Just as it would hurt you to walk on a solid metal sole nailed to your feet 24/7. No horse needs shoes nowadays. Its a very archaic practice that is still done mainly because of tradition.
No it doesn't hurt them. There are no nerves in the hooves so when the shoe is either nailed on or put in hot, it won't hurt them. Additionally it's not for traditional purposes, it's a far healthier option than barefoot, because it can prevent abnormal wear to the hoof and injury.
