleep well (with youtube videos).
I think the low-carb carnivore inspired eating plan will work for me; in fact, all cravings for banana-nut bread, muffins and such, are gone as long as I get my next meal when I need it.
I just think the quantities you recommend are more than I could handle. And I’ve been doing this for ten days. I think you have to realize that 70+ individuals generally have to tailor-make this diet to their own needs. And I can’t understand why anyone would eat rice or pizza!
I’m glad people like you are there, but recommendations cannot be one size fits all.

“People often think of aging as a linear thing; there’s a slow steady decline. In reality that is not what happens. And this is some of the foundational work from Doug Patten Jones where he really highlighted this idea of a catabolic crisis. A fall, a hip break creates this catabolic crisis. The subsequent effect on muscle tissue and muscle strength creates a downward cascade of health and wellness; there’s changes in blood sugar regulation; typically changes in fatty acid oxidation; changes in mitochondria health with loss of skeletal muscle… And then when they are also sedentary… and then when they’re hospitalized and contract an illness, people get pneumonia, it is just a downward spiral. And so it is not that treating the hip injury isn’t important, it’s how do we PREVENT that? How do we prevent injury? And that becomes a really critical component.” - Dr. Gabrielle Lyon 42:30
---- wow, this is so important! Let's help our elderly (and ourselves) live healthier longer lives. P.S. Feel free to correct my grammar! lol
